    Author: Andrea Kohútová Title: A case Study of Physiotherapy Treatment Concerning a Patient with Transfemoral Amputation Objectives: The main goal of this bachelor thesis is to create a case study of physiotherapy treatment concerning a patient with transfemoral amputation and to summarize the theoretical knowledge base corresponding with this diagnosis, its causes and possibilities of rehabilitation treatment. Methods: This bachelor thesis is split into two parts, a theoretical one and a practical one. The main focus of the theoretical part is to establish a brief summary of the anatomy and kinesiology of the lower limb, the theoretical information about amputation and their etiology, the operation techniques and after-surgery complications. It also contains some general findings about lower limb ischemic disease. The first part focuses on the problematics concerning Orthopedic Prosthetics, subsequent rehabilitation care and physiotherapeutic methods. The second part contains a detailed case study of physiotherapy treatment concerning a patient with transfemoral amputation. This case study includes the anamnesis of the patient, a detailed initial and final complex kinesiology examination, the proposal and aims of the therapy, records of performed individual therapeutic units and an afterwards..

Cíl: Cílem bakalářské práce je zpracování kazuistiky fyzioterapeutické péče o pacienta s diagnózu jednostranné nadkolenní amputace a shrnutí teoretických znalostí souvisejících s touto diagnózou, jejími příčinami a s možnostmi rehabilitační léčby. Metody: Bakalářská práce je zpracována ve dvou částech, teoretické a praktické. Úkolem teoretické části je stručně shrnout anatomii a kineziologii dolních končetin, teoretické informace o amputacích a jejich etiologii, operačních technikách a pooperačních komplikacích. Jejím obsahem jsou i obecné poznatky o ischemické chorobě dolních končetin. Dále se první část věnuje problematice ortopedické protetiky, následné rehabilitační péči a fyzioterapeutickým postupům a metodám. Druhou, praktickou částí, je podrobně zpracovaná kazuistika pacienta s diagnózou jednostranné nadkolenní amputace. Součástí kazuistiky je anamnéza pacienta, vstupní a výstupní kineziologický rozbor, návrh a cíle terapie, záznam průběhu samotných terapeutických jednotek, a na závěr zhodnocení efektu terapie. Výsledky: V průběhu dvoutýdenní spolupráce s pacientem jsme pozitivně ovlivnili jeho zdravotní stav. Došlo ke zlepšení kondice a zvýšení svalové síly.     Abstract:

    The bachelor thesis deals with the life quality of patients, who have undergone amputation of the lower limb. Theoretical part of the thesis explains the concept and describes the issue of amputation, presents the options of psychological and social support and Orthopedic Prosthetics. Further, it characterizes the nursing care of patients after amputation, clarifies the concept of life quality and summarizes current research on this topic. In the empirical part, using a qualitative investigation, the thesis look into the life quality of five specifically selected respondents after amputation of lower limb. All five respondents undergone amputation due to complications in connection with diabetes mellitus. Amputation in the lower leg was made in two cases, amputation in the thigh was made in two cases and amputation of first leg in the lower leg and second leg in the thigh in one case. Health status of patients before amputation is being mapped, psychosomatic impacts on the patients are evaluated, subjective experience and social context of altered life quality is analyzed. The results are compared with the domestic and foreign studies in this field. V teoretické části objasňuje pojem amputace, popisuje problematiku amputace, možnosti psychologické a sociální opory a ortopedické protetiky. Dále charakterizuje ošetřovatelskou péči u pacientů po amputaci, objasňuje pojem kvalita života a shrnuje dosavadní výzkumy na toto téma. V empirické části pomocí kvalitativního šetření zkoumá kvalitu života po amputaci dolní končetiny u pěti cíleně vybraných respondentů. Všichni prodělali amputaci z důvodu komplikací, které provázejí diabetes mellitus.     Abstract:

    The complexity of demands in the healthcare field requires greater plasticity in management actions involving multifaceted and trans-disciplinary approaches. The creation of spaces promoting the development and analysis of healthcare information constitutes an innovation strategy, especially when promoting the systematization of knowledge in everyday life. National policy concerning health information enabled the emergence of the OTICS - Observatory of Information Technology and Communication in Health Services and Systems (Observatório de Tecnologias de Informação e Comunicação em Sistemas e Serviços de Saúde - OTICS) project. The focus of OTICS lies in supporting new forms of healthcare management by using information to strengthen healthcare practice. The Conceição Hospital Group School Station (CHG-SS) [Estação Escola Grupo Hospitalar Conceição (EE-GHC)] was created within the scope of the OTICS project. This paper aimed to analytically describe an experiment performed in partnership with OTICS and the CHG [GHC] School. First, a conceptual approach was used to address the challenges of performing in highly complex, specialized scenarios, and the potential innovations of using information and associated technologies in everyday health work and management. Two different experiences were analyzed regarding the potential use of information in health surveillance technology development, focusing on the concept of holistic health: the creation of a program for health education and promotion related to toxicological events in children (PEPSET) and another based on the construction of a monitoring and tracking system for Orthopedic Prosthetics. Regarding PEPSET, multifocal action was taken by training community health agents and other healthcare professionals. As a result, ANVISA (National Agency of Health Surveillance) [Agência Nacional de Vigilância Sanitária]  approved the methodology, making it viable for disseminating the experiment in other locations and supporting the evaluation mechanism of actions that are still under development. Since its implementation, CHG-SS [EE-GHC] has managed not only to increase the use of information for the qualification of hospital services but also to create a space for the development and analysis of projects. In doing so, it has contributed robust data destined to improve the performance and management of health surveillance within an inter-institutional network and directed toward quality in individual and collective care. In this case, integration is strengthened by inter-institutional articulation with the formation of closer structural and practical bonds between health surveillance and assistance, with the production of useful knowledge for everyday work and prospection in health surveillance. Above all, this takes place by strengthening the role that permanent education plays in healthcare.A complexidade das demandas da área da saúde requer cada vez mais plasticidade no modo de gerir, com abordagens multifacetadas e transdisciplinares. A criação de espaços que promovam o desenvolvimento e analise de informação em saúde é estratégia de inovação, principalmente aqueles que promovem a sistematização do conhecimento no cotidiano. A política nacional na área de informação em saúde viabilizou o surgimento do projeto Observatório de Tecnologias de Informação e Comunicação em Sistemas e Serviços de Saúde (OTICS). O apoio às novas formas de gestão em saúde, através do uso da informação para o fortalecimento de práticas em saúde é o foco do OTICS, onde foi criada a Estação Escola Grupo Hospitalar Conceição (EE-GHC). Neste artigo fez-se uma análise descritiva da experiência realizada na parceria entre o OTICS e a Escola GHC, utilizando-se inicialmente uma abordagem conceitual relativa aos desafios da atuação em cenários especializados e de grande complexidade e das potencialidades à inovação do uso da informação e suas tecnologias no cotidiano do trabalho e da gestão em saúde. Duas experiências foram analisadas e colocam em questão a potencialidade de uso da informação no desenvolvimento de tecnologias de vigilância sanitária mais voltadas à ideia da integralidade na saúde: a criação de um programa de educação e promoção em saúde relacionado com eventos toxicológicos (PEPSET) em crianças e outra na construção de sistema de monitoramento e rastreamento de próteses ortopédicas. Quanto ao PEPSET foi implementada uma ação multifocal capacitando agentes comunitários de saúde e demais profissionais da saúde. Como resultado, a ANVISA aprovou a metodologia sendo viável para disseminação da experiência em outras localidades e incentivou o mecanismo de avaliação dessas ações que ainda estão em desenvolvimento. A EE-GHC desde a sua implementação conseguiu não somente potencializar o uso da informação para a qualificação dos serviços hospitalares, mas também produziu espaços de desenvolvimento e analise de projetos contribuindo para a obtenção de dados robustos destinados ao fortalecimento da gestão e da atuação em vigilância sanitária, em rede interinstitucional e voltada à qualidade da atenção aos indivíduos e coletividades. A integralidade, nesse caso, se fortalece com a articulação interinstitucional, com a aproximação de estruturas e práticas da vigilância sanitária e da assistência, na produção de conhecimentos úteis ao cotidiano do trabalho e na prospecção em vigilância sanitária e, sobretudo, no fortalecimento da educação permanente em saúde.Palavras-chave: Ambientes híbridos, Informação em Saúde, Redes de Conhecimento, Observatório de Saúde

    Locked-in syndrome (LIS) is a rare condition character-ised by quadriplegia and anarthria and is usually caused by a bilateral ventral ischemic pontine lesion. Patients are normally fully conscious, but their only mode of com-munication is with vertical eye movements and/or blink-ing. Although the mortality rate is high, it has been shown that patients can survive for a significant period of time. Once an LIS patient becomes medically stable, given ap-propriate medical care, his or her life expectancy may be several decades. LIS patients may suffer appreciably if they are treated by hospital staff as nonresponsive.
