    An explosion of new hospital building has created the opportunity for nurse leaders to transform the Patient Care experience with evidence-based architecture, technology innovations, and new Patient Care delivery models. The authors share the first-year results of the creation of a hospital of the future in which staff actively participated and addressed the challenges of transforming the Patient Care experience. Positive results include Patient satisfaction at the 99th percentile, successful integration of 63 software applications, and energized nursing staff.

    A residency requirement has been proposed as a prerequisite for all pharmacists providing direct Patient Care. This editorial explores the basis for requiring a residency, direct Patient Care offered by pharmacists with or without a residency, needs to increase interprofessional team-based health Care, importance of distinguishing levels of pharmacy practice, role of pharmacy technicians, availability of residency positions, and future of pharmacy residencies. All PharmD graduates should be able to provide a certain level of direct Patient Care, including medication therapy management services, without completing a residency, and residency programs should be offered for complex levels of direct Patient Care in all practice settings.

    Abstract:

    Integration of Care is emerging as a central challenge of health Care delivery, particularly for Patients with multiple, complex chronic conditions. The authors argue that the concept of "integrated Patient Care" would benefit from further clarification regarding (a) the object of integration and (b) its essential components, particularly when constructing measures.To address these issues, the authors propose a definition of integrated Patient Care that distinguishes it from integrated delivery organizations, acknowledging that integrated organizational structures and processes may fail to produce integrated Patient Care. The definition emphasizes Patients' central role as active participants in managing their own health by including Patient centeredness as a key element of integrated Patient Care. Measures based on the proposed definition will enable empirical assessment of the potential relationships between the integration of organizations, the integration of Patient Care, and Patient outcomes, providing valuable guidance to health systems reformers.
