    Abstract:

    Although Internet of Things (IoT) brings significant advantages over traditional communication technologies for smart grid and smart Home applications, these implementations are still very rare. Relying on a comprehensive literature review, this paper aims to contribute towards narrowing the gap between the existing state-of-the-art smart Home applications and the prospect of their integration into an IoT enabled environment. We propose a holistic framework which incorporates different components from IoT architectures/frameworks proposed in the literature, in order to efficiently integrate smart Home objects in a cloud-centric IoT based solution. We identify a smart Home Management model for the proposed framework and the main tasks that should be performed at each level. We additionally discuss practical design challenges with emphasis on data processing, as well as smart Home communication protocols and their interoperability. We believe that the holistic framework ascertained in this paper can be used as a solid base for the future developers of Internet of Things based smart Home solutions.

    Article-at-a-Glance Background Asthma exacerbation is one of the most common causes for pediatric hospitalization. One of the three Joint Commission quality measures—which has proven the most challenging—addresses the provision of a Home Management plan of care (HMPC) for discharge of pediatric inpatients with a primary diagnosis of asthma. A user-friendly electronic medical record (EMR)–generated HMPC was developed and implemented at Lucile Packard Children's Hospital (LPCH) Palo Alto, California, an HPMC needed to be completed before entry of an inpatient discharge order. Methods A cohort study using historical controls was conducted in 2010–2011. Patients were eligible to receive an HMPC if they were between the ages of 2 and 17 years old at discharge, had a length of stay Results Compliance with the HMPC increased from 65.3% for the 39 months (April 1, 2007–June 30, 2010) before integration of the HMPC into EMR to 93.7% for the 18 months after integration (July 1, 2010, through December 31, 2011); p Conclusion Lessons learned at LPCH included the need for a continuous surveillance and improvement model, which resulted in several iterations of the HMPC; the importance of soliciting user input, which resulted in improvements in work flow; and consistent support from the quality Management and information technology departments, which are crucial to eliminating barriers and facilitating improvement.

  • Home Management of febrile convulsion in an african population a comparison of urban and rural mothers knowledge attitude and practice

    Abstract:

    To determine the knowledge, attitude and practice (KAP) of Home Management of febrile convulsion (FC), by mothers in the community, focus group discussions (FGD) were conducted in two communities, Uselu (urban) and Evbuomodu village (rural), both in Edo State, Southern Nigeria. The study was conducted between December 2000 and February 2001. Our findings show that 71% of urban mothers compared to 25% of rural mothers attributed the cause of FC to fever (v 2 =24.17: p<0.001). Seventy-five percent of mothers from rural community and 28.6% of urban mothers attributed the cause to witchcraft and/or evil spirits. Twenty-five percent of rural mothers also attributed abnormality of the spleen as a cause of FC. All the mothers, both urban and rural, were not directly involved in the Management of the convulsive episode due to panic and confusion. Ninety-two percent of urban and all the rural mothers permitted the use of traditional medicine while 7.1% of urban mothers employed prayers during convulsion. Twenty percent of urban and twenty-two percent of rural mothers use urine (human and or cow’s) for treating FC at Home. Other Home remedies include kerosene, fuel and crude oil.Mass enlightenment campaign for the community, especially the rural, against use of harmful traditional remedies to treat FC at Home is strongly advised.     Background Community distribution of anti-malarials and antibiotics has been recommended as a strategy to reduce the under-five mortality due to febrile illnesses in sub-Saharan Africa. However, drugs distributed in these interventions have been considered weak by some caretakers and utilization of community medicine distributors has been low. The aim of the study was to explore caretakers' use of drugs, perceptions of drug efficacy and preferred providers for febrile children in order to make suggestions for community Management of pneumonia and malaria.

    Abstract:

    Malaria and pneumonia are the leading causes of child death in Sub-Saharan Africa (SSA). Integrated Management of childhood illness (IMCI) at health facilities is presumptive: fever for malaria, and cough/difficult breathing with fast breathing for pneumonia. Of 3671 Ugandan under-fives at 14 health centres, 30% had symptoms compatible both with malaria and pneumonia, necessitating dual treatment. Of 2944 "malaria" cases, 37% also had "pneumonia". The Global Fund and Roll Back Malaria are now supporting Home Management of malaria strategies across SSA. To adequately treat the sick child, these community strategies need to address the malaria-pneumonia symptom overlap and manage both conditions.
