    Abstract:

    This review presents a classification system for Maxillofacial prostheses, while explaining its types. It also aims to describe their origin and development, currently available materials, and techniques, predicts the future requirements, and subsequently discusses its avenues for improvement as a restorative modality. A literature search of the PubMed/Medline database was performed. Articles that discussed the history, types, materials, fabrication techniques, clinical implications, and future expectations related to Maxillofacial prostheses and reconstruction were included. Fifty-nine articles were included in this review. Maxillofacial prostheses were classified as restorative or complementary with subclassifications based on the prostheses finality. The origin of Maxillofacial prostheses is unclear; however, fabrication techniques and materials have undergone several changes throughout history. Currently, silicones and acrylic resins are the most commonly used materials to fabricate customized prostheses. Maxillofacial prostheses not only restore several types of orofacial defects but also improve the patients' quality of life. Although the current clinical scenario concerning the field of Maxillofacial prostheses is promising, improvements in material quality and techniques for Maxillofacial prostheses may be expected in the future, to produce better results in the treatment of patients.

    Abstract:

    Objectives: The objective of this study was to evaluate by a visual method of comparison the color stability of nonpigmented and pigmented facial silicones after accelerated aging. Materials and Methods: Two kinds of silicones were used in this study; one specifically formulated for facial prostheses and the other an acetic silicone for industrial use. Twenty-four trial bodies were made for each silicone. These were divided into colorless and intrinsically pigmented groups: ceramic, make-up, and iron oxide. The groups were submitted to accelerated aging for nonmetallic materials. An initial reading and subsequent readings were made at 163, 351, 692, and 1000 hours using a visual method of comparison. The values were annotated in a spreadsheet by two observers, according to scores elaborated for this study. Results: All groups presented color stability in the visual method. According to the results obtained and analyzed in this study, we can conclude that both silicones, Silastic 732 RTV and Silastic MDX 4-4210, behaved similarly, they can therefore be indicated for use in Maxillofacial Prosthesis. The time factor of aging influenced negatively, independently of the pigmentation, or lack of it, and of silicones and no group had visually noticeable alterations in any of the accelerated aging time, independently of the addition or not of pigments.

    AIM The aim of this report is to describe a quick and simple method of positioning retention magnets when deformed patients are rehabilitated with an oculopalpebral prostheses attached to a maxillary denture/obturator. BACKGROUND In patients with deformities requiring complex rehabilitations, the use of magnets is the most efficient means of providing combined prostheses with retention quality and stability. Usually prostheses with magnets are in sections and have a magnet in each section. When the sections are put together properly, the magnets are attracted to each other and retain the sections. REPORT An edentulous patient presented for a post-surgical evaluation of a Maxillofacial Prosthesis used to repair a partial maxillectomy and left orbital exoneration which removed all adjacent tissues leaving an open communication between the oral, nasal, and orbital cavities. The proposed treatment plan included construction of a maxillary complete denture with a palatal obturator and a mandibular complete denture. Magnets were used to attach the oculopalpebral Prosthesis to the maxillary denture/obturator. SUMMARY Use of retention magnets simplify the clinical and laboratorial phase, retains the denture, and makes it stable and comfortable for the patient. This treatment is one successful approach to the restoration of oral function and increases the patient's quality of life.

    Objectives: This study aimed to evaluate the cytotoxicity of different adhesive systems used to bond acrylic resin and facial silicone in implant-retained Maxillofacial Prosthesis, in keratinocytes cells (HaCat), through the analysis of cell proliferation, production of proinflammatory cytokines and extracellular matrix proteins. Methods: For this, 21 samples were divided into 7 groups: Resin, Silicone, Resin + Silastic Medical Adhesive Type A + Silicone (RAS), Resin + DC 1205 Primer + Silicone (RDCpS), Resin + Sofreliner Primer + Silicone (RSpS), Resin + DC1205 Primer + Silastic Medical Adhesive Type A + Silicone (RDCpAS), and Resin + Sofreliner Primer + Silastic Medical Adhesive Type A + Silicone (RSpAS). Eluates corresponding to 24 hours (h) of sample immersion in medium were prepared, in which HaCat cells were exposed for 72 h. Cytotoxicity was evaluated by cell viability MTT test. The Interleukin 1β, Interleukin 6 (IL6), Tumor Necrosis Factor α and Macrophage Inflammatory Protein 1α levels were evaluated by ELISA, and mRNA expression for Collagen Type IV (COL IV), Matrix Metalloproteinase 9 (MMP9) and Transforming Growth Factor β (TGFβ), by RT-PCR. Data were submitted to ANOVA and Bonferroni tests (p<0.05). Results: The materials did not present cytotoxic potential. IL1β, TNF α e CCL3/MIP1α were not detectable. RDCpS presented the highest concentrationof IL6. Concentrations of COL IV, MMP9 e TGF β were not statistically different among groups. Conclusions: Materials evaluated were not toxic and DC 1205 primer not associated with adhesive (RDCpS group) may trigger an inflammatory process. Acknowledgment : FAPESP #2012/05247-1

    Abstract:

    The composition and properties of silicone–hollow microsphere composites were studied in view of establishing a new type of Maxillofacial Prosthesis material. Two types of microspheres were used in different concentrations and were well dispersed in the silicone matrix. The mechanical properties of the composites were evaluated. Expancel hollow microspheres improve the density, Shore A hardness, and breaking elongation of the materials but degrade their tensile strength, tear strength, and dynamic mechanical properties, while opposite trends were observed with hollow SiO2 microspheres. Ideal properties for Maxillofacial prosthetic applications can thereby be obtained by blending the two types of hollow microspheres in specific proportions.

    A Maxillofacial Prosthesis is a successful treatment modality to restore missing facial parts. Digital technologies and 3D printing are employed in constructing facial prostheses such as ears; however, their application is still partial, and final prostheses are usually manufactured conventionally using stone molds. This report aims to introduce a complete digital workflow to construct a nasal Prosthesis and compare it to the conventional workflow of a patient requiring a nasal Prosthesis. A computer tomography scan showing the defect was exported to specialized software to create 3D reconstructions of the patient's face and underlying bone. The nose was digitally designed restoring facial esthetics, anatomy, shape, and skin color. Different skin tones were digitally matched to skin tissues adjacent to the defect area using the Spectromatch system. The design was 3D printed in flexible and colorful material at 16 μm resolution using a 3D printer. External color pigmentations were applied to the nose for optimum esthetics, and the prosthetic nose was sealed in silicone and left to heat polymerize for 15 minutes. The prosthetic nose was retained in place using biomedical adhesive, and the patient was pleased with it. This report proposes a complete digital workflow to directly design and fabricate a prosthetic nose of acceptable esthetics. Such a workflow can lead to enhanced Prosthesis reproducibility and acceptability and may become an effective treatment option for treatment of patients with facial defects.
