    AbstractObjective: To investigate the effect of bracket–ligature combination on the amount of Orthodontic Space Closure over three months.Design: Randomized clinical trial with three parallel groups.Setting: A hospital Orthodontic department (Chesterfield Royal Hospital, UK).Participants: Forty-five patients requiring upper first premolar extractions.Methods: Informed consent was obtained and participants were randomly allocated into one of three groups: (1) conventional pre-adjusted edgewise brackets and elastomeric ligatures; (2) conventional pre-adjusted edgewise brackets and Super Slick® low friction elastomeric ligatures; (3) Damon 3MX® passive self-ligating brackets. Space Closure was undertaken on 0·019×0·025-inch stainless steel archwires with nickel–titanium coil springs. Participants were recalled at four weekly intervals.     Abstract:

    The congenitally missing maxillary lateral incisor is the most common agenesis in the anterior region. There are several treatment options for this anomaly, which causes severe deficiencies: Orthodontic Space Closure, tooth-supported restoration, or single-tooth implant. Each of these solutions has a high degree of success if used in the correct situation. An implant-supported restoration with an interdisciplinary approach provides a predictable outcome. This article describes the treatment of a patient with agenesis of the maxillary left lateral incisor. After Orthodontic Space management, it was decided to restore the tooth with an all-ceramic crown cemented on a zirconia custom abutment, which fractured after only 6 weeks of service. Fractographic analysis revealed that the failure was due to over-reduction of the buccal wall to correct the labial emergence of the implant. Zirconia abutments should be designed with even wall thicknesses of at least 0.8 mm to avoid areas that may compromise functional success.

    Abstract:

    Objective To investigate the effect of immediate bone grafting at mandibular first molar extraction socket on maintaining alveolar bone height after Space Closure. Methods Thirty adult Orthodontic patients who need to extract mandibular first molar, totally 38 target teeth, were included. The samples were divided into two groups randomly: graft group and non-graft group. All extraction Space was closed Orthodontically. Dental models of all patients were taken before extraction (T0), before Space Closure(T1) and after Space Closure (T2). The distance, time of the Space Closure and the velocity of tooth movement were recorded. Probing depth (PD) and clinical attachment level (CAL) at six sites (mesial buccal, buccal, mesial lingual, lingual, distal buccal and distal lingual) on adjacent teeth were measured before extraction (T0) and after Space Closure (T2). Cone-beam CT (CBCT) was taken at T0 and T2 to compare the changes of alveolar bone height at six sites on adjacent teeth using Invivo Dental 5.0 software. Results The extraction Space in both graft group and non-graft group was closed successfully. However, the Space in graft group was closed more slowly than in non-graft group. In graft group, PD and CAL at the six sites on both the second molar and the second premolar did not change significantly after Space Closure, and CBCT showed that the alveolar bone height of the second premolar had no significant difference after treatment. In non-graft group, alveolar bone height decreased in both adjacent teeth and periodontal attachment loss was found after Space Closure. On average, alveolar bone height and periodontal attachment of the second premolar decreased (0.75±0.16) mm and (0.64±0.15) mm, respectively. Meanwhile, alveolar bone height and periodontal attachment of the second molar decreased (0.79±0.23) mm and (0.80±0.24) mm, respectively. Conclusions Bone graft immediately after mandibular first molar extraction could delay alveolar bone resorption and preserve the periodontal attachment of the adjacent teeth during Space Closure.     Abstract:

    Objective To construct a 3-D finite element model with dentition, PDL, maxilla and customized lingual appliance used in Space Closure, and further biomechanic research for lingual Orthodontics can be based on this model.Methods After CT scaning, Mimics 10.01, Geomagic Studiao 9 and ANSYS 10.0 were employed to develop a 3-D finite element model with dentition, PDL, maxilla and customized lingual appliance.Results 38 volume models were involved in the constructed 3-D modle, and the model had a good similarity to the clinic.The total number of the nodes was 471, 208 , and the muber of the elements was 29774.Conclusion The 3-D finite element model for the customized ligual appliance constructed by CT scanning and Computer Aided Design technology in combination with finite element analysis is feasible to resembling the clinical situation, and can be a platform for the further research of biomechanic for the lingual orthodontcis.     Aims: To compare the analgesic effect of topical benzocaine (5%) and ketoprofen (1.60 mg/ml) after 2 mm activation of 7 mm long delta loops used for maxillary en-masse Orthodontic Space Closure. Subjects and methods: Twenty patients (seven males, 13 females, 15–25 years of age, mean age of 19.5 years) participated in a randomised crossover, double-blind trial. After appliance activation, participants were instructed to use analgesic gels and record pain perception at 2, 6, 24 hours and 2, 3 and 7 days (at 18.00 hrs), using a visual analogue scale ruler (VAS, 0–4). Each patient received all three gels (benzocaine, ketoprofen, and a control (placebo)) randomly, but at three different appliance activation visits following a wash-over gap of one month. After the first day, the patients were instructed to repeat gel application twice a day at 10:00 and 18:00 for three days. The recorded pain scores were subjected to non-parametric analysis. Results: The highest pain was recorded at 2 and 6 hours. Pain scores were significantly different between the three groups (Kruskal–Wallis test, p 0.05). Conclusion: A significant pain reduction was observed following the use of ketoprofen when tested against a control gel (placebo). The highest pain scores were experienced in patients administered the placebo and the lowest scores in patients who applied ketoprofen gel. Benzocaine had an effect mid-way between ketoprofen and the placebo. The highest pain scores were recorded 2 hours following force application, which decreased to the lowest scores at after 7 days.
