    Abstract:

    Diabesity is often associated with subfertility and recurrent miscarriages. Evidence links systemic and local uterine cytotoxicity to the pathogenesis of implantation failure (IF) in diabetes. Immunosuppression with tacrolimus improved pregnancy outcomes in obese and diabetic mice and repeated IF in women with elevated Th1/Th2 blood cell ratios. However the mode of action of tacrolimus in protecting against IF and the molecular mechanisms associated with recurrent miscarriages in the obese and diabetic subjects are yet to be elucidated. Here we administered tacrolimus (FK506) (0.1 mg/kg) for four consecutive weeks to the NONcNZO10/LtJ mice, a model of human PCOS, chronically fed with 60% kCal fat for 16 consecutive weeks to simulate human obesity-associated T2DM. Compared to those immunosuppressed with tacrolimus and their normative controls, high-fat fed (HFD) diabetic NONcNZO mice exhibited higher rates of peri- and post-implantation resorption and had aberrant expression of uterine IFNγ and progesterone receptor (PGR) and its immunophilin co-chaperone FKBP52 at Nidation. Immature uterodomes and lack of activation of uterine STAT3 and NFκB at implantation were characteristics of IF in the HFD-dNONcNZO dams also low in the deciduogenic factors IL11 and GM-CSF. Therapeutic interventions with tacrolimus or metformin normalized the expression of decidual IFNγ, PGR and FKBP52, increased co-localization of protein inhibitor of activated STATy (PIASy) to PGR and resulted in the upregulation of uterine IL11and LIF. Rescued phosphorylation of STAT3 and NFκBp65 and uterodome maturation at Nidation defined implantation success in treated dams. To our knowledge this is the first report to show that the impact of HFD on the hemochorial implantation is at least in part mediated through disruption of PGR signaling at Nidation and that immunosuppression with tacrolimus or treatment with metformin restores PGR-mediated influences during implantation in the obese and diabetic subjects.

    Abstract:

    Objective To investigate the fertility of rabbits after tubal injection of methotrexate (MTX). Design The side of injection of MTX was randomly decided in a prospective experimental trial. Participants Fourteen female New Zealand White Rabbits were believed to be of normal reproductive status. Interventions Methotrexate or Ringer's solution were injected into the tubes of the rabbits. The Nidation index was used to measure the fertility of the animals. Results The average Nidation index of the control tubes was not statistically different of the one of the tubes treated with MTX. Conclusion Tubal injection of MTX has no adverse effect on fertility of rabbits. It seems ethical to investigate tubal injection of MTX as an alternative to laparoscopic management of unruptured ectopic pregnancy in the human.

    Abstract:

    Abstract The uterine luminal epithelium during the period of receptivity for Nidation displays characteristic protrusions of the apical surface named pinopodes. The effects of oestradiol and progesterone, singly or in combination, on the formation and regression of pinopodes were investigated using scanning electron microscopy. The appearance of pinopodes was found to be strictly progesterone dependent. When given together with progesterone, before the development of pinopodes, high doses of oestradiol (plasma level approximately 300 pmol/l) inhibited pinopode formation; on the contrary, low doses of oestradiol (nidatory doses) did not interfere with the process until the 4th day of treatment. When oestradiol was given as a single injection, after pinopode formation, both doses were equivalent in inducing their regression 48-72 h later. It appears that the hormonal conditioning for pinopode formation and for the development of uterine receptivity for egg implantation is the same. These observations support the hypothesis that pinopodes could be an extremely useful tool to estimate uterine receptivity. The experiments we describe here, together with observations made a few years ago, in stimulated cycles in the human, suggest that implantation failure as a result of a hormonal imbalance during the time intervening between ovulation and Nidation, may be a general phenomenon.

    Autonomic nerves controlling uterus of adult Wistar rats were cut off by surgical operation.The uterine tissue was collected,and mast cells were observed and counted by histochemistry.The results showed:1.Most of the experiment rats whose autonomic nerves controlling uterus were cut off couldn′t pregnancy,Nidation was delayed in the other rats.Cutting off the nerves of rats after pregnancy didn′t affect keeping of pregnancy. 2.After the nerves being cutted off,the total number of rats uterine MCs was greatly increased,but difference was not significant by t test.It indicates that the nerves may not totally affect Nidation by change of the number of MCs,maybe,by the change of releasing of mediator in MCs or result of species differential.The results reveal that autonomic nerves controlling uterus may play a certain role in setup of Nidation,and wasn′t necessary in keeping of pregnancy.The effect mechanism of autonomic nerves on Nidation needs to be studied deeply.
