    Abstract:

    Objective To study the X-ray feature of congenital duodenum stenosis complicated by small Intestine Malrotation and evaluate the value of X-ray examination to congenital duodenum obstruction.Methods The X-ray feature of congenital duodenum stenosis complicated by small Intestine Malrotation which confirmed by surgery were analyzed retrospectively and summarized,abdominal plain film,radiography with iodine solution of upper digestive tract and barium enema were performed in all patients.Results On the plain film,double bubble or tri-bubble were found in all cases.Dilatation of stomach,too much leftover in stomach,duodenum dilataltion above stensosis and reversed peristalsis were noticed in opacification of upper digestive tract with meglumine diatrizoate.Lune prolapse were noticed in cases of membranous stenosis.Little meglumine diatrizoate reach to other small Intestine and colon,and spend much time.The filament of meglumine diatrizoate were noticed slowly go through the stenosis bowel.Conclusions Congenital duodenum stenosis is the cause of congential duodenum obstruction,which complicated with small Intestine Malrotation or other malformations.Radiography and contrast examination can offer important value to clinical diagnosis.

    Abstract:

    Objective To discuss the roles of alimentary tract radiography in diagnosing congenital Intestine Malrotation.Methods A retrospective analysis was done on the x-ray imaging features of the 21 cases which were diagnosed as congenital Intestine Malrotation by surgery.Result Under x-ray film or fluoroscopy, most of the patients with congenital Intestine Malrotation turn out to be "double-bubbled" and the barium enema showed the abnormal location of cecum,appendix and ascending colon.The iodine or barium meal showed fixed flexion or corkscrew descending of superior part of jejunum,or abnormal location of superior part of jejunum,which rose up to the same height of duodenal bulb.At the same time,the abnormal location of cecum,appendix and ascending colon can also be seen. Conclusion Barium enema or/and iodine or barium meal is of decisive significance in the diagnosis of Congenital Intestine Malrotation.

  • SURGICAL TREATMENT OF RARE COMBINATION OF INTESTINAL Malrotation WITH SECONDARY LYMPHANGIECTASIA

    Abstract:

    The rare combination of intestinal lymphangiectasia with Malrotation of the duodenum in a child of three months of life is described. Basing on the literature review only 3 similar cases were described in the world practice. The boy with protein-losing enteropathy was examined at Moscow Scientific Centre of Children’s Health. The child had vomiting, diarrhea, loss in body weight, hypoproteinemia, lymphopenia. The infectious nature of the disease was excluded. It had been suggested the Waldman desease (primary intestinal lymphangiectasia). The prognosis for such disease is unfavorable. An examination of the child was continued against the backdrop of ongoing symptomatic therapy. Complete physical examination included monitoring laboratory blood tests, X-ray examination with contrast, CT-scan, gastroduodenoscopy with biopsy of the mucosa of the small Intestine. Malrotation duodenum with the recurrent mid-gut volvulus with the development of secondary intestinal lymphangiectasia was diagnosed. Modern methods of examination and multidisciplinary approach made it possible to diagnose the case. Operation to eliminate fixation duodenum resulted in the recovery of the patient. At the present time the child grows and develops according to age and does not require treatment. The prognosis for this disease is regarded as favorable.

    Abstract:

    This research is based on the analysis of immediate and long-term treatment results of children with intestinal Malrotation (ok) (IM). The research was carried out basing on examination of 46 patients out of 74 patients with various forms of IM dismissed from hospital after open treatment or conservative management. The patients were treated in the hospital of pediatric surgery of TashPMI during the period from 2002 to 2013. The nature and frequency of post-surgery complications are described. Long-term results were evaluated as good - for 28 patients (66.7%), satisfactory - for 5 patients (11.9%), unsatisfactory - for 9 patients (21.4%). Inefficiency of conservative management is emphasized. Frequently observed chronic colostasis of various intensity and abdominal pains in the long-term period after the treatment were, in most cases, related to total or segmental colon elongation.
