    Abstract:

    Background: Delirium is a neuropsychiatric disorder characterised by severe cognitive impairment, but the specific neuropsychological profile of this condition has not yet been clearly delineated. Psychiatric symptoms of Perceptual Disturbance—such as hallucinations, illusions and misperceptions—are common in delirium, suggesting that patients may have deficits in the cognitive systems underlying visual perception. Methods: Five neuropsychological tests of visual perception were administered to 17 older patients with delirium, as well as to two control groups comprising 14 patients with Alzheimer’s dementia and 18 cognitively normal patients. The Mini Mental State Examination and the Consortium to Establish a Registry for Alzheimer9s Disease verbal memory test were also administered to assess the specificity of any Perceptual impairments. Results: Patients with delirium scored significantly lower than cognitively normal patients on all Perceptual tasks and significantly lower than patients with dementia on three of these tasks. Mini Mental State Examination scores did not differ between the delirium and dementia groups, and patients with delirium showed significantly better verbal recognition performance than those with dementia. Conclusions: Patients with delirium have specific visual Perceptual deficits that cannot be accounted for by general cognitive impairment. These novel findings provide insights into the neural mechanisms underling delirium and might help to improve clinical detection and management of the disorder. The results also support previous suggestions that cognitive Perceptual deficits play a causal role in eliciting psychiatric symptoms of Perceptual Disturbance.

    The authors estimated Perceptual Disturbance in children with spastic diplegia from the difference between the visual and performance IQ scores (VIQ-PIQ) on the Wechsler Intelligence Scale for Children-Revised (WISC-R), having found a strong negative correlation between this score and the PQ obtained on the Frostig Developmental Test of Visual Perception (DTVP). The ratio of the areas of the posterior horns to the anterior horns (P/A) correlated negatively with visuoPerceptual Disturbance. This ratio can therefore be used to assess Perceptual Disturbance at an early age in children with spastic diplegia.

    The intellectual status of twelve children with spina bifida and hydrocephalus was evaluated. Seven children were considered to have verbal and non-verbal skill discrepancies. It was considered that the Verbal IQ-Performance IQ score by WISC-R could be used as an index of their visuoPerceptual Disturbance. The Disturbance was closely related to the morphological characteristics of the lateral ventricles on cerebral MRI. The ratio of the areas of the posterior horns to the anterior horns (P:A) showed a negative correlation with visuoPerceptual ability. The visual pathway, visual cortex, and the ventral system were thus considered to be the sites of the affected lesions. From P.A changes with time, it was evident that adequate shunting would prevent the Perceptual Disturbance.

    In 1955, English psychiatrist John Todd (1914-1987) described Alice in Wonderland syndrome (AIWS) as self-experienced paroxysmal body image illusions involving distortions of the size, mass, or shape of the patient's own body or its position in space, often occurring with depersonalization and derealization.(1) Todd named AIWS for the Perceptual disorder of altered body image experienced by the protagonist in the novel Alice's Adventures in Wonderland (1865), written by Lewis Carroll(2) (the pseudonym of Reverend Charles Lutwidge Dodgson [1832-1898]), possibly based in part on Dodgson's own migrainous experiences.(3) In the story, Alice followed a talking white rabbit down a rabbit hole and then experienced several dramatic changes in her own body size and shape (e.g., shrinking to 10 inches high, growing unnaturally large, and growing unnaturally tall but not any wider).(2) Although Todd's report was the most influential, Lippman(4) provided an earlier description in 1952. In Lippman's article, one of the patients reported feeling short and wide as she walked, and referenced Alice's Adventures in Wonderland in regard to her body image illusions, referring to them as a "Tweedledum" or "Tweedledee" feeling.
