    Abstract:

    To analyze and compare the outcomes of ossiculoplasty in terms of hEaring results and intra and post operative course, using autograft incus and titanium Middle Ear Prosthesis in patients with ossicular chain erosion. Patients between 12 and 60 yEars of age with a history of chronic Ear discharge with moderate conductive deafness (>40 dB HL) were included in the study. The patients after assessment of ossicular erosion were subjected to ossiculoplasty with autologous incus or titanium Prosthesis randomly. The patients were followed up after 3 months to analyze the functional and anatomical results. Out of 21 patients with ossicular chain defect were included in the study, of whom 10 patients underwent ossiculoplasty with autograft incus and 11 with titanium Prosthesis. Postoperative hEaring evaluation by pure tone audiogram was done after 3months, which showed successful hEaring Complications and extrusion rate were nil. In patients with incus average Post operative pure tone average was 43.5 dB and Net gain in hEaring was 10.6 decibels. In patients with titanium Prosthesis, average Post operative pure tone average was 41.4 dB and Net gain in hEaring was 15.9 decibels. Incus and titanium has equal postoperative hEaring. In terms of hEaring gain Titanium Prosthesis gave a better haring gain than Ossicular transposition. Complications in the short period studied are nil in both groups

    Introduction This prospective cohort study aims to analyze and compare the outcomes of ossiculoplasty in terms of hEaring results, intra and post operative course, using autograft incus and titanium Middle Ear Prosthesis. Materials and Methods Patients with a history of chronic Ear discharge and conductive deafness were included in the study. Out of 21 patients with ossicular chain defect included in the study, 10 patients underwent ossiculoplasty with autograft incus and 11 with titanium Prosthesis. Pure tone audiogram was done after three months. Result In patients with incus ossiculoplasty, average Post operative PTA was 43.5 dB ± 7.934SD and Net gain in hEaring was 10.7 decibels ± 15.478SD. In patients with titanium Prosthesis ossiculoplasty, average Post operative PTA was 41.4 dB ± 4.789SD and Net gain in hEaring was 16 decibels ± 11.981SD. Discussion Three patients underwent only Tympanoplasty post operative PTA was 42 ± 5.292 dB (incus- 39 dB, titanium- 48dB) and hEaring gain was 13.33 ± 12.583 (incus-20dB, titanium-0dB). Five patients underwent intact canal wall with Tympanoplasty: Post operative PTA was 35 ± 3 dB (incus-35dB, titanium-30dB) and hEaring gain was 23 ± 15.379 (incus-18.33dB, titanium-30dB). Thirteen patients underwent canal wall down procedure with Tympanoplasty: Post operative PTA was 45.67 ± 5.228 dB (incus-50.4dB, titanium-41.8dB) and hEaring gain was 9.33 ± 12.309 (incus-2.4dB, titanium-14.285dB). Complications in the short period studied were nil in both groups. Conclusion Incus and titanium have equal postoperative hEaring but in terms of hEaring gain Titanium Prosthesis gave a better hEaring gain than incus

    In this experimental animal study, a cartilage disk was interposed between a synthetic Middle Ear Prosthesis and the tympanic membrane in guinea pigs to investigate its effect on the extrusion process of the implant. Two groups of guinea pigs were studied. One group consisted of animals in which the Prosthesis was directly in contact with the tympanic membrane, and the other group consisted of animals in which a cartilage disk had been inserted between the head of the Prosthesis and the tympanic membrane. Before histologic processing, in situ inspection was performed with an operating microscope. After fixation and embedding, light microscopic and transmission electron microscopic examination were performed. We studied the histopathologic aspects of the tympanic membrane with regard to the protrusion and extrusion processes of the Middle Ear implant. In this experimental model, protrusion and extrusion of a hydroxylapatite Middle Ear Prosthesis was greatly reduced by interposition of a cartilage disk. Further clinical evaluation of these experimental results is needed in the human Middle Ear

    Middle Ear ossicular reconstruction is a challenging task for any otologist. Over a period of time, surgeons have tried various types of materials as ossicular Prosthesis with varying amount of success. In the last decade, numerous studies have been conducted that have proved that titanium Prosthesis is biocompatible. We have conducted this study to investigate the efficiency of titanium Middle Ear Prosthesis (Eon Meditech Pvt. Ltd.) in tympanoplasty. Nineteen patients who underwent tympanoplasty with titanium Prosthesis placement from January to October 2010 were included in the study. Fifteen patients underwent reconstruction using titanium PORP while four patients required titanium TORP. The average follow up period was 11.1 months. The pre-operative and post-operative PTAs and the closure of the air-bone gap (ABG) at 0.5, 1, 2, 3 KHz (as per the AAO-HNS guidelines) were analyzed. As per the Indian speech and hEaring association (ISHA) guidelines, the patients were classified into groups based on the amount of hEaring loss. The improvement in the mean PTA (AC) was 16.21 dB in the PORP group while it was 20.47 dB in the TORP group (P value < 0.05). According to ISHA guidelines, 18 out of 19 (94.8%) patients had either a normal hEaring or a mild hEaring loss post-operatively. The overall success rate (post-operative ABG ≤ 20 dB) of this series is 68.4%. In this series, there has been no extrusion of the Prosthesis. Titanium Prosthesis is delicate, easy and quick to handle. They are efficient and suitable implants for Middle Ear ossicular reconstruction. The use of cartilage between the Prosthesis and the graft helps to prevent extrusion of the Prosthesis.
