    Abstract:

    Leprosy affects skin and peripheral nerves, and acute inflammatory type 1 reactions (reversal reaction) can cause neurologic impairment and disabilities. Single skin lesion Paucibacillary Leprosy volunteers (N = 135) recruited in three Brazilian endemic regions, treated with single-dose rifampin, ofloxacin, and minocycline (ROM), were monitored for 3 years. Poor outcome was defined as type 1 reactions with or without neuritis. IgM anti-phenolic glycolipid I, histopathology, Mitsuda test, and Mycobacterium leprae DNA polymerase chain reaction (ML-PCR) were performed at baseline. chi(2) test, Kaplan-Meir curves, and Cox proportional hazards were applied. The majority of volunteers were adults with a mean age of 30.5 +/- 15.4 years; 44.4% were ML-PCR positive. During follow-up, 14.8% of the patients had a poor clinical outcome, classified as a type 1 reaction. Older age (> or = 40 years), ML-PCR positivity, and lesion size > 5 cm were associated with increased risk. In multivariate analysis, age (> or = 40 years) and ML-PCR positivity remained baseline predictors of type 1 reaction among monolesion Leprosy patients.

    Peu d'informations existent au Bresil en ce qui concerne les caracteristiques cliniques et epidemiologiques des patients qui presentent une lesion cutanee unique paucibacillaire (LCU-PB). Il n'y a que tres recemment que le systeme officiel de depistage a distingue les patients hanseniens presentant un seule lesion comme une entite clinique distincte, pour lesquels une administration unique de rifampicine, ofloxacine et minocylcine (ROM) a ete recommandee. Nous decrivons ici les caracteristiques cliniques et immunologiques d'une cohorte de LCU-PB selectionnee entre decembre 1997 et decembre 1998. Les patients furent recrutes dans des centres de soins localises dans les regions suivantes: sud-est = Rio de Janeiro; nord = etats de Rondonia et de l'Amazone; centre ouest = etat de Gaia. Les cas selectionnes etaient des patients hanseniens nouvellement detectes, non-traites, presentant une lesion unique sans evidence de lesion d'epaississement des nerfs. Ces cas furent evalues par des examens cliniques, bacilloscopiques et histopathologiques, ainsi que des reactions de Mitsuda et des test serologiques (ELISA) anti-PGL-I. Parmi 299 patients hanseniens LCU-PB, 259 (86,6%) presentaient les criteres pour un traitement unique ROM. Nos resultats montrent que les patients enroles a partir de sites differents presentaient des caracteres similaires, si l'on considere les profils cliniques et immunologiques. Les patients etaient en majorite adultes (moyenne de 32,4 ans; D.S. = 16,0) et une cicatrice de BCG fut detectee chez 76,7% des enfants (≤15 ans). Il n'y eut que 7 cas qui furent diagnostiques comme etant du type multibacillaire, ce qui ne represente que 3% des patients qui furent re classifies a posteriori. Nos donnees suggerent que, au Bresil, l'evaluation des cas LCU-PB basee sur des criteres cliniques et bacterioscopiques peut etre definie de facon precise dans le cadre d'un programme de controle de routine; 75,0% des cas SLC-PB etaient positifs a la reaction de Mitsuda (>5 mm), et 17,3% des patients etaient seropositifs au test anti-PGL-I. Ces donnees sont compatibles avec une efficace immunite a mediation cellulaire et une charge basse en bacilles, suggerant une issue clinique favorable pour la majorite des patients LCU-PB de cette cohorte.

    Abstract:

    Five alternative methods were used to explore in vitro the effects of normal and activated murine macrophages on the metabolic well-being of intracellular Mycobacterium leprae: fluorescein diacetate-ethidium bromide staining, ATP content, synthesis of phenolic glycolipid 1, and two techniques to quantitate oxidation of palmitic acid. In relatively short-term experiments (7 to 10 days), each of these procedures provided strong evidence that activated macrophages exerted a deleterious effect on the Leprosy bacillus. These findings appear to confirm the contention that activated macrophages underlie host resistance to clinical Leprosy and limitation of M. leprae growth in Paucibacillary Leprosy.
