The Design and Biomechanical Evaluation of Individual Mandibular Prosthesis

The normal Mandible 3D model was remodeled using data extracted from computed tomography (CT) scan. A large surgical defect was simulated on the right side of the Mandible model beginning mesially from the angle of the Mandible and extending to the mesial side of canine teeth along the body of the Mandible. The local mandibular Prosthesis model with the suitable dental implants was designed to reconstruct the defected Mandible based on the dimension and contour of the resected Mandible segment. The material of the Prosthesis is titanium. The biomechanics effect of the Prosthesis was evaluated using finite element analysis (FEA) method. The most stress concentration areas are the neck of abutment. The stress decreases significantly with the increase of the diameter of the implant hole. And the ratios of the height of the abutment with the depth of the hollow have little effect on the stress. The stress in the Prosthesis is approximately 10 times higher than that of the nature bone .There is an obvious barrier effect of the stress. The stress in the Prosthesis is symmetric and much lower than the static yield limit. The results suggest that this individual Mandible Prosthesis design is secure and feasible. And reducing the volume of the Prosthesis under the premise of biomechanics ensures the soft-tissue coverage. Using this Prosthesis to reconstruct the Mandible, main complication will be decreased in clinic.

Survey of treatment-seeking complete denture wearers concerning tooth loss, retention behavior and treatment expectations

MarinelloAbstract:: The aim of the current study was to gather information about time and etiology of tooth loss, and to analyze the reasons for treatment needs with a questionnaire used among edentulous patients. 60 patients (age 44-98 years) who requested dental treatment were included. More than half of the patients were edentulous before the age of 60, in almost all cases the maxilla was affected first. This difference was more pronounced in women, who experienced tooth loss in the maxilla 17 years prior to the Mandible. At the time of edentulism women were about ten years younger than men. According to patients'report, periodontal disease was the most common reason for tooth loss. In the Mandible, Prosthesis retention was influenced by alveolar ridge resorption, while in the maxilla there was no relation between the amount of resorption and perceived retention. This indicates that maxillary prostheses retention is influenced by other factors besides the alveolar ridge morphology. So, permanent intake of drugs potentially causing xerostomia was related to poorer assessment of prostheses retention in the upper jaw. The results of the questionnaire "Patient satisfaction related to prosthetic restoration" before treatment indicated that the general satisfaction with the existing complete denture in the Mandible was influenced by functional problems and the impairment of patients'self-confidence (R-squared = 0.754). The main reason for requesting treatment was the poor retention of the lower denture. More than half of the participants stated that they refrained from eating certain foods due to a reduced chewing ability.

Temperature changes in a cemented mandibular endoProsthesis: in vitro and in vivo studies

JansenAbstract:Abstract Using polymethylmethacrylate (PMMA) cement in cemented endoprostheses may cause heat necrosis and Prosthesis failure due to the highly exothermic reaction. This study determined the magnitude and duration of temperature change during the cementation of a mandibular endoProsthesis in an in vitro and in vivo Macaca fasicularis model. In the in vivo study the median maximum temperature ( T max ) around the Mandible-Prosthesis unit (MPU) was 31.0°C with the peak T max at hole 1 (1mm from stem). The in vitro study recorded a lower T max and indicated a trend that increased spacing (groove) around the Prosthesis results in a higher T max . All the T max MPU measurements were lower than normal body temperature (38°C). In the in vivo study the median maximum temperature change ( T maxΔ ) was 1.8°C; in the vitro study, the T maxΔ of the 4mm groove width was significantly higher than all others. Temperature increases were transient, with the temperature returning to baseline a median of 6.0min after T max . Histological analysis showed surrounding tissue at the cement–bone interface with mild inflammation. Within the parameters tested, there was minimal risk of thermal damage. The temperature changes were influenced by the quantity of cement used and the distance from the Prosthesis stem.
