    Abstract:

    Rheumatoid arthritis (RA) is a systemic and chronic autoimmune inflammatory disease characterized by severe synovial hyperplasia associated with progressive cartilage degradation. Due to the severe pain and disability caused by RA, effective therapeutic strategies that could simultaneously alleviate the inflammatory response and delay the disease progression are urgently needed. As a major alternative therapy in traditional Chinese medicine, Moxibustion has been demonstrated that it could reduce the chronic inflammatory responses of a series of musculoskeletal diseases; however, whether Moxibustion has protective effects on RA is still unclear. To investigate the effects of Moxibustion on RA, Moxibustion was applied to Zusanli (ST36) and Shenshu (BL23) acupoints in a RA rabbit model. HE staining of articular cartilage showed that Moxibustion alleviated the cartilage degradation and bone destruction. In addition, Moxibustion decreased the osteoclast number in RA rabbits. Real-time PCR revealed that Moxibustion decreased the expression of RANKL mRNA while increased the expression of OPG mRNA, indicating a restoration of the balance between osteogenesis and osteoclastogenesis. Taken together, our results indicated that Moxibustion had promising antiarthritic effects and could be an useful alternative method in RA therapeutics.

    Abstract:

    In the book of Chinese Acupuncture and Moxibustion by CHENG Dan-an, direct Moxibustion with moxa cone was regarded as a classic style of Moxibustion because of its innate character of heat induced thermal stimulus and burn toxin. CHENG Dan-an made the quantity of Moxibustion more controllable through the selection of moxa wool, the size and number of moxa cones, and the tightness of moxa cones, etc. CHENG Dan-an 's clinical experience is summarized through treating stubborn and infectious diseases with direct Moxibustion with moxa cone. The results showed that only with understanding the characteristics of Moxibustion and key pathological mechanism of suitable disease could direct Moxibustion with moxa cone and the appropriate methods be performed correctly. CHENG Dan-an confirmed the clinical effect of treating stubborn and intectious diseases with direct contact Moxibustion with moxa cone, and optimized the specific operation scheme of direct Moxibustion with moxa cone. It tell us that we should deeply study on Moxibustion, to make it wide spread in the hereafter, bring benefit to mankind.

    Abstract:

    WANG Ji's opinion on Moxibustion therapy in Questions and answers of acupuncture and Moxibustion (Zhenjiu Wendui) was explored and analyzed. With his two contraindications of Moxibustion as breakthrough points, the arrangement and discussion was performed. The results showed that based on summary of Moxibustion treatment theory before Ming dynasty, WANG Ji could provide characteristic idea, which is of great reference value and guide significance to the inheritance and development of Moxibustion.

    Abstract:

    Moxibustion penetration is a therapy in which the Moxibustion sensation promoted by moxa is felt deeply inside the tissues. When Moxibustion is practiced to certain extent, the sensation conducts from the Moxibustion site to the deep tissues and far ends, or muscular pulsation appears at the Moxibustion site, or the patient feels comfortable, painful, heavy and itching or the patient has reaction as skin redness and even perspiration throughout the body after the Moxibustion. The best effect of Moxibustion is expected when local site becomes slightly and evenly red or there is sweating. The observation has shown that the Moxibustion dosage is the key to enhancing the therapeutic effect.

    The authors sum up some methods for increasing therapeutic effect of Moxibustion through long-term and a great number of investigations. For Moxibustion, proper Moxibustion methods should be selected for different patient's condition, and points are selected fewer but better, accurately find response position and use enough Moxibustion amount, making the patient produce comfortable Moxibustion sensation. Put forward the concept of deqi and the method for point-through-point Moxibustion, i. e., judge Moxibustion amount, Moxibustion time and therapeutic results according to skin response of the patient after Moxibustion, and even flush and sweating on local skin after Moxibustion are used for standard of point-through-point Moxibustion. The present article specially indicates that Moxibustion-box Moxibustion is of advantages of convenient manipulation, easily control of temperature and high safety, and can be widely applied clinically.

    YANG Ji-zhou, a famous scientist of acupuncture and Moxibustion in the Ming Dynasty. He was a great master not only in manipulation methods of acupuncture, but also in Moxibustion therapy, with unique and profound deep cognition. His main contributions include the three aspects: (1) collected a great deal of methods and experience of Moxibustion treatment from Moxibustion physicians and gathered and mustered a great works about Moxibustion methods before the Ming Dynasty; (2) combined with his own clinical experiences, systematized the methods and techniques of Moxibustion, including the relationship between posture and point-fathoming, the treatment orders, the size of moxa cone, the number of moxa cones, the techniques of burning moxa, the methods for vesiculation Moxibustion, etc. forming a more systematic manipulation standard; (3) invented and formed YANG's characteristics of clinical Moxibustion treatment, i. e selecting a few points to dredge the meridian-qi, grasping opportunity of Moxibustion, combining acupuncture and Moxibustion with medicine, and consolidating the therapeutic effect by proper diet after treatment.
