    Abstract:

    Traditional concepts of long-term residential treatment of children are adapted for use on short-term psychiatric units. A model of Milieu Therapy is proposed which emphasizes the fostering of discriminate interpersonal attachments. Specific interventions, the use of limits, and the role of other therapies are discussed in the context of this model.

    Abstract:

    The purpose of the current study was to evaluate the effectiveness of combining Milieu Therapy and functional communication training (FCT)] to replace aberrant behavior with functional communicative skills in 3 male preschool or elementary aged children with Autism Spectrum Disorders (ASD). Study activities were conducted in the natural environments of the participants and parents acted as change agents. A concurrent multiple baseline design across participants was used to evaluate the effectiveness of the modified Milieu Therapy intervention. Results indicate that aberrant behavior decreased concurrent with an increase in total percentage of communication responses (PCR). The children maintained communication and low rates of aberrant behavior, and generalized their communication from the home to the classroom. A discussion of limitations and future research directions is included.

    Abstract:

    Several researchers have employed Milieu Therapy to address the communication needs of children with autism spectrum disorder (ASD). Thus, the purpose of this review is to examine Milieu Therapy, particularly, the environments and individuals involved in the training and the effectiveness of Milieu Therapy with children who have a diagnosis of ASD and to provide suggestions for practitioners and researchers. Milieu Therapy consistently increases communication and generalizes to other settings, while maintaining over time; however, Milieu Therapy does not analyze connections to challenging behaviors and few studies have focused on teachers in the classroom or peers. Future research teams should continue to train parents and teachers while addressing the connection to challenging behavior and including peers in the interventions.
