It is acknowledged that Big Data represents a large volume of data, and this brings a matter of ways on how to manage storage capacity as data continues to grow exponentially, that may drive to storage technologies to uphold its pace to the predicament. In addition, due to an incomparably large amount of data received, processed and transferred, it also affects the ability of its data transmission speed. In other hand, the employability condition of information technology (IT) professionals, which is desperately essential in the rapid change of the technological world. This challenge defines the aspects, without paying necessarily much attention to the lack of information technology staff and services, but is more catered towards the capability of one’s uphold and acknowledgement of new information upon new inventions of computing and telecommunications devices in the market. Here, the empowerment of information technology comes from the aspect of how it helps as a platform for these fields to operate successfully and embrace positive changes in delivering efficient as well as effective work, also benefits business companies in decision making, marketing and business growth, and resource management, and globalization. it is impossible to attain long term business success without leveraging the benefits of IT in this digital age.

    Abstract:

    Objective Electronic medical records (EMRs) are being increasingly utilized to conduct clinical and epidemiologic research in numerous fields. To monitor and improve care of HIV-infected patients in Washington, DC, one of the most severely affected urban areas in the United States, we developed a city-wide database across 13 clinical sites using electronic data abstraction and manual data entry from EMRs. Materials and Methods To develop this unique longitudinal cohort, a web-based electronic data capture system (Discovere®) was used. An Agile software development methodology was implemented across multiple EMR platforms. Clinical informatics staff worked with information technology specialists from each site to abstract data electronically from each respective site’s EMR through an extract, transform, and load process. Results Since enrollment began in 2011, more than 7000 patients have been enrolled, with longitudinal clinical data available on all patients. Data sets are produced for scientific analyses on a quarterly basis, and benchmarking reports are generated semi-annually enabling each site to compare their participants’ clinical status, treatments, and outcomes to the aggregated summaries from all other sites. Discussion Numerous technical challenges were identified and innovative solutions developed to ensure the successful implementation of the DC Cohort. Central to the success of this project was the broad collaboration established between government, academia, clinics, community, information technology staff, and the patients themselves. Conclusions Our experiences may have practical implications for researchers who seek to merge data from diverse clinical databases, and are applicable to the study of health-related issues beyond HIV.

Most independent journals face the additional obstacle of not having the financial resources for hiring information technology staff. I am going to describe the kind of decisions we had to make to set up a comprehensive Website for the Houston Journal of Mathematics (HJM). The result is a publishing format which may not have all the characteristics of a primarily electronic journal, but it offers more than a paper journal with free abstracts, and file access to its subscribers. Like some other journals, HJM decided to make electronic editions freely available for registered subscribers of the print edition. Nevertheless, one still needs a License Agreement Form. This is a legal document that defines the Scope of the License and which includes a Copyright clause. In its present form, our license excludes electronic files from Inter Library Loans. I will try to explain our opinion on this controversial topic. All journals, whether commercial or independent, face the problem of archival quality of current file formats, primarily of the.pdf format. But there is also the related problem of shared legal responsibilities of possible file maintenance. The paper will present some thoughts on these most pressing issues.

  • Smithsonian Team Flickr: a library, archives, and museums collaboration in web 2.0 space

    The Flickr Commons was created as a forum for institutions to share their rich photographic collections with the emerging Web 2.0 audience of Flickr; the Smithsonian Institution was the fourth member of the Commons. The Smithsonian effort was a direct collaborative effort of the libraries, archives, museums, and information technology staff that generated new pathways for collaboration between these units. As the world’s largest museum complex, these Smithsonian units serve as a microcosm for collaboration in the information age. The Flickr Commons project provided insights into how the knowledge, skills, and abilities of libraries, archives, and museums (LAM) can converge in the Web 2.0 environment to provide collection access to new, and in some cases unknown, audiences. Simultaneously, by putting “LAM” content into an environment that allows for direct interaction by these audiences, the knowledge of the content for holding institutions is enriched. By exposing Smithsonian content within the Flickr environment, the Institution is learning what content is desired by the Web 2.0 world, how to bring crowd-sourcing into professionally curated collections, and how to bring diverse institutional skills together in a collaborative project.

    Background Advanced Computerized Decision Support Systems (CDSSs) assist clinicians in their decision-making process, generating recommendations based on up-to-date scientific evidence. Although this technology has the potential to improve the quality of patient care, its mere provision does not guarantee uptake: even where CDSSs are available, clinicians often fail to adopt their recommendations. This study examines the barriers and facilitators to the uptake of an evidence-based CDSS as perceived by diverse health professionals in hospitals at different stages of CDSS adoption. Methods Qualitative study conducted as part of a series of randomized controlled trials of CDSSs. The sample includes two hospitals using a CDSS and two hospitals that aim to adopt a CDSS in the future. We interviewed physicians, nurses, information technology staff, and members of the boards of directors ( n  = 30). We used a constant comparative approach to develop a framework for guiding implementation. Results We identified six clusters of experiences of, and attitudes towards CDSSs, which we label as “positions.” The six positions represent a gradient of acquisition of control over CDSSs (from low to high) and are characterized by different types of barriers to CDSS uptake. The most severe barriers (prevalent in the first positions) include clinicians’ perception that the CDSSs may reduce their professional autonomy or may be used against them in the event of medical-legal controversies. Moving towards the last positions, these barriers are substituted by technical and usability problems related to the technology interface. When all barriers are overcome, CDSSs are perceived as a working tool at the service of its users, integrating clinicians’ reasoning and fostering organizational learning. Conclusions Barriers and facilitators to the use of CDSSs are dynamic and may exist prior to their introduction in clinical contexts; providing a static list of obstacles and facilitators, irrespective of the specific implementation phase and context, may not be sufficient or useful to facilitate uptake. Factors such as clinicians’ attitudes towards scientific evidences and guidelines, the quality of inter-disciplinary relationships, and an organizational ethos of transparency and accountability need to be considered when exploring the readiness of a hospital to adopt CDSSs.

    Abstract:

    Background Computerized clinical decision support systems (CDSSs) have been shown to improve the efficiency and quality of patient care by connecting healthcare professionals with high quality, evidence-based information at the point-of-care. The mere provision of CDSSs, however, does not guarantee their uptake. Rather, individual and institutional perceptions can foster or inhibit the integration of CDSSs into routine clinical workflow. Current studies exploring health professionals’ perceptions of CDSSs focus primarily on technical and usability issues, overlooking the social or cultural variables as well as broader administrative or organizational roles that may influence CDSS adoption. Moreover, there is a lack of data on the evolution of perceived barriers or facilitators to CDSS uptake across different stages of implementation. Methods We will conduct a qualitative, cross-sectional study in three Italian specialty hospitals involving frontline physicians, nurses, information technology staff, and members of the hospital board of directors. We will use semi-structured interviews following the Grounded Theory framework, progressively recruiting participants until no new information is gained from the interviews. Discussion CDSSs are likely to become an integral and diffuse part of clinical practice. Various factors must be considered when planning their introduction in healthcare settings. The findings of this study will guide the development of strategies to facilitate the successful integration of CDSSs into the regular clinical workflow. The evaluation of diverse health professionals across multiple hospital settings in different stages of CDSS uptake will better capture the complexity of roles and contextual factors affecting CDSS uptake.
