BMJ Open, 2017Co-Authors: D Romero, K FlandrickAbstract:New York City (NYC) has experienced various changes in Hospital providers over the last several years. In addition, the federal Patient Protection and Affordable Care Act of 2010 (“Obamacare”) has led to changes in the numbers of people without health insurance. This presentation will discuss related aspects of two qualitative research projects on provision of safety-net Hospital services – one of a borough in NYC with several private Hospitals at risk of closure due to financial difficulties and the other of the NYC public Hospital system reduced fee-for-service Options Program for those ineligible for health insurance (predominantly undocumented immigrants). These two projects involved collaboration across sectors (municipal government; health department; community-based non-profit service providers; public Hospital system; professional trade organizations; advocacy organizations; academia). Methods of data collection included focus groups, key-informant interviews, and web-based community survey, with varying levels of collaboration in the analytic process. The discussion focuses on how engagement with multiple partners in the research process both provides (1) valuable access and insights, and how a Collective Impact Framework may function as a useful implementation tool, as well as (2) challenges to rigorous study design, validity of findings, and appropriate dissemination.

cluster status migrainosus with a weekly periodicity responsive to high flow oxygen a case report

We believe this is the first documented case of “cluster-status migrainosus”.CaseA 54 year-old female Hospital Administrator presented with headaches with a unique periodicity and semiology that were acutely responsive to high-flow oxygen. She experienced cluster headache attacks every Thursday morning at 3–4 am, which would evolve into status migrainosus lasting through Sunday evening. These attacks were preceded by prodromal depressed mood changes and fluid retention, and later followed by postdromal euphoria and auto-diuresis. These attacks initially occurred every other week and progressed to weekly attacks for 1.5 years. These headaches did not respond to trials of propanolol, sodium valproate, topiramate, amitriptyline, gabapentin, and carbamazepine for preventive treatment or to oral sumatriptan and butalbital-acetaminophen-caffein...

The effective use of financial variables and subsequently performance evaluation of Hospitals would help to facilitate the continuation of activities which provides the ground for the economic administration of Hospital industry. The purpose of this study was to evaluate the viewpoints of executive Administrators of Hospitals in Iran to evaluate the feasibility of using financial variables of health in Hospitals. Material and Methods: The present study was an applied research which is conducted using AHP (Analytical Hierarchy Process) technique. After extracting the evaluation variables, the financial performance of Hospitals was evaluated using the questionnaire which was confirmed by Cronbach’s Alpha equal to 0.85. Moreover, the group of experts in this study included 62 Iranian Hospital Administrators. Results: Global indices for the economic evaluation of Hospitals were identified which were categorized into 9 domains/factors using AHP (Analytical Hierarchy Process); furthermore, they were prioritized regarding the importance and feasibility in Hospitals. Conclusion: Utilizing financial variables in evaluating the economic performance of Hospitals can determine their economic growth. Moreover, analysis of the current status of Hospitals can be used to make strategic decisions and provides a basis for comparisons between Hospitals and their rankings. [GMJ. 2016;5(1):25-30]
