  • Pemphigus Vegetans presenting as acrodermatitis continua suppurativa

    Abstract:

    The authors present a 51‐year‐old patient with clinical signs of acrodermatitis continua suppurativa of the toes. Since histological findings failed to unambiguously support the diagnosis of acrodermatitis continua suppurativa, immunohistology was performed. The results showed intercellular epidermal fluorescence. The subsequent clinical picture (vegetating formation), the repeated histological examination (suprabasal cleft and eosinophilia), as well as the data of immunohistological findings supported the diagnosis of Pemphigus Vegetans Hallopeau. The patient was treated successfully with a combination of cyclosporine‐A and low‐dose steroid therapy. The case is of special interest, because there is only a single publication about phalangeal and nail involvement in Hallopeau‘s type Pemphigus Vegetans in the dermatological literature according to our knowledge.

    Abstract:

    A 43-year-old woman developed annular and pustular cutaneous lesions preceded by tiny yellow pustules coating the surface of the oral mucosa. The clinical, histological and immunopathological evidence clearly showed that the patient had pyodermatitis-pyostomatitis Vegetans. It is suggested that this disease is a distinct entity which should be differentiated from Pemphigus Vegetans.

    A 78-year-old woman developed intertriginous vegetating plaques, mouth ulcers, and a cerebriform tongue after 11 months of captopril therapy. Findings of clinical, histologic, and immunofluorescence testing were consistent with a diagnosis of Pemphigus Vegetans. In addition, Charcot-Leyden crystals were observed within some of the intraepidermal, eosinophilic abscess cavities, which were bordered by granular cells undergoing keratinization. The skin lesions cleared rapidly after discontinuation of captopril. To the best of our knowledge, this report describes the first case of Pemphigus Vegetans induced by captopril.

    Abstract:

    Pemphigus Vegetans is a rare variant of Pemphigus vulgaris characterized by vegetating lesions in the folds and mouth and by the presence of autoantibodies against desmoglein 3. We describe two Caucasian patients with Pemphigus Vegetans, one of them presented antibodies to desmoglein 3 and 1 and the other one to desmoglein 3. Both patients also had circulating antibodies against a 190 kDa protein co-migrating with periplakin. Anti-periplakin reactivity is usually detected in paraneoplastic Pemphigus, while it has never been reported in Pemphigus Vegetans. Our observation enlarges the spectrum of autoantibodies which may be associated with Pemphigus Vegetans. However, the pathophysiological significance of anti-periplakin reactivity in this Pemphigus variant remains to be determined.

    The case of a patient with a plaque on her left clavicular area developing in about 9 months is described. Direct immunofluorescence from a perilesional area showed intercellular deposits of IgG and C3 in the epidermis. Granular deposits of C3 in a linear distribution were also present at the dermo-epidermal junction. No circulating autoantibodies were detected in the serum. The lesion improved in 2 months without any specific treatment. The term localized Pemphigus Vegetans for this form is proposed.

    : Pemphigus is a chronic autoimmune vesiculobullous disease caused by autoantibodies directed against desmogleins (Dsgs). Pemphigus Vegetans is a rare variant of Pemphigus vulgaris, characterized by vegetative plaques, primarily occurring in flexures. It is a less common disease and involves the skin and mucosa due to disintegration of cellular adherence (acantholysis), resulting in intradermal split. We report this case of Pemphigus Vegetans of Neumann type for its rare presentation. We report a case of Pemphigus Vegetans, involving lips in a middle-aged female and sparing flexural areas. This patient was diagnosed on the basis of histopathology and treated successfully with intralesional steroids, resulting in the resolution of her plaques.
