A review of the MAFF Optimal Nutrition Status research programme: folate, iron and copper.
Public health nutrition, 2002Co-Authors: Judy Buttriss, J HughesAbstract:OBJECTIVE The objective was to conduct a critical appraisal of research conducted within one of the UK government's research programmes, Optimal Nutrition Status, and to place the findings of this work in the context of the international research effort, to assist policy makers and advisers. Nine nutrients are addressed within the programme; the findings for three of these are reported here: folate/folic acid, iron and copper. DESIGN To conduct the review, the researchers had access to all unpublished progress reports, submitted to officials, arising from the projects. The overall assessment criterion was whether the information generated by the research programme could be regarded as reliable experimental data of direct relevance to setting Optimal dietary requirements for the particular micronutrients. However, findings were also assessed against specific scientific criteria concerning understanding of the bioavailability, interactions, development of functional markers and inter-individual variations in metabolism, for each of the nutrients scrutinised. RESULTS The results of the review indicated that many important questions are indeed being addressed by the UK government's research programme, and that the work is contributing to the overall research effort being conducted world-wide on this important subject. CONCLUSIONS Many major questions still need to be addressed before it will be possible to identify Optimal intakes for various sub-populations. These priorities are summarised in the paper.

early Optimal Nutrition improves neurodevelopmental outcomes for very preterm infants
Nutrition Reviews, 2014Co-Authors: Chienchou Hsiao, Mingluen Tsai, Chihchen Chen, Hungchih LinAbstract:Recent advances in perinatal and neonatal intensive care have resulted in significant improvements in the survival of preterm extremely low-birthweight (PELBW) infants; however, extrauterine growth restriction (EUGR) and underNutrition occur frequently during hospitalization and are associated with adverse outcomes, including bronchopulmonary dysplasia, sepsis, and neurodevelopmental impairment. Early Optimal parenteral Nutrition with adequate amino acids and lipids, especially long-chain polyunsaturated fatty acids, has been shown to decrease the incidence of EUGR, bronchopulmonary dysplasia, necrotizing enterocolitis, sepsis, and retinopathy of prematurity in animal models and clinical trials. In PELBW infants, breast milk and probiotics have been shown to reduce the incidence of necrotizing enterocolitis, and lactoferrin has been demonstrated to prevent late-onset sepsis. Thus, early administration of Optimal postnatal parenteral and enteral nutrients can help prevent neurodevelopmental impairment caused by EUGR, necrotizing enterocolitis, sepsis, bronchopulmonary dysplasia, and retinopathy of prematurity, and recent evidence indicates such treatment is feasible.

“There’s a lot of talent in the room but it’s only really the medical talent that gets heard”: a qualitative exploration of multidisciplinary clinicians’ perspectives of Optimal Nutrition care of patients with head and neck cancer

The best-available evidence has been published in clinical Nutrition guidelines; however, translation into practice has lagged. Aim This project aimed to explore multidisciplinary team (MDT) clinicians’ perspectives regarding barriers and enablers to best-practice Nutrition care in order to inform the design of a new model of care. Method Qualitative interviews were conducted with clinicians who were purposively sampled from a major HNC tertiary referral centre in Sydney, Australia. To elicit information regarding barriers and facilitators to change, a semi-structured interview schedule was developed, interviews were transcribed verbatim and analyzed employing an inductive thematic approach. The Consolidated Framework for Implementation Research (CFIR) was used to guide data analysis and interpretation of key themes identified. Results Nineteen participants (11 supportive care and eight medical clinicians) representing allied health, medical, and nursing disciplines participated. Five key themes were identified: (1) acknowledgement of dietetics expertise and access to resources to deliver Nutrition care; (2) proactive versus reactive Nutrition care; (3) integrated and coordinated care—“The One Stop Shop”; (4) MDT favours the medical model; and (5) leadership—within disciplines, within the MDT. Conclusions MDT clinicians expressed similar views regarding delivering Optimal Nutrition care to this high Nutritional risk patient group. However, perspectives differed at times between medical and supportive care clinicians, attributable to perceptions that current service structure favours the medical model. In order to design and deliver an evidence-based model of care, specific strategies will be required to ensure: early and ongoing access to expert Nutrition care; Nutrition care processes are proactive; integrated and coordinated care; and leadership, both intra- and inter-disciplinary. This novel exploration of MDT clinicians’ views provides supporting evidence that multi-component implementation strategies comprising individual, team and system-level approaches will be essential to leverage sustainable change.

Selected macro/micronutrient needs of the routine preterm infant.

The "reference fetus," developed by Ziegler et al,(1) has served as a model to define Nutritional needs and studies designed to determine nutrient requirements. Revision of nutrient requirements and provision of Optimal Nutrition may lead to improved outcomes in preterm infants. Appropriate provision of nutrients also may help prevent Nutritional disorders, such as metabolic bone disease and anemia. In this review, we discuss calcium, phosphorus, magnesium, vitamin D, iron, and copper, and define Optimal intakes based on the available published data.
