The concentration-dependent inhibitory profile for Ag-induced PLD activation was parallel to those for secretory response and inositol phosphate formation. In contrast, TBX had no effect on PLD activation caused by calcium ionophore A23187 or phorbol myristate acetate. These results suggest that TBX inhibits Ag-induced PLD activation by interfering with the signal transduction pathway upstream of Ca2+ mobilization and protein kinase C activation.

The 42 and 44 kDa proteins were identified as mitogen-activated protein (MAP) kinases with immunoblotting of anti-MAP kinase antibody. The effects of an antiallergic drug, Pemirolast potassium (TBX) on Ag-induced protein tyrosine phosphorylation and MAP kinase activation were investigated. When RBL-2H3 cells were stimulated with Ag in the presence of TBX, tyrosine phosphorylation of three proteins (33, 42 and 44 kDa) was inhibited concentration-dependently (0.1-10 micrograms/ml). Inhibition of Ag-induced tyrosine phosphorylation of 33 kDa protein, which could be a beta subunit of Fc epsilon RI, suggests that TBX may prevent the activation of Fc epsilon RI. TBX suppressed activation of MAP kinases (42 and 44 kDa) in response to Ag as well as phorbol myristate acetate (100 nM) or calcium ionophore A23187 (500 nM), implying that the drug acts on signal transduction component(s) between the second messengers and MAP kinases. However, TBX had no effects on protein tyrosine phosphorylation and MAP kinase activation in MC3T3-E1 osteoblastic cells. These results indicate that TBX may affect Fc epsilon RI and also may act as a step distal of Ca2+ mobilization and protein kinase C activation leading to MAP kinase activation in RBL-2H3 cells.

At the same concentrations, the drug suppressed both the formation of inositol 1,4,5-trisphosphate and the mobilization of Ca2+, indicating the prevention of phospholipase C activation. The production of 1,2-diacylglycerol and phosphatidic acid, which was mainly due to phosphatidylcholine hydrolysis, was also suppressed. Moreover, TBX reduced Ag-induced liberation of arachidonic acid, a precursor of eicosanoids, implying the inhibition of phospholipase A2. These data suggest that TBX inhibits the activation of phospholipase C, leading to decreased formation of the signal transducing molecules necessary for cell activation.

Penelitian ini dilakukan untuk mengetahui penurunan konsentrasi IL-4 dan perbedaan penurunan konsentrasi IL-4 air mata penderita alergi okuli sesudah diterapi dengan Pemirolast potassium 0,1% dan cromolyn sodium 4%. Desain uji klinis acak tersamar ganda dilakukan pada 32 sampel penderita alergi okuli yang dibagi menjadi dua kelompok, yaitu 16 subjek yang mendapatkan tetes mata Pemirolast potassium 0,1% dan 16 sampel yang mendapatkan tetes mata cromolyn sodium 4%. Air mata penderita diambil dua kali untuk dilakukan pemeriksaan konsentrasi IL-4, yaitu saat datang dan satu minggu setelah terapi. Konsentrasi IL-4 diperiksa menggunakan ELISA. Analisa data dengan uji t berpasangan dan uji t tidak berpasangan. Pada kelompok Pemirolast potassiumterdapat penurunan signifikan (p<0,001) rata-rata konsentrasi IL-4 sebelum pemberian (24,65±SD 2,04 pg/ml) dan sesudah pemberian (20,17±SD 2,34 pg/ml). Penurunan IL-4 yang signifikan (p<0,001) juga didapat pada kelompok cromolyn sodium dengan rata-rata konsentrasi IL-4 awal 23,50±SD 0,95 pg/ml menjadi 22,51 ± SD 1,07 pg/ml. Penurunan konsentrasi IL-4 pada kelompok Pemirolast potassium (4,47±SD 1,74 pg/ml) secara signifikan (p<0,001) lebih besar dibanding kelompok cromolyn sodium (0,99±SD 0,50 pg/ml). Dapat disimpulkan pemberian Pemirolast potassium 0,1% atau cromolyn sodium 4 % dapat menurunkan konsentrasi IL-4 air mata dengan penurunan lebih besar pada pemberian Pemirolast potassium 0,1%.

A double blind randomized clinical trial was performed in 32 patients with ocular allergy divided into two groups, 16 patients received Pemirolast potassium 0,1% and the rest of them received cromolyn sodium 4%. To measure the IL-4 concentration the tears were taken twice in the first day and one week after treatment. IL-4 concentration was evaluated using ELISA. Data were analized using paired t test and independent t test. There was a significant reducing of IL-4 concentration (p<0,001) in Pemirolast potassium group from 24,65±2,04 pg/ml before treatment into 20,17±2,34 pg/ml after treatment. A significant reducing of IL-4 concentration in tears (p<0,001) from 23,50±0,95 pg/ml into 22,51±1,07 pg/ml also found in cromolyn sodium group. The decreasing concentration on Pemirolast potassium group (4,47±1,74 pg/ml) was significantly higher (p<0,001) compared to cromolyn sodium group (0,99±0,50 pg/ml). It can be concluded that administration of Pemirolast potassium 0,1% and cromolyn sodium 4% will reduce the concentration of IL-4 in tears with higher reduction in Pemirolast potassium.

2012Co-Authors: Saiful Anwar MalangAbstract:Penyakit alergi okuli adalah penyakit alergi pada konjungtiva atau kulit kelopak mata, berhubungan dengan reaksi inflamasi karena menempelnya immunoglobulin E (IgE) di sel mast. Penatalaksanaan penyakit alergi okuli meliputi meminimalkan kontak dengan alergen dan pemberian obat-obatan (antihistamin, stabilisator sel mast (sodium cromoglycate, Pemirolast potassium, lodoxamide, nedocromil sodium), kombinasi antihistamin dan stabilisator sel mast (kortikosteroid, siklosporin). Tujuan penelitian ini adalah untuk mengetahui penurunan konsentrasi IgE dan perbedaan penurunan konsentrasi IgE penderita alergi okuli sesudah diterapi dengan Pemirolast potassium 0,1% atau sodium cromoglycate 4%.Penelitian ini merupakan pre and post clinical trial design pada 32 penderita alergi okuli. Sebanyak 16 penderita mendapatkan tetes mata Pemirolast potassium 0,1% dan 16 lainnya mendapatkan tetes mata sodium cromoglycate 4%. Air mata penderita diambil saat datang dan satu minggu setelah terapi untuk pemeriksaan konsentrasi IgE dan diperiksa menggunakan ELISA reader. Penurunan konsentrasi IgE sebelum dan setelah terapi pada masing-masing obat diuji menggunakan uji T-test dependent, sedangkan perbandingan penurunan konsentrasi IgE antara kedua obat diuji menggunakan uji T-test independent. Pada kelompok Pemirolast potassium 0,1%, konsentrasi IgE awal 730,73 ± SD 40,17 IU/ml dan menurun menjadi 678,61 ± SD 56,20 IU/ml. Kelompok sodium cromoglycate 4%, konsentrasi IgE awal 731,16 ± SD 32,13 IU/ml dan menurun menjadi 686,73 ± SD 43,08 IU/ml(p=0,000). Perbedaan penurunan konsentrasi IgE pada kelompok Pemirolast potassium 0,1% sebesar 52,12 ± SD 41,00 IU/ml, kelompok sodium cromoglycate 4% sebesar 44,43 ± SD 32,63 IU/ml. Perbedaan penurunan konsentrasi IgE antara kedua obat tidak bermakna (p=0,561). Dapat disimpulkan tidak didapatkan perbedaan bermakna efek Pemirolast potassium 0,1% dan sodium cromoglycate 4% dalam menurunkan konsentrasi IgE air mata penderita alergi okuli. This study was aimed to evaluate the different effect of Pemirolast potassium 0,1% and sodium cromoglycate 4% in decreasing IgE concenration. A pre and post clinical trial design was performed in 32 patients with ocular allerg divided equaly in two treatment groups. The tears of patients were taken in the first day and one week after treatment to perform IgE concentration, and was evaluated using ELISA reader. The level of IgE concentration before and after treatment in every group was analyzed with paired t-test dependent, and differences between two drugs was compared using unpaired Ttest . In Pemirolast potassium 0,1% group, the initial IgE concentration was 730,73 ± 40,17 IU/ml and decreased into 678,61 ± 56,20 IU/ml. While the initial IgE concentration of sodium cromoglycate group was 731,16 ± 32,13 IU/ml and decreased into 686,73 ± 43,08 IU/ml (p=0,000). The difference of decreasing concentration on Pemirolast potassium 0,1% group was 52,12 ± 41,00 IU/ml and 44,43 ± 32,63 IU/ml on sodium cromoglycate 4% group. Those differences of decreasing concentration of the two groups were not significant. It can be concluded that there were no differences between Pemirolast potassium 0,1% and sodium cromoglycate 4% in decreasing the IgE concentration of ocular allergic patients.

This research was aimed to evaluate the decreasing concentration ofIL-4 and differences of decreasing IL-4 concentration between the tears of ocular alergic patients treated with Pemirolast potassium 0,1% and cromoyn sodium 4%. A double blined randomized clinical trial was performed in. 32 patients with acular allergy divided into two groups, 16 patients received potossium 0.1% and the rest of them receved cromolyn sodium 4%.
