TI can benefit or degrade environmental outcomes, depending on RE penetration: it complements emissions abatement by mitigating dispatch problems associated with volatile and spatially dispersed RE but also promotes higher average generation from low-cost coal if RE production is too low. Against the backdrop of European decarbonization and planned TI expansion, we find that emissions increase for current and targeted year-2020 levels of RE production and decrease for year-2030 targets. Enhanced TI yields sizeable gains from trade that depend positively on RE penetration, without creating large adverse impacts on regional equity.

Readiness for Delivering Digital Health at Scale: Lessons From a Longitudinal Qualitative Evaluation of a National Digital Health Innovation Program in the United Kingdom

Despite positive evidence from localized implementations, new technologies have proven slow to become accepted, integrated, and routinized at scale. Objective: The aim of our study was to examine barriers and facilitators to implementation of digital health at scale through the evaluation of a £37m national digital health program: ‟Delivering Assisted Living Lifestyles at Scale” (dallas) from 2012-2015. Methods: The study was a longitudinal qualitative, multi-stakeholder, implementation study. The methods included interviews (n=125) with key implementers, focus groups with consumers and patients (n=7), project meetings (n=12), field work or observation in the communities (n=16), health professional survey responses (n=48), and cross program documentary evidence on implementation (n=215). We used a sociological theory called normalization process theory (NPT) and a longitudinal (3 years) qualitative framework analysis approach. This work did not study a single intervention or population. Instead, we evaluated the processes (of designing and delivering digital health), and our outcomes were the identified barriers and facilitators to delivering and mainstreaming services and products within the mixed sector digital health ecosystem. Results: We identified three main levels of issues influencing readiness for digital health: macro (market, Infrastructure, Policy), meso (organizational), and micro (professional or public). Factors hindering implementation included: lack of information technology (IT) Infrastructure, uncertainty around information governance, lack of incentives to prioritize interoperability, lack of precedence on accountability within the commercial sector, and a market perceived as difficult to navigate. Factors enabling implementation were: clinical endorsement, champions who promoted digital health, and public and professional willingness. Conclusions: Although there is receptiveness to digital health, barriers to mainstreaming remain. Our findings suggest greater investment in national and local Infrastructure, implementation of guidelines for the safe and transparent use and assessment of digital health, incentivization of interoperability, and investment in upskilling of professionals and the public would help support the normalization of digital health. These findings will enable researchers, health care practitioners, and Policy makers to understand the current landscape and the actions required in order to prepare the market and accelerate uptake, and use of digital health and wellness services in context and at scale. What is the Current State of Digital Health “Readiness” in the UK?: Findings from the Delivering Assisted Living Lifestyles at Scale (dallas) Programme

O'donnell, Sally Wyke, Nick WatsonAbstract:Problem: Digital health has the potential to support wellbeing, preventive care, and care delivery for those with chronic illness and multimorbidity. However, despite many demonstration projects and trials, and positive evidence from localised implementations, new technologies have proven slow to become accepted, integrated and routinized at scale. Previous studies have examined implementation issues in relation to single digital health products/interventions and/or single geographies but no studies have yet explored the full ecosystem readiness of a nation from stakeholders (professionals and public) to health care organisations (primary and secondary care), and wider commercial market, Infrastructure, and Policy environment to the deployment of a wide range of digital health products and services ranging from telecare services to web portals and apps. Here we have examined the readiness of the UK health system and wider environment to implement digital health at scale, through the evaluation of a national digital health and wellbeing programme called ‘Delivering Assisted Living Lifestyles at Scale’ (dallas, 2012-2015, £37M). The aim of this study was to identify barriers and facilitators to digital health and to make clear recommendations about key steps to help promote uptake and utilisation of digital health. Approach: Qualitative study involving Interviews (n=126) with key stakeholders; Focus groups (n= 7) with professionals and public using dallas services; dallas leads meetings (N=12); ethnographic field work/participant observation in one community (n=16); and cross programme documentary evidence (N=215) were used to evaluate the implementation of the dallas programme. We used a sociological theory, Normalisation Process Theory, and a longitudinal (3 years) qualitative framework analysis approach to identify emerging barriers and facilitators to the uptake and mainstreaming of digital health. Findings: Issues influencing readiness for digital health were noted at three levels: Macro (market; Infrastructure; Policy), Meso (organisational) and Micro (professional/public). Factors hindering implementation included: lack of IT Infrastructure both locally and nationally; uncertainty around information governance; lack of incentives to prioritise interoperability; lack of precedence on accountability within commercial sector; a market perceived as difficult to navigate; inadequate implementation resources; low IT skills and access across users (professional and lay); and concerns surrounding security and safety. Factors enabling implementation included: clinical endorsement; digital health champions; and public and professional willingness to embrace digital health. Consequences: There is receptiveness to digital health, but substantial barriers to widespread use remain. Our work has enabled us to produce ten clear recommendations to ensure progress in this sphere. These include: greater investment in national and local Infrastructure; implementation of clear systems for accreditation and quality assurance; incentivisation of interoperability; and investment in upskilling of professionals and public would help support normalisation of digital health.

2013Co-Authors: Daniel Albalate, Germa Bel, Xavier FagedaAbstract:Spain’s transport Infrastructure Policy has become a paradigmatic case of oversupply and of mismatch with demand. The massive expansion of the country’s transport Infrastructure over the last decade has not been a response to demand bottlenecks or previously identified needs. For this reason, the intensity of use today on all interurban modes of transport in Spain falls well below that of other EU countries. This paper analyzes the institutional and regulatory factors that have permitted this Policy, allowing us to draw lessons from the Spanish case that should help other countries avoid the pitfalls and shortcomings of Spanish Policy. Based on our analysis, we also discuss Policy remedies and suggest reforms in different regulatory areas, which could help improve the performance of Spain’s Infrastructure Policy.

Business History, 2011Co-Authors: Germa BelAbstract:This paper analyses Spanish Infrastructure Policy since the early 1700s: road building in the eighteenth century, railway creation and expansion in the nineteenth, motorway expansion in the twentieth, and high speed rail development in the twenty-first. The analysis reveals a long-term pattern, in which Infrastructure Policy in Spain has been driven not by the requirements of commerce and economic activity, but rather by the desire to centralise transportation around the country's political capital. As commerce has been unable to sustain the development of this Policy, regulation and subsidies from the national budget have regularly been used to decide the priorities regarding Infrastructure creation and to fund the development, maintenance, and operation of the networks. When high roads, bridges, canals, etc. are in this manner made and supported by the commerce which is carried on by means of them, they can be made only where that commerce requires them, and consequently where it is proper to make them. Their expense too, their grandeur and magnificence, must be suited to what that commerce can afford to pay. They must be made consequently as it is proper to make them. A magnificent high road cannot be made through a desert country where there is little or no commerce, or merely because it happens to lead to the country villa of the intendant of the province, or to that of some great lord to whom the intendant finds it convenient to make his court.
