    Abstract:

    Objective To present two cases of Interstitial Pregnancy treated successfully by use of laparoendoscopic single-site surgery (LESS). Design Case series. Setting Department of obstetrics and gynecology at a tertiary health care facility. Patient(s) Two women with Interstitial Pregnancy requiring surgical treatment. Intervention(s) Laparoscopy by use of LESS with the single-incision laparoscopic surgery (SILS) system. Main Outcome Measure(s) Feasibility of LESS with the SILS system. Result(s) Two patients were treated successfully by use of LESS with the SILS system. After laparoscopic confirmation of the Interstitial Pregnancy, a cornual resection was performed with bipolar forceps and scissors for the first patient and with an automatic stapler for the second. Conclusion(s) Laparoscopic treatment of Interstitial Pregnancy by use of LESS seems feasible. Larger series are necessary to confirm these findings.

    Abstract:

    We report on management and subsequent fertility outcome of Interstitial Pregnancy in a retrospective cohort study (Canadian Task Force classification II-3) at a university affiliated teaching hospital. Of 706 women with extrauterine Pregnancy, 14 consecutive women with Interstitial Pregnancy were treated by methotrexate, laparotomy or laparoscopy between 1997 and 2007. The first four women, with significant hemoperitoneum, were treated by laparotomy. Of the next 10 women, four were selected for medical treatment with methotrexate. Only one case was treated successfully. The other six women had laparoscopic treatment. Of nine laparoscopies, one was converted to laparotomy due to excessive blood loss during the procedure. Of nine women desiring a child, three were infertile, whereas six conceived with an intrauterine Pregnancy. A change from diagnosis later in Pregnancy and laparotomy to more conservative treatment, mainly by laparoscopy, suggests a possibly better subsequent Pregnancy rate.

    Abstract:

    Abstract Interstitial Pregnancy is among the most dangerous types of ectopic Pregnancy. Four such pregnancies in three women were treated by three conservative modalities with favorable results. A 10-week Interstitial Pregnancy was successfully treated by laparoscopic-guided local methotrexate (MTX) injection into the gestational sac. Six years later the same woman had a repeat unruptured Interstitial Pregnancy at 9 weeks' gestation, with the gestational sac located in the same location as the previous one. Laparoscopic cornuostomy was performed. An asymptomatic woman in the eighth week of an Interstitial Pregnancy was treated with systemic MTX, but despite decreasing β-human chorionic gonadotropin levels, cornual rupture occurred. The patient was successfully treated by laparoscopic cornuostomy. The final patient was admitted in hypovolemic shock and hemoperitoneum and was treated successfully for ruptured 8-week Interstitial Pregnancy by laparoscopic cornuostomy.

    Abstract:

    Interstitial Pregnancy has been treated so far mainly by either resection of the cornu or hysterectomy. In this article, two patients with cornual Pregnancy were operated upon laparoscopically with an uneventful postoperative recovery. This laparoscopic treatment is simple, does not require extensive operative laparoscopic expertise, and is relatively short and less expensive than laparotomy. We suggest that laparoscopic conservative repair be used to replace cornual resection by laparotomy or hysterectomy in cases of early diagnosed Interstitial Pregnancy.
