Treatment of Pediculosis Capitis: A Critical Appraisal of the Current Literature
American Journal of Clinical Dermatology, 2014Co-Authors: Hermann FeldmeierAbstract:Pediculosis Capitis is the most common ectoparasitic disease in children in industrialized countries and extremely common in resource-poor communities of the developing world. The extensive use of pediculicides with a neurotoxic mode of action has led to the development and spread of resistant head lice populations all over the world. This triggered the development of compounds with other modes of action. The current literature on treatment approaches of head lice infestation was searched, and published randomized controlled trials were critically analyzed. The following compounds/family of compounds were identified: spinosad, a novel compound with a new neurotoxic mode of action, isopropyl myristate, 1,2-octanediol, ivermectin, plant-based products, and dimeticones. The efficacy and safety of these compounds are reviewed and recommendations for the treatment of Pediculosis Capitis in individuals as well as the interruption of ongoing epidemics are provided.

Prevalence and risk factors associated with Pediculosis Capitis in an impoverished urban community in lima, peru.

In developing countries it frequently affects children and adults, but epidemiological data at the community level are rare. Objectives: To assess prevalence and risk factors associated with Pediculosis Capitis in a resource-poor community in Lima, Peru. Materials and Methods: In total, 736 persons living in 199 households in a circumscribed neighbourhood were examined for head lice and nits by visual inspection. At the same time, socio-demographic data were collected using a structured questionnaire. Variables associated with Pediculosis were identified by performing a bivariate analysis, followed by a multivariate logistic regression analysis. Results: Prevalence of Pediculosis Capitis was 9.1% (95% confidence interval (CI): 7.0-11.2 %) in the general population and 19.9% (CI: 15.4-24.4%) in children ≤15 years of age. Multivariate analysis showed that Pediculosis Capitis was significantly associated with age ≤ 15 years (OR: 16.85; CI:7.42-38.24), female sex (OR: 2.84; CI: 1.58-5.12), household size of >4 persons (OR: 1.98; CI: 1.11-3.55), low quality of house construction material (OR:2.22; CI: 1.20-4.12), and presence of animals in the household (OR: 1.94; CI: 1.11-3.39). Conclusion: Pediculosis Capitis was a very common disease in the studied community in Lima, Peru. Our logistic regression analysis affirms that young age is the most important risk factor for Pediculosis Capitis. Moreover, female sex, large household size, living in wooden houses and the presence of animals were identified as being significantly associated with head lice infestation.

Pediculosis Capitis: new insights into epidemiology, diagnosis and treatment

Head lice are highly specialised parasites which can propagate only on human scalp and hair. Transmission occurs by direct head-to-head contact. Head lice are vectors of important bacterial pathogens. Pediculosis Capitis usually occurs in small epidemics in play groups, kindergartens and schools. Population-based studies in European countries show highly diverging prevalences, ranging from 1% to 20%. The diagnosis of head lice infestation is made through the visual inspection of hair and scalp or dry/wet combing. The optimal method for the diagnosis of active head lice infestation is dry/wet combing. Topical application of a pediculicide is the most common treatment. Compounds with a neurotoxic mode of action are widely used but are becoming less effective due to resistant parasite populations. Besides, their use is restricted by safety concerns. Dimeticones, silicone oils with a low surface tension and the propensity to perfectly coat surfaces, have a purely physical mode of action. This group of compounds is highly effective and safe, and there is no risk that head lice become resistant. The control of epidemics requires active contact tracing and synchronised treatment with an effective and safe pediculicide.

Visual inspection of 5 predilection sites (temples, behind the ears, and neck) was performed first, followed by wet combing of hair moistened with conditioner. Presence of mobile stages was defined as active infestation, presence of nits alone as historic infestation. Design Observer-blinded comparison of 2 diagnostic methods. Setting Five primary schools in which head lice infestation was epidemic. Participants A total of 304 students aged 6 to 12 years. Main Outcome Measures Presence of nymph, adults, and nits; sensitivity, predictive value, and accuracy of both methods. Results Visual inspection underestimated the true prevalence of active infestation by a factor of 3.5. The sensitivity of wet combing in diagnosing active infestation was significantly higher than of visual inspection (90.5% vs 28.6%;P Conclusions Wet combing is a very accurate method to diagnose active head lice infestation. Visual inspection is the method of choice, if one aims to determine the frequency of carriers of eggs or nits.

Pediculosis Capitis im kindesalter epidemiologische und sozialmedizinische erkenntnisse einer reihenuntersuchung von schulanfangern
Gesundheitswesen, 2008Co-Authors: C Jahnke, E Bauer, Hermann FeldmeierAbstract:Obwohl die Pediculosis Capitis die haufigste Parasitose des Kindesalters ist, sind Kenntnisse zur Epidemiologie und Morbiditat vergleichsweise rudimentar. Fur Deutschland gibt es keine Daten aus bevolkerungsbasierten Studien. Im Rahmen der Schuleingangsuntersuchung der Stadt Braunschweig wurden alle Kinder des Einschulungsjahrgangs 2007 (n=1 890) auf die Prasenz von Kopflausen und kopflausassoziierter klinischer Pathologie untersucht. Die Diagnose der Kopflausinfestation erfolgte mittels visueller Inspektion von funf Pradilektionsstellen. Das Wissen der Erziehungsberechtigten uber die Pediculosis Capitis und die Krankheitsperzeption wurde mithilfe eines Fragebogens erfasst. Die routinemasig bei der Schuleingangsuntersuchung erhobenen sozialdemografischen Daten werden mit derzeitiger oder anamnestischer Kopflausinfestation assoziiert. Bei 0,7% (14 von 1 890) der Kindern wurde ein Kopflausbefall diagnostiziert. Die tatsachliche Pravalenz liegt bedingt durch die niedrige Sensitivitat der visuellen Inspektion um einen Faktor 2–4 hoher. Zusatzlich hatten 5,6% der Kinder bereits in den vergangenen 12 Monaten schon einmal eine Pediculosis Capitis durchgemacht. Daraus ergibt sich eine Inzidenz von 598/10 000 Kinder im Alter von 5–6 Jahren/Jahr. Familien mit niedrigem Bildungsstand und ohne Migrationshintergrund waren signifikant haufiger von einer Pediculosis Capitis betroffen als Familien mit hohem Bildungsgrad und mit Migrationshintergrund. Die Auswertung der Fragebogendaten zeigte erhebliche Wissenslucken der Eltern in Bezug auf die Pediculosis Capitis, eine Neigung zu Polypragmasie bei Behandlungsmasnahmen und teilweise exaltierte Reaktionen bis hin zu Hysterie. Der Wissensstand war signifikant geringer bei Eltern mit niedrigem Bildungsniveau. Unsere Studien erbrachten erstmals fur Deutschland zuverlassige Daten uber Pravalenz, Inzidenz und sozialmedizinische Merkmale der Pediculosis Capitis im Kindesalter sowie zum Wissensstand in der Bevolkerung. Although Pediculosis Capitis is the most frequent parasitosis in childhood, reliable data on its epi-demiology and morbidity are scarce. In Germany population-based data do not exist. During the routine medical examination of 5–6-year-old pre-school children in Braunschweig city (n=1 890) the children were also examined for the presence of head lice and head lice-associated pathology. Visual inspection of five predilection sites was used to diagnose head lice infestation. Knowledge of careers on head lice infestation and disease perception were analysed using a standardised questionnaire. Socio-demographic variables of the households to which the children belonged were correlated to current or historical head lice infestation. Head lice infestation was diagnosed in 14 out of the 1 890 children (0.7%). Considering the low sensitivity of visual inspection the true prevalence should be higher by a factor 3 to 4. In addition, 5.6% of the children examined had suffered from a head lice infestation in the previous 12 months. This results in an incidence of 598 cases per 10 000 children aged 5 to 6 years per year. Households with a low educational level of the parents and without a background of migration were significantly more often affected by Pediculosis Capitis during the previous 12 months. The analysis of the questionnaires showed a rather low level of knowledge about Pediculosis Capitis, a tendency towards polypragmatic therapeutic approaches and partially aberrant reactions such as hysteria. The knowledge about Pediculosis Capitis was significantly lower in parents with a low educational level. Our study provides for the first time reliable data on the prevalence, incidence and disease perception of head lice infestation as well as concerning the knowledge parents have about this parasitic skin disease.

Gatal merupakan gejala utama dari Pediculosis Capitis. Salah satu faktor penyebab Pediculosis Capitisadalah personal hygienedan tingkat pengetahuan seseorang terhadap Pediculosis Capitis. Penelitian ini bertujuan untuk mengetahui tingkat pengetahuan dan personal hygienedengan kejadian Pediculosis Capitispada santri putri Madrasah Tsanawiyah (MTs) di Pesantren X Kecamatan Mempawah Timur hubungan. Penelitian analitik observasional dengan pendekatan rancangan penelitian jenis cross-sectional. Jumlah sampel sebanyak 139 orang. Variabel bebas penelitian ini adalah tingkat pengetahuan dan personal hygienesubjek penelitian sedangkan variabel terikatnya adalah kejadian Pediculosis Capitispada santri putri Madrasah Tsanawiyah (MTs) di Pesantren X Kecamatan Mempawah Timur. Sebanyak 98,3% subjek penelitian memiliki tingkat pengetahuan yang baik tentang penyakit Pediculosis Capitisdan 94,2% subjek penelitian memiliki personal hygieneyang baik tentang penyakit Pediculosis Capitis. Uji statistik yang telah dilakukan tidak terdapat hubungan yang bermakna pada kedua variabel dengan nilai p>0,05. Kesimpulannya adalah tidak terdapat hubungan antara tingkat pengetahuan dan personal hygienedengan kejadian Pediculosis Capitispada santri putri Madrasah Tsanawiyah (MTs) di Pesantren X Kecamatan Mempawah Timur.

Pediculosis Capitis terjadi 41 kali lipat lebih sering pada anak perempuan dan pada anak usia 9-16 tahun. Salah satu faktor penyebab Pediculosis Capitis adalah personal hygiene yang buruk. Penyuluhan diperlukan untuk mengurangi angka kejadian Pediculosis Capitis. Tujuan penelitian ini adalah untuk mengetahui efektivitas penyuluhan menggunakan media audiovisual terhadap personal hygiene dan angka kejadian Pediculosis Capitis pada santri putri Madrasah Tsanawiyah (MTs) di Pondok Pesantren X Kecamatan Mempawah Timur. Jenis penelitian yang digunakan adalah penelitian eksperimen semu dengan rancangan yang digunakan adalah one group pretest posttest. Jumlah sampel sebanyak 139 orang. Variabel bebas penelitian ini adalah penyuluhan dengan menggunakan media audiovisual sedangkan variabel terikatnya adalah personal hygiene dan angka kejadian Pediculosis Capitis pada santri putri Madrasah Tsanawiyah (MTs) di Pesantren X Kecamatan Mempawah Timur. Hasil penelitian ini adalah subjek penelitian dengan perilaku personal hygiene baik sebelum penyuluhan (95,0%) dan sesudah penyuluhan (98,6%). Subjek penelitian positif Pediculosis Capitis sebelum penyuluhan (87,8%) dan sesudah penyuluhan (61,2%). Berdasarkan uji statistik McNemar yang telah dilakukan tidak terdapat perbedaan bermakna pada variabel personal hygiene (p>0,05), dan terdapat perbedaan bermakna pada variabel angka kejadian (p<0,05). Penelitian ini menyimpulkan bahwa penyuluhan menggunakan media audiovisual secara statistik memberikan efektivitas yang tidak signifikan dalam menentukan perubahan personal hygiene, tetapi memberikan efektivitas yang signifikan dalam mengurangi angka kejadian Pediculosis Capitis pada santri putri Madrasah Tsanawiyah di Pondok Pesantren X Kecamatan Mempawah Timur.Kata kunci: Pediculosis Capitis, santri putri, penyuluhan media audiovisual, personal hygiene, angka kejadia

Faktor Yang Berhubungan Dengan Kejadian Pediculosis Capitis Pada Siswa Sekolah Dasar Inpres Benteng Timur Selayar

Penyakit ini telah menyebabkan morbiditas yang signifikan di antara anak sekolah di seluruh dunia. Penelitian ini bertujuan mengetahui hubungan jenis kelamin, pendapatan keluarga, jumlah anggota keluarga, panjang rambut dan frekuensi cuci rambut dengan kejadian Pediculosis Capitis pada siswa SDI Benteng Timur Selayar. Metode: Jenis penelitian yang digunakan adalah observasional analitik dengan rancangan “Cross Sectional Study”. Jumlah sampel dalam penelitian ini yaitu 139 siswa SDI Benteng Timur Selayar. Pengambilan sampel dilakukan dengan menggunakan teknik Proportional Stratified Random Sampling. Analisis data dilakukan dengan menggunakan uji statistic Chi square. Hasil: ada hubungan antara jenis kelamin dengan kejadian Pediculosis Capitis dengan p = 0,000 dan φ = 0,592, ada hubungan antara pendapatan keluarga dengan kejadian Pediculosis Capitis dengan p = 0,001 dan φ = 0,288, ada hubungan antara jumlah anggota keluarga dengan kejadian Pediculosis Capitis dengan p = 0,000 dan φ = 0,450, tidak ada hubungan antara panjang rambut dengan kejadian Pediculosis Capitis dengan p = 0,131 dan ada hubungan antara frekuensi cuci rambut dengan kejadian Pediculosis Capitis dengan p = 0,000 dan φ = 0,830. Kesimpulan: para siswa disarankan untuk menjaga kebersihan dan kesehatan rambutnya dengan menghindari kontak kepala dengan penderita Pediculosis Capitis, tidak bertukar pakaian dengan penghuni rumah dan mencuci rambut minimal tiga kali seminggu
