Annals of the Rheumatic Diseases, 2015Co-Authors: I Klymas, L KhimionAbstract:Background Patients with rheumatoid arthritis (RA) often develop wide spectrum of co-morbidities and metabolic changes, particularly involving Hepatobiliary System (HBS) which can influence the safety efficacy of the RA pharmacological treatment. Objectives To determine the metabolic syndrome (MC) prevalence according to WHO (1998) and IDF (2005) classifications and its influence on HBS damage development in RA patients. Methods We investigated MS criteria and state of the HBS in 126 RA patients (102 women and 24 men, average age 43,82±2,8 years) and 30 subjects (control group) without autoimmune and inflammatory diseases (25 women and 5 men, average age 42,4±2,6 years). RA was diagnosed according to ARA criteria (1987); patients with hepatitis, alcohol abuse, >55 years old were not included. MS was diagnosed according to WHO (1998) and IDF (2005) criteria; HBS was investigated by ultrasound technique and biochemistry tests. Results Three criteria of MS according to WHO classification were diagnosed in 28,75% RA patients and 10,0% of control group (p Conclusions Both WHO (1998) and IDF (2005) classification MS criteria have the same diagnostic value for the RA patients. Systematic review of the role of high intensity focused ultrasound hifu in treating malignant lesions of the Hepatobiliary System

The aim of this review was to assess the efficacy, safety and optimal technical parameters of HIFU to treat malignant lesions of the Hepatobiliary System. Methods A Systematic search of the English literature was performed until March 2020, interrogating Pubmed, Embase and Cochrane Library databases. The following key-words were input in various combinations: ‘HIFU’, ‘High intensity focussed ultrasound’, ‘Hepatobiliary’, ‘Liver’, ‘Cancer’ and ‘Carcinoma’. Extracted content included: Application type, Exposure parameters, Patient demographics, and Treatment outcomes. Results Twenty-four articles reported on the clinical use of HIFU in 940 individuals to treat malignant liver lesions. Twenty-one studies detailed the use of HIFU to treat hepatocellular carcinoma only. Mean tumour size was 5.1 cm. Across all studies, HIFU resulted in complete tumour ablation in 55% of patients. Data on technical parameters and the procedural structure was very heterogeneous. Ten studies (n = 537 (57%) patients) described the use of HIFU alongside other modalities including TACE, RFA and PEI; 66% of which resulted in complete tumour ablation. Most common complications were skin burns (15%), local pain (5%) and fever (2%). Conclusion HIFU has demonstrated benefit as a treatment modality for malignant lesions of the Hepatobiliary System. Combining HIFU with other ablative therapies, particularly TACE, increases the efficacy without increasing complications. Future human clinical studies are required to determine the optimal treatment parameters, better define outcomes and explore the risks and benefits of combination therapies.

We believe that fusion images are extremely valuable for applications such as preoperative planning for hepatic and bile duct resection.

Each modality has individual strengths and limitations, which usually vary depending on the specific clinical situation. This review discusses several specific common clinical situations where imaging of the liver and biliary System is necessary and describes the various imaging options. Space-occupying liver lesions are discussed, and particular attention is paid to the assessment of liver metastasis, hepatoma, and incidentally discovered liver lesions such as hemangioma, adenoma, and focal nodular hyperplasia. The value of ultrasound, computed tomography, magnetic resonance imaging, and scintigraphic techniques in this patient population is described. Isolated sulfur colloid hepatic scintigraphy is not of great value in the evaluation of these patients. Therefore, this review describes in some detail the value of physiological liver scintigraphy, including gallium and iminodiacetic acid (IDA) scanning as well as dynamic flow imaging of the liver such as hepatic artery perfusion scintigraphy and tagged red cell scintigraphy. Imaging of the biliary tree also is described. The roles of ultrasound and scintigraphy are compared and contrasted as related to the diagnosis of acute cholecystitis, common duct obstruction, and postoperative complications.
