    Abstract:

    Objective Inspired by the simvastatin' s brain protective effect,we seek to investigate its protective effect and mechanism of the spinal cord ischemia/reperfusion injury.Methods 24 healthy adult tibet mini-pigs of either sex,weighted 20-30 kg,were randomly divided into 3 groups,8 pigs in each group.Group A,Sham operation group.Group B,Control group,i.e.the simple Ischemia/Reperfusion group.Group C,simvastatin experimental group.Creating the models of spinal cord Ischemia/Reperfusion by thoracic aorta occlusion (70 min).Isolating the accessory Hemiazygos Vein and ligating the distal end.1000 ml lactated ringer solution were pumped into the Vein and the rate is 860 ml/h and pumping time 70 minutes.Proximal blood pressure,distal blood pressure,heart rate and the nasopharyngeal temperature were recorded every 10 minutes during the operation.After the operation,pigs were kept free access to food and water.Pigs of the simvastatin experimental group were fed with simvastatin tablets 80 mg/day,orally for 30 days.Recording the motor function score at 6,24,48 h after reperfusion (see Taylor motor function scoring system).All 8 pigs of each group were killed at 30 days after reperfusion and take the L2-L5 and sacral spinal cord as quickly as possible to observe the express changes of IL-1、BDNF and GDNF applying immunochemistry,electron microscope and Elisa method.Results In the simvastatin experimental group,the 24,48 h motor function scores were significantly higher than that in the control group (P < 0.05).Compared with the control group,higher express changes of BDNF and GDNF and lower express of IL-1 can be found in the experimental group.Conclusion Simvastatin can significantly improve the neurological function recovery of spinal cord ischemia reperfusion injury.And it is related to higher express of BDNF GDNF and lower express of IL-1.   • Protective effect of retrograde venous perfusion of cryogenic liquid via accessory Hemiazygos Vein and treated with resveratrol on spinal cord injury in swine

    Abstract:

    OBJECTIVES To observe the protective effect of retrograde venous perfusion of cryogenic liquid via accessory Hemiazygos Vein and treated with resveratrol on spinal cord injury and evaluate the expression changes of microtubule-associated protein 2 (MAP-2) after spinal cord ischemia reperfusion injury (SCII) in swine. METHODS Eighteen swine were divided into 3 groups: group I/R (n = 6, operation group), group CL (n = 6, retrograde venous perfusion of cryogenic liquid), group CL+Res (n = 6, retrograde venous perfusion of cryogenic liquid and treated with resveratrol after ischemia). In the group I/R, the aorta was clamped for 60 minutes and then removed. In the group CL and CL+Res, 9 g/L cold (4 °C) saline solution (perfusion rate, 16.65 ml/min) was infused into the accessory Hemiazygos Vein during ischemia.In the group CL+Res, the swine were treated with resveratrol (10 mg/kg) after spinal cord ischemia. Arterial pressure, blood gas analysis and the spinal canal and nasopharyngeal temperature changes were monitored during the surgery. Nervous function were assessed at 6 hours, 1, 2 days, 1, 2, 4 weeks and MAP-2 expression were detected at 4 weeks after reperfusion by using Western blot analysis in spinal cord tissue. RESULTS After operation 18 swine were all survival. Behavioral scores of all groups decreased until 1 week after reperfusion and increased as time went by. The scores of group CL and CL+Res were higher than group I/R (F = 8.612, 17.276 and 11.985, P = 0.035,0.011 and 0.023) at 6 hours, 1, 2 days, group CL+Res were higher than group CL(P = 0.021) at 1 days after surgery. After descending aortic cross clamping, the spinal canal and nasopharyngeal temperature were obviously decreased in all groups and dropped to the lowest at 60 minutes after ischemia and 20 minutes after reperfusion in group I/R and the other groups respectively(F = 23.187-55.029, P < 0.01).In group CL(0.54 ± 0.26) and CL+Res (0.66 ± 0.31), the MAP-2 expression were higher than group I/R(0.37 ± 0.18) (F = 9.381, P = 0.037) , and the level in group CL+Res was higher than in group CL (P = 0.021) . CONCLUSION Retrograde venous perfusion of cryogenic liquid via accessory Hemiazygos Vein and treated with resveratrol can relieve the ischemia-induced spinal cord injury in swine.

    Abstract:

    Abstract Objective The superior vena cava (SVC) obstruction by malignant diseases is either by direct invasion and compression or by tumour thrombus of the SVC. Whatever is its cause, obstruction of the SVC causes elevated pressure in the Veins draining into the SVC and increased or reversed blood flow through collateral vessels. Severity of the syndrome depends on the collateral vascular system development. Therefore, imaging of the collateral Veins with variable location and connection is important in determining the extension and management of the disease. Our aims are to describe collateral vessels of the superior vena cava syndrome (SVCS) related with the malignant diseases and to assess the ability of multi-detector row CT with multiplanar and 3D volume rendering techniques in determining and describing collateral circulations. Materials and methods We present CT angiography findings of seven patients with small cell carcinoma of the lung ( n  = 2), squamous cell carcinoma of the lung ( n  = 3), Hodgkin disease of the thorax ( n  = 1), and squamous cell carcinoma of the oesophagus ( n  = 1). The patients received contrast-enhanced CT scans of the chest and abdomen on a multi-detector row CT during breath holding at suspended inspiration. Results CT images revealed the cause and level of the SVC obstruction in all patients with axial and multiplanar reconstructed images. The SVC showed total obstruction in five patients and partial obstruction in two patients. The most common experienced collateral vessels were azygos Vein (6), intercostal Veins (6), mediastinal Veins (6), paravertebral Veins (5), Hemiazygos Vein (5), thoracoepigastric Vein (5), internal mammary Vein (5), thoracoacromioclavicular venous plexus (5), and anterior chest wall Veins (5). While one case showed the portal-systemic shunt, V. cordis media and sinus coronarius with phrenic Veins were enlarged in two cases, and the left adrenal Vein was enlarged in a patient. In one case, the azygos Vein with reversed blood flow was drained into both inferior vena cava and Hemiazygos Vein with the left renal Vein. Conclusion Multi-detector row CT with multiplanar and 3D imaging is an effective tool in evaluation of the SVCS and has a greater advantage than the other imaging techniques. 3D volume rendering is a useful technique in determining and describing collateral circulations in addition to the primary disease process.

Methods This article analyzed retrospectively and followed up the effect of transcatheter occlusion of the azygos/Hemiazygos Vein in patients after bidirectional Glenn procedure hospitalized during February 2012 to September 2017. Results Transcatheter occlusions of azygos Veins were performed in 21 patients and left superior vena cava(LSVC) was occluded in one of patients meanwhile. Azygos Veins and Hemiazygos Veins were both occluded in two patients. One patient had the Hemiazygos Vein occluded. Aortic-to-pulmonary collaterals were occluded in 6 patients in the meantime. Atrial septal defect occluders were applied in the transcatheter occlusions of azygos Veins, Hemiazygos Veins and LSVC, except one patient with an azygos Vein which was 5.5 mm in diameter. Saturation was increased from 0.78 to 0.85 through occlusion(P 0.05). No complications associated with interventional procedures occurred. There was no significant decrease in blood oxygen saturation. Facial and upper limb edema, varicose Veins in the chest and abdominal wall, pleural effusion and peritoneal effusion did not happen during follow up of 2 years. Two patients underwent TCPC operation one year and five years after interventional occlusion separately. Suitable indications meant that the pressure of pulmonary artery and superior vena cava was less than 20 mmHg in the basal state, and the pressure of superior vena cava was also less than 20 mmHg before the release of the occluder. Occlusions may increase the oxygen saturation of patients, improve the quality of life, and even promote the development of pulmonary vascular bed, so as to achieve the indications of TCPC operation in some patients.
