The use of short message service (SMS) to reduce outPatient Attendance in ophthalmic clinics during the coronavirus pandemic

Actively calling Patients to postpone appointments is a labour-intensive process. We were the first ophthalmic clinics in Hong Kong to use short message service (SMS) to dispatch messages simultaneously to a large number of Patients to offer postponement of appointments. The aim of this study is to evaluate whether SMS is an effective method to reduce outPatient Attendance during the COVID-19 pandemic. Methods This is an observational study reviewing data on SMS messages sent to all Patients attending ophthalmology clinics of a tertiary eye centre in Hong Kong. All SMS were sent at least 5 days before the scheduled appointments. The text message included an enquiry hotline for postponement of appointments and offered drug refill. The study included data from February to April 2020. Two hundred Patients were invited to take part in a questionnaire on satisfaction level and reason(s) for appointment rescheduling. Results During the study period, a total of 17,028 SMS were sent. The overall response rate was 23.6%. 14.3% postponed their appointments. This led to an overall 13.9% reduction of clinic Attendance. The overall satisfaction was high (96%). The main reason for postponing appointment was worries about infection risk (93.1%). Conclusion SMS was an efficient and cost-effective flow-control method which was well accepted by Patients and can reduce outPatient Attendance. The time saved can potentially allow healthcare workers to conduct other infection control measures during the COVID-19 pandemic.

Patient Attendance and Outcomes in a Structured Weight Management Program

DavisAbstract:Objectives To determine service utilization and identify Patient characteristics associated with service utilization in youth with obesity presenting for structured weight management, and to explore weight-related outcomes associated with service utilization. Study design In this retrospective study conducted between January 2008 and December 2013, we examined variables associated with the care of 2089 Patients aged 2-18 years presenting for an initial visit to 2 tertiary care-based, multidisciplinary structured weight management clinics. Results Only 53% of Patients returned for a second visit, 29% returned for a third visit, and virtually none (0.5%) completed the recommended 6 visits within 6 months. Patients who were Hispanic, government-insured, and whose parent/s spoke Spanish were more likely to return to clinic. Of those Patients who returned for at least a second visit, 70% demonstrated a reduction in or maintenance of body mass index z -score. Conclusions Patient retention remains a significant barrier to effective pediatric weight management. Structured weight management programs should increase their efforts to engage Patients and families at the initial visit and identify and address barriers to follow up.

Factors Impacting Physician Referral To and Patient Attendance at Weight Management Programs Within a Large Integrated Health System

RothbergAbstract:Background Identifying which Patients receive referrals to and which ones attend weight management programs can provide insights into how physicians manage obesity. Objective To describe Patient factors associated with referrals, which primarily reflect physician priorities, and Attendance, which reflects Patient priorities. We also examine the influence of the individual physician by comparing adjusted rates of referral and Attendance across physicians. Design Retrospective cohort study. Participants Adults with a body mass index (BMI) ≥ 30 kg/m^2 who had a primary care visit between 2015 and 2018 at a large integrated health system Main Measures Referrals and visits to programs were collected from the EHR in 2019 and analyzed in 2019–2020. Multilevel logistic regression models were used to identify the association between Patient characteristics and (1) receiving a referral, and (2) attending a visit after a referral. We compared physicians’ adjusted probabilities of referring Patients and of their Patients attending a visit. Key Results Our study included 160,163 adults, with a median BMI of 35 kg/m^2. Seventeen percent of Patients received ≥ 1 referral and 29% of those attended a visit. The adjusted odds of referral increased 57% for Patients with a BMI 35–39 (versus 30–34) and 32% for each comorbidity ( p < 0.01). Attending a visit was less strongly associated with BMI (aOR 1.18 for 35–39 versus 30–34, 95% CI 1.09–1.27) and not at all with comorbidity. For the physician-level analysis, the adjusted probability of referral had a much wider range (0 to 83%; mean = 19%) than did the adjusted probability of Attendance (range 27 to 34%). Conclusions Few Patients attended a weight management program. Physicians vary greatly in their probability of referring Patients to programs but not in their Patients’ probability of attending.

Hepatic venous Doppler in the evaluation of fetal extrasystoles.

Furthermore, to estimate the prevalence and persistency of atrial and ventricular extra beats throughout pregnancy and delivery, and explore the coexistence of congenital heart disease. METHODS: Retrospective study of 256 singleton pregnancies on out-Patient Attendance due to fetal extra beats. Hepatic venous Doppler and detailed fetal echocardiography were performed. Fetal heart rate patterns during labor and neonatal conditions were collected. Congenital heart malformations and the frequency and persistence of fetal extrasystole were noted. RESULTS: 228 (89%) showed signs of supraventricular extrasystole (SVES) and 19 (11%) had ventricular extrasystole (VES). One fetus with SVES developed atrial flutter during pregnancy and another supraventricular tachycardia postnatally. SVES persisted in 28 fetuses until labor and delivery (12.3%) and in 6 with VES (21.4%). In 31 of 34 having extra beats during labor and delivery, the conduction pattern normalized within three days (91.2%).Five neonates were referred for evaluation by a pediatric cardiologist.
