This mechanism results in injury to the anterior talofibular Ligament alone or in conjunction with the calcaneofibular Ligament and posterior talofibular Ligament. Patients experiencing recurrent ankle sprains despite nonoperative measures often require surgical management. Recent focus has been on augmentation procedures to improve the stability of a lateral ankle Ligament Repair by protecting it during the healing phase and supporting early mobilization. This article describes, with video illustration, anterior talofibular Ligament Repair with suture tape augmentation.

Scaffolds for Tendon and Ligament Repair and Regeneration

Scaffolds have been used for mechanical and biologic reinforcement of Repair and regeneration with mixed results. This review summarizes efforts made using biologic and synthetic scaffolds using rotator cuff and ACL as examples of clinical applications, discusses recent advances that have shown promising clinical outcomes, and provides insight into future therapy.

Ulnar Collateral Ligament Repair With Suture Augmentation.

AhmadAbstract:Reconstruction of the ulnar collateral Ligament (UCL) remains the gold standard for treating overhead throwing athletes with valgus instability secondary to UCL pathology. Although surgical techniques for reconstruction have evolved over time, current methods allow 90% of patients to return to their preinjury level of activity. Despite encouraging results with reconstruction, UCL Repair remains a valuable treatment option for patients with UCL pathology fitting specific criteria. There are a number of advantages associated with a direct Repair, and further, the development of collagen-coated sutures for Ligament Repair augmentation makes this procedure an attractive surgical option under the correct circumstances. This article provides a detailed description and video demonstration of the surgical steps used to perform a UCL Repair with suture augmentation.

deltoid Ligament Repair versus syndesmotic fixation in bimalleolar equivalent ankle fractures

NunleyAbstract:OBJECTIVES To compare the outcomes of bimalleolar equivalent ankle fractures treated with lateral malleolus open reduction and internal fixation (ORIF) with transsyndesmotic fixation versus lateral malleolus ORIF with deltoid Ligament Repair. DESIGN Retrospective design. SETTING Single level I trauma center. PATIENTS/PARTICIPANTS Subjects with bimalleolar equivalent ankle fractures were eligible for enrollment. Patients with posterior or medial malleolar fractures and age <16 years were excluded. Twenty-seven total subjects met inclusion criteria. Fifteen patients were treated with lateral malleolus ORIF with transsyndesmotic fixation, whereas 12 were treated with lateral malleolus ORIF with deltoid Ligament Repair. INTERVENTION Lateral malleolus ORIF with transsyndesmotic fixation versus lateral malleolus ORIF with deltoid Ligament Repair. MAIN OUTCOME MEASUREMENTS Lower Extremity Function Scale, Foot and Ankle Disability Index, Short Musculoskeletal Function Assessment, Foot and Ankle Outcome Score, American Orthopaedic Foot and Ankle Society Ankle-Hindfoot Scale, Visual Analog Pain Scale, and overall function of lower extremity. RESULTS Univariate analysis of the outcome questionnaire variables demonstrated no statistically significant differences between the 2 groups. All patients in the syndesmotic fixation group underwent a subsequent procedure for removal of the syndesmotic implant. Additionally, there were 2 complications in the syndesmotic group that required repeat operative intervention. CONCLUSIONS Repairing the deltoid Ligament at the time of lateral malleolus fixation demonstrates subjective, functional, and radiologic outcomes that are comparable with lateral malleolus fixation with syndesmotic fixation for bimalleolar equivalent ankle fractures. The former avoids the costs and inherits surgical risks that occur during a subsequent operation for removal of the syndesmotic implant. LEVEL OF EVIDENCE Therapeutic Level III.
