The cerebral capillary endothelium, or blood-brain barrier, is completely impermeable to this compound. It is unlikely, therefore, that Metrizamide pass any cell membranes. Since the affinity for rat brain hexokinase is three orders of magnitude below that of Dglucose, it is equally unlikely that Metrizamide attain intracellular concentrations in brain sufficient to inhibit glucose phosphorylation.

In vivo, however, the present autoradiographic study with labeled 2-deoxyglucose revealed a local increase of glucose metabolic rate in an area of direct Metrizamide application to the cerebral cortex. The local increase was 137% above the metabolic rate calculated for the contralateral region. Regions outside the application site showed no difference between the glucose metabolic rates of the ipsi- and contralateral hemispheres. Evidence from the literature suggests that Metrizamide may stimulate cortical activity by inhibiting acetylcholinesterase.

Acta Neurologica Scandinavica, 2009Co-Authors: Patricia A Pimental, Philip B GorelickAbstract:ABSTRACT – Aphasia following Metrizamide myelography has been reported infrequently. During a seven-month period, we examined two patients who developed Broca's aphasia, apraxia of speech, oral-buccal-facial apraxia and neurogenic stuttering after intrathecal Metrizamide administration. In each case, focal neurologic deficits were accompanied by clinical, electroencephalographic and radiologic signs of generalized neurologic disease. Serial speech and language evaluations initially revealed severe deficits that were largely resolved by the third day post-myelography. Out-patient follow-up examinations demonstrated persistence of mild speech and language abnormalities in each case. Our findings suggest that Metrizamide may cause longlasting neurologic dysfunction.
