A Survey of Common Payment Methods and Their Determinants in Dental Clinics, in Tehran, 2018.

Although Payment systems may enhance job satisfaction, reduce the costs, and make better treatment decisions, there is little information about how to achieve these objectives. The aim of the present study was to survey the Payment Methods and the related factors in dental clinics of Tehran. Using the latest list published by the Iranian Ministry of Health, we visited all dental clinics located in Tehran, and used a checklist to collect the data of the type of management, geographical location of the clinic in Tehran, Payment Method and its amount, history of Payment Method changes in the last decade, relationship between the amount of Payment and the position of technical supervisor dentist, any difference in the amount of Payment to male and female dentists and to young and experienced dentists. Then, the relationship between the amount of Payment and the above-mentioned variables was investigated. The governmental sector tends to use fixed salary Methods and the private sector usually uses the fee-for-service (FFS) Method. Geographical location, type of management, date of establishment, and having the position of technical supervisor dentist had a significant relationship with the amount of Payment. The dentist's gender and years of experience did not have any relationship with the amount of Payment. According to the results of the present study, the Method and amount of Payment to dentists were related to the date of clinic establishment, having the position of technical supervisor dentist, and geographical location of the clinic. These factors could be considered as the main elements in balancing costs in clinics and improving job satisfaction among dentists. Dental Investigation Society.

Findings will not only aid the National Health Insurance Authority (NHIA) to improve the implementation arrangements but also provide useful inputs for other low and middle-income countries (LMICs) in their quest to reform their provider Payment systems. MATERIALS AND MethodS We conducted a cross-sectional survey of 200 credentialed health care providers' in the three regions of Ghana on providers' preference for Payment Method. We administered closed-ended questionnaires employing 5-point Likert scales for measurement of Payment Method preference. Descriptive and regression analysis were performed to examine healthcare providers' background characteristics and their association with preferred Payment Method for primary care. RESULTS In general, health care providers prefer the Ghana-Diagnosis-Related Grouping (G-DRG) Payment Method to fee-for-service and capitation Payment Methods. Effects of a fixed-Payment Method on demand for imaging tests in a university hospital

The aim of this study was to explore the effects of this Payment Method on the demand for imaging tests. Methods Data were obtained retrospectively from electronic medical records. The main outcome measures were the number of imaging tests, the Payment per outpatient and the rate of negative results of imaging tests. Results The overall mean number of imaging tests per outpatient was 0.75+0.13. This value decreased significantly after implementation of the fixed-Payment Method, and then increased after its cancellation (P , 0.01). The overall mean Payment per outpatient was $14.1+2.3. For patients with social security, there was a significant decrease in the mean Payment after implementation of the new Method, followed by an increase after its cancellation (P ¼ 0.02); in contrast, no significant changes were observed in patients not covered by social security (P . 0.05). No significant differences were observed in the rate of negative results for imaging tests (P . 0.05).
