simultaneous integrated boost intensity modulated radiation therapy for Mediastinum Metastasis in patients with esophageal carcinoma after operation
Journal of Modern Oncology, 2010Co-Authors: Li TaoAbstract:Objective:To investigate the clinical efficacy and significance of simultaneous integrated boost intensity-modulated radiotherapy (SIB-IMRT) for the patients with esophageal carcinoma after the radical esophagectomy and with mediastinal Metastasis.Methods:SIB-IMRT plans were designed for 39 patients with esophageal carcinoma after the radical esophagectomy and with mediastinal Metastasis.Two target volumes were predefined:PTV1,the target volume of the lymph node region,which was given to 69Gy(2.3Gy fractions),and PTV2,the target volume of electively treated region,which was given to 54Gy(1.8Gy fractions).Five equispaced coplanar beams were applied.Results:The plan with 5 intensity-modulated beams could produce good dose distribution for the two target volumes.All the patients completed the treatments in 6 weeks.The outcome was evaluated 3 months after SIB-IMRT.Patients had CR in 66.7%(36/54);PR 33.3%(18/54).The overall response rate was 100.0%.Conclusion:SIB-IMRT can reach a higher value of some radiation physical parameters for the patients with esophageal carcinoma after the radical esophagectomy and with mediastinal Metastasis.Local-regional control rate was improved by using SIB-IMRT.The acute radiation toxicity of SIB-IMRT was well tolerated.SIB-IMRT is a promising radiotherapy modality for the advanced esophageal carcinoma.

Acta Academiae Medicinae Nanjing, 2005Co-Authors: Zhou JiannongAbstract:Objective: To study the effects of late-course accelerated hyperfrationated 3-dimmentional conformal radiation therapy (LCAH 3-DCRT) for Mediastinum Metastasis in patients with esophageal carcinoma after carcinoma. Methods:One hundred and four patients were randomly divided into 2 groups(conventional treatment group and study group).All accepted conventional RT at first. When the dose reached 38Gy,the conventional treatment group changed the angle to avoid the spinal cord, with the total dose of 60Gy. The study group underwent 3-DCRT with a new plan(1.5Gy per time,2 times per day,5 days per week),with the total dose of 68Gy. Results:All the patients were reexamined with CT 3 months after the treatment finished. Out of the patients in conventional treatment group and study group,26.9 percent and 55.8 percent experienced CR,and 50 percent and 42.3 percent experienced PR, respectively. The total effective rate(CR + PR) was 76.9 percent and 98.1 percent. Conclusion: LCAH 3-DCRT can efficiently improve the local control rate of Mediastinum Metastasis after the failure of esophageal operation. There is significant difference between patients with CR(P ≤ 0.005)and those with CR + PR(P ≤ 0.05).The acute toxicity is tolerable.

A full understanding of the characteristics of PBML is important for its sequential treatment and prognosis. Patient concerns In this report, a 36-year-old female patient with previous uterine leiomyoma who underwent 3 surgical resections of the uterus, bilateral fallopian tubes, and partial omentum was investigated. The physical examination revealed a tumor in the right lower lobe and Mediastinum and a solid nodule in the right middle lobe. Diagnoses Chest computed tomography (CT) confirmed a tumor in the right lower lobe and Mediastinum and a solid nodule in the right middle lobe. Further positron-emission tomography computed tomography (PET-CT) with 18F-fluorodeoxyglucose (FDG) of the whole body showed mildly intense accumulation of 18F-FDG in the tumor (maximum standardized uptake value [SUV max], 2.6). A pathological examination then confirmed the presence of fibrous and vascular tissue after CT-guided percutaneous biopsy of the tumor in the right lower lobe. Additionally, surgical resection of the tumor and nodule was performed for histological analysis and immunohistochemical assays for estrogen receptor (ER) and progesterone receptor (PR). Interventions The patient underwent complete tumor surgical resection and nodule wedge resection. Outcomes No postoperative complications occurred. No recurrence or other signs of Metastasis were found during an 18-month follow-up observation period. Conclusion In this case, lung and mediastinal Metastasis of uterine fibroids was observed. However, depending on only a postoperative histological analysis is insufficient for the diagnosis of PBML. Histological analysis combined with an evaluation of the expression levels of ER and PR is crucial for the diagnosis and treatment of PBML.

Chinese Journal of Clinical Oncology and Rehabilitation, 2007Co-Authors: L I SiqingAbstract:Objective To evaluate the clinical efficacy of concurrent chemoradiotherapy in treatment of patients with Mediastinum Metastasis from esophageal carcinoma.Methods Seventy-six esophageal carcinoma patients who had Mediastinum Metastasis were divided into two groups at random:the observation group(concurrent chemoradiotherapy)had 36 patients and the control group(3 dimensional conformal radiation therapy)had 40 patients.The observation group was treated with 3 dimensional conformal radiation therapy(2.5-3.5 Gy per fraction,5 fractions per week with a total dose of 60-68 Gy) and concurrent chemotherapy(5-Fu 500 mg/ m2 d1-5,DDP 20mg/ m2 d1-5).The control group was treated with 3 dimensional conformal radiation therapy(2.5-3.5 Gy per fraction,5 fractions per week with a total dose of 60-68 Gy)only.Results Reexamination was performed after one month.Twenty-six patients got complete remission and 10 patients partial remission in the observation group,while in the control group,19 patients got complete remission and 19 patients partial remission.Both groups had no progress patients.The 1-,2-and 3-year survival rates in observation group were 82.4%,52.9% and 23.5%,compared with 60.5%,28.9% and 5.3% in the control group.Conclusion The efficacy of concurrent chemoradiotherapy in treatment of patients with Mediastinum Metastasis from esophageal carcinoma was better than radiotherapy alone.
