    Abstract:

    Hospital Information System (HIS) is important in the healthcare industry as it supports a wide range of highly specialized health-care tasks and services. In Malaysia, HIS diffusion is still in its early stage and there is slow rate of adoption among large, medium, and small public Hospitals. To investigate the factors influencing the adoption of HIS in the Hospitals' work processes, this study proposed the initial theoretical framework based on the combined Technology Organization Environment (TOE), institutional theory, and Human Organization Technology (HOT) fit model. A nationwide survey was conducted on small, medium, and large public Hospitals involving both HIS adopters and non-adopters in Malaysia to verify the validity of the initial integrated framework. We find that relative advantage, compatibility, security concern, Hospital size, mimetic pressure-competitors, vendor support, perceived technical competence of IS staff, and employees IS knowledge as the influential factors on HIS adoption. In addition to the only moderator of this study (occupational level), any significant moderating effects on the Technology-Organization-Environment-Human dimensional factors and the adoption of HIS by Malaysian public Hospitals was not found. Hence, these results indicated that the developed integrated framework provides an effective prediction of the HIS adoption by Malaysian public Hospitals. An adoption framework for Malaysian public Hospitals was developed.The model was developed based on TOE, institutional theory, and HOT fit.A nationwide survey was conducted on small, medium, and large public Hospitals.Both HIS adopters and non-adopters in Malaysia participated in this research survey.An effective prediction of the HIS adoption was achieved by the framework.

  • Statistical Estimation of Software Quality in Hospital Information System

    Abstract:

    Clinical environment is very complex, and flexible and adaptive service improvement is crucial in maintaining quality of medical care. Thus, incremental software development in Hospital Information System and its evaluation are important. This paper introduces a statistical estimation method of an embedded software in which service logs are used to measure the differences between responsive time before and after a new interface has been introduced. The empirical results show that statistical methods are useful to evaluate the System performance in a real clinical environment.

  • Construction of clinical pathway from histories of clinical actions in Hospital Information System

    Abstract:

    This paper proposes a method which induces a clinical pathway by using sample and attribute clustering of the histories of nursing orders stored in Hospital Information System. The method consists of the following five steps: first, frequencies of nursing orders are extracted from Hospital Information System. Second, orders are classified into several groups by using sample clustering. Then, attributes clustering is applied to the data for feature selection. Fourth, the method compares between generated functions for sample and attribute clustering which relate the number of clusters and calculated similarities. Fifth, if attribute clustering gives better performance with respect to the function, the dataset is decomposed into subtables by using the grouping of attribute clustering. Then, the first step will be repeated in a recursive way. After the grouping results are stable, a new pathway will be constructed from all the induced results. The method was applied to datasets of a disease extracted from a Hospital Information System. The results show that the proposed method is useful for construction of a clinical pathway.

    Abstract:

    Hospital Information has become an intelligence infrastructure in large-scale Hospitals, and incremental software development is important for improving service quality. This paper introduces a statistical estimation of a service log to measure the differences between responsive time before and after a new interface has been introduced. The results show that even simple statistical methods can compare System performances using the log recorded in the Hospital Information System.

    Abstract:

    Schedule management of Hospitalization is important to maintain or improve the quality of medical care. Application of a clinical pathway has been proposed as one of the important solutions for the management. This research proposed an data-oriented maintenance and construction of clinical pathways by using data on histories of nursing orders stored in Hospital Information System.. The method was evaluated on data extracted from a Hospital Information System. The results show that the reuse of stored data will give a powerful tool for management of nursing schedule and lead to improvement of Hospital services.

  • Granularity-based temporal data mining in Hospital Information System

    Abstract:

    This paper proposes granularity-based temporal data mining method which constructs clinical process conducted by nurses. The methods consist of three process. First, data on counting sum of executed orders are extracted from Hospital informaton System with a given temporal granularity. Then, similarity-based methods, such as clustering and multidimensional scaling (MDS) are applied to the data and the labels for grouping are obtained. By using the labels, rule induction is applied, and classification power of each attribute is estimated. The attributes are sorted by an index of classification power, the original dataset is decomposed into subtables. Clustering, rule induction and table decomposition methods are applied to the subtables in a recursive way. The method was applied to datasets stored in Hospital Information System stored in 10 years. The results show that the reuse of stored data will give a powerful tool for construction of clinical process, which can be viewed as data-oriented management of nursing schedule.
