Design Double-blind, randomized, crossover trial with four periods of observation, each lasting 2 weeks. Setting General internal medicine outpatient clinic. Participants Ambulatory outpatients who experienced an estimated two or more typical nocturnal Leg Cramps per week. Intervention 200 mg of quinine taken at bedtime. Main Outcome Measures Self-reported ratings of Leg Cramp frequency, duration, and intensity. Results Sixteen patients completed the trial. During the 2 weeks patients used quinine, there was no statistically significant reduction in mean Leg Cramp number (quinine 3.5 vs placebo 4.2, P = 0.48), mean Leg Cramp duration (quinine 152 seconds vs placebo 163 seconds, P = 0.89), or patient ratings of severity using a 1 (low) to 10 (high) scale (quinine = 4.2 vs placebo = 4.0, P = 0.83). Conclusion No significant reduction in nocturnal Leg Cramp frequency, intensity, or duration could be found using nightly quinine in this study. Since quinine is not without the potential for side effects and drug interactions, clinicians need to carefully consider the likelihood of modest benefit associated with quinine against the potential for side effects and drug-drug interactions.

Treatment of nocturnal Leg Cramps. A crossover trial of quinine vs vitamin E.

Shirley, John H Wasson, David W NierenbergAbstract:Objective.— This study compared the efficacy and safety of quinine sulfate, vitamin E, and placebo in the treatment of nocturnal Leg Cramps. Design.— A random-order, double-blind, placebocontrolled crossover trial was performed. Setting.— The study was conducted at the Veterans Affairs Medical Center, White River Junction, Vt. Participants.— Twenty-seven male veterans, aged 38 to 73 years, who experienced at least six Leg Cramps per month were recruited through the general medicine walk-in clinic or were referred from other clinics. Fifty-five subjects were contacted, 30 were enrolled consecutively, and 27 completed the study. Intervention.— Subjects received, in random order, quinine sulfate (200 mg at supper and 300 mg at bedtime), vitamin E (800 U at bedtime), or placebo for 4-week periods. These periods were separated by 4-week washout intervals. Outcome Measures.— Patients reported Cramp frequency, severity, and sleep disturbance caused by Cramps. Results.— Compared with treatment with placebo, quinine reduced the frequency of Cramps and sleep disturbance, but not the average Cramp severity. Thirteen of 27 patients had at least a 50% reduction in the number of Cramps while receiving quinine; the response was usually seen within 3 days. There was evidence of a mild increase in side effects while subjects received quinine. Vitamin E was not effective in reducing Leg Cramp frequency, severity, or sleep disturbance. Conclusions.— Quinine sulfate, but not vitamin E, is superior to placebo in the treatment of nocturnal Leg Cramps. (Arch Intern Med. 1992;152:1877-1880)

Leg Cramps in relation to metabolic syndrome.
Georgian medical news, 2020Co-Authors: M N ManiiaAbstract:: A Leg Cramp is a pain that comes from a Leg muscle. It is due to a muscle spasm which usually occurs in a calf muscle, below and behind a knee. Leg Cramps (often called night Cramps) usually occur most commonly at night when in bed. Night Leg Cramps are involuntary painful contractions of skeletal muscles arose in the calves and soles of the feet. Although in most cases they aren't harmful and resolve easily in some instances they have a long duration and can result in intense pain, disturb normal sleep and make a person feel anxious. Pathophysiology of Leg Cramps is poorly understood. The aim of our study was to determine the role of metabolic syndrome in relation to night Leg Cramps. The study included 86 subjects aged 34 to 88 years. Metabolic syndrome group consisted of 40 subjects (10 men (25%) and 30 women (75%)); the control group consisted of 46 persons (9 men (19.5%) and 37 women (80.5%)). According to frequency and intensity of manifestation Leg Cramps were subdivided into less frequent and frequent Leg Cramps. Blood samples were analysed for lipids, fasting glucose, red blood cells and electrolytes. Persons were screened for Leg vein insufficiency as well. The investigation showed that 77.5% (31/40) of patients with metabolic syndrome had Leg Cramps, from which 60% (24/40) had frequent Leg Cramps. In control group 73.9% (34/46) had Leg Cramps, from which 50% (23/46) had frequent Leg Cramps. Among known predisposing factors Leg Cramps most often were associated with deep vein insufficiency and superficial vein varicose. High frequency of night Leg Cramps in our study is due to female predominance (75% versus 25% women and men, respectively) and age distribution in our study population (from 34 to 88 years old). The investigation showed that people often experience nocturnal Leg Cramps. Leg Cramp is slightly increasing among the patients with metabolic syndrome. Frequent Leg Cramps were observed in 60% of cases in metabolic syndrome group versus 50% of cases in controls.

Nocturnal Leg Cramps in children: incidence and clinical characteristics.

ChanAbstract:The records of 2527 healthy children seen in an ambulatory care clinic were evaluated for nocturnal Leg Cramps in the preceding 12 months, frequency and duration of the Cramps, whether the Cramps affected one Leg or both Legs at a time, whether there was associated muscle Cramps in feet, whether the Cramps occurred when the child was awake or asleep, and whether there was residual tenderness in the affected muscles. Nocturnal Leg Cramps were present in 185 children for an overall incidence of 7.3%. Leg Cramps were noted only in children aged > or = 8 years. The incidence increased at 12 years and peaked at 16 to 18 years of age. A majority (81.6%) of the affected children had nocturnal Leg Cramps 1 to 4 times per year. The mean duration of episodes was 1.7 minutes. Leg Cramps were unilateral in 98.9% of cases and the ipsilateral foot also was involved in 18.9% of cases. One hundred thirty-five (73%) children had Leg Cramps while asleep, and the remaining 23 (12.4%) children had Leg Cramps in either state. Fifty-seven (30.8%) children had residual tenderness in the affected muscles. The mean duration of residual tenderness was 33.2 minutes (range: 2 minutes-1 day). We conclude that nocturnal Leg Cramps are common in children aged > 12 years. A majority of the affected children have Leg Cramps 1 to 4 times per year. The Cramps are usually unilateral and occur when the children are asleep. Normal duration of the Leg Cramp is < 2 minutes. Residual tenderness is present in approximately 30% of the affected children. Residual tenderness, if present, usually lasts for half an hour.

Idiopathic nocturnal Leg Cramp is the most common form of Cramps. Occasionally, Leg Cramps may signify a significant systemic disorder. Investigations are usually not necessary unless indicated by the history or physical examination. Symptomatic treatment consists of stretching the affected calf muscle by forcible dorsiflexion of the foot.
