Intima-Media Thickness in Obese Children Before and After Weight Loss

Obesity in childhood is related to increased Intima-Media Thickness, a noninvasive marker for early atherosclerotic changes. The objective of this study was to analyze the changes of Intima-Media Thickness in obese children with weight loss. METHODS. We analyzed the changes of Intima-Media Thickness and, as markers of cardiovascular risk factor profile, systolic and diastolic blood pressure, triglycerides, high-density lipoprotein cholesterol, glucose, insulin, and insulin resistance index (homeostasis model analysis) in 56 prepubertal obese children (median: 9 years old) before and after a 1-year outpatient intervention program. The control group consisted of 10 nonobese children of the same age and gender. We determined the carotid Intima-Media Thickness by B-mode ultrasound with a 14-MHz linear transducer. Substantial weight loss was defined by a reduction of overweight of at least 0.5 standard deviation scores in BMI. RESULTS. Obese children demonstrated a significantly thicker Intima-Media compared with the control group. Furthermore, blood pressure, triglycerides, insulin, and insulin resistance index were significantly higher in obese children, whereas high-density lipoprotein cholesterol was significantly lower. In the 24 obese children with substantial weight loss, Intima-Media Thickness, blood pressure, triglycerides, insulin, and insulin resistance index decreased significantly, whereas high-density lipoprotein cholesterol increased significantly. In the 32 obese children without substantial weight loss, there were no significant changes apart from an increase of insulin and insulin resistance index. CONCLUSIONS. Because obese children demonstrated a thicker Intima-Media, vascular changes seemed to occur already in childhood obesity. Paralleling the improvement of the cardiovascular risk factor profile, Intima-Media Thickness decreased in obese children with substantial weight loss, suggesting the reversibility of early atherosclerotic changes.

Antiplatelet drugs attenuate progression of carotid Intima-Media Thickness in subjects with type 2 diabetes.

Ueda, Masatsugu HoriAbstract:The Intima-Media Thickness of the carotid artery has been established as a surrogate of definite atherosclerosis in subjects with high risk of vascular events. This study was done to evaluate the effectiveness of long-term antiplatelet therapy in attenuating progression of the Intima-Media Thickness of the carotid artery of subjects with type 2 diabetes. Subjects who had an Intima-Media Thickness over the threshold of the normal subjects but showed no symptoms of vascular events were randomly divided into groups given antiplatelet drugs [ticlopidine (n=34) or a small dose of aspirin (n=40)] or no drugs (n=74). For the follow-up period (3.0±0.06 years), the subjects not given antiplatelet drugs showed a significantly higher progression of Intima-Media Thickness (0.067±0.009 mm/year) than those given ticlopidine (0.034±0.013 mm/year) or aspirin (0.033±0.010 mm/year). Stepwise multivariant regression analysis showed that long-term administration of ticlopidine or aspirin significantly reduced the progression of Intima-Media Thickness of diabetic subjects by 0.041 mm/year or 0.032 mm/year, respectively. These data indicated that despite differences of their pharmacological mechanisms, antiplatelet drugs could attenuate the progression of Intima-Media Thickness of the carotid artery wall of asymptomatic type 2 diabetics who had early-stage carotid atherosclerosis.

Relation of Intima-Media Thickness to atherosclerotic plaques in carotid arteries. The Vascular Aging (EVA) Study.

B-mode ultrasonography of the CAs was performed in a cohort of 516 men and 756 women aged 59 to 71 years who had been recruited for the European Vascular Aging Study. Ultrasound examination included measurement of Intima-Media Thickness of the common CA (CCA) and the sites of plaque in the internal CA and bifurcations. Significant associations between increases in CCA Intima-Media Thickness and both the presence and severity of atherosclerotic plaque were found in men and women. Examination of specific risk factors for increases in CCA Intima-Media Thickness in the presence of plaque showed that, after adjustment for sex, both ultrasound measurements were independently related to age, body mass index, hypertension, and ever smoking (versus never smoking). Diabetes and current smoking were associated with Intima-Media Thickness only, whereas hypercholesterolemia was related to plaque only. However, when subjects who were taking lipid-lowering drugs were excluded, lipoproteins and apolipoproteins were more consistently related to Intima-Media Thickness than to plaque. In subjects free from any antihypertensive treatment, both Intima-Media Thickness and plaques were independently associated with systolic blood pressure. After adjustment for sex and other risk factors, the odds ratio for having at least one plaque associated with a 0.10-mm increase in CCA Intima-Media Thickness was 1.18 (95% confidence interval, 1.05 to 1.32). In this relatively aged population, increases in Intima-Media Thickness as measured in the CCAs were clearly related to locally detected atherosclerosis and known risk factors for atherosclerosis. Longitudinal studies are needed to clarify the role of arterial wall thickening in the atherosclerotic process.
