    Objectives: We prospectively assessed the association between alcohol consumption and onset of Perimenopause in women of late reproductive age using data from the Harvard Study of Moods and Cycles. Specific types of alcoholic beverages – red wine, white wine, beer, and liquor – were evaluated. Methods: Among 502 women aged 36–45 years residing in seven Boston communities, we assessed self-reported perimenopausal symptoms over a 5-year period. The onset of Perimenopause was defined using changes in menstrual characteristics. We administered a semiquantitative food frequency questionnaire at enrollment to measure regular alcohol consumption during the past year. Hazard ratios (HRs) and 95% confidence intervals were derived from Cox regression models. Results: No association was found between total alcohol consumption and time to Perimenopause. Compared with women consuming <1 alcoholic drink per month, HRs for those drinking red wine in quantities of 1–3 glasses per month, 1 glass per week, or ≥2 glasses per week were all below 1.0, after accounting for other sources of alcohol. Among never smokers, the inverse association with red wine was stronger and a positive association was observed with liquor consumption although numbers were small. No associations were observed with white wine or beer. Conclusion: While there was no association between total alcohol consumption and onset of Perimenopause, there was some suggestion of an inverse association between red wine and risk of Perimenopause, particularly among never smokers.

    Perimenopause represents a significant transition in a woman's life. The evidence to support an association between Perimenopause and depression is mixed, yet recent prospective studies have provided stronger evidence to support such an association. Interpretation of study data are complicated by methodological issues, such as a lack of standard definition for Perimenopause or depression, reducing comparability. A variety of causal factors, including psychological, genetic and physiological, have been implicated in depression during Perimenopause, which lends weight to a multifactorial model. Physicians should consider initiating dialogue about menopause and symptom relief at age 40 and screening perimenopausal women for depressive symptoms. Selective serotonin reuptake inhibitors are a first line of treatment for depression, but hormone therapy could be considered for women experiencing menopausal symptoms unless there are contraindications. Future research should focus on establishing temporality and studying these potential relationships among women of different ethnicities.

    Abstract:

    Study objective: To investigate of the extent to which violence over the life course accelerates the onset of Perimenopause, as measured by menstrual changes. Design: Prospective cohort study. Setting: Boston, Massachusetts. Participants: 603 premenopausal women aged 36–45 years at baseline who completed a cross sectional survey on childhood and adult violence history. Main outcome measure: Time to Perimenopause, defined as time in months from baseline interview to a woman's report of (1) an absolute change of at least seven days in menstrual cycle length from baseline or subjective report of menstrual irregularity; (2) a change in menstrual flow amount or duration; or (3) cessation of periods for at least three months, whichever came first. Main results: Experience of abuse was associated with delayed onset of menstrual changes indicative of onset of Perimenopause. Women reporting childhood or adolescent abuse entered Perimenopause about 35% slower than women who reported no abuse (IRRadj = 0.65, 95% CI 0.45 to 0.95) after adjusting for age, age at menarche, ever live birth, ability to maintain living standard, smoking, BMI, and depression. There was a similar association among women who reported first abuse during adulthood (IRRadj = 0.72, 95% CI 0.28 to 1.80). These findings persisted when the cohort was restricted to non-depressed women (childhood/adolescent IRRadj = 0.57, 95% CI 0.36 to 0.90; adulthood IRRadj = 0.63, 95% CI 0.23 to 1.77). Conclusions: This study is the first longitudinal analysis of the timing of Perimenopause to show an association with a history of physical or sexual abuse. Further study of the relation between violence and reproductive aging is needed.

Methods A total of 135 perimenopausal women aged 40–56 years participated in the longitudinal Swiss Perimenopause Study. For the purpose of this investigation, a cross-sectional design was chosen. A wide range of validated psychosocial questionnaires were used to compare women with and without prior depression regarding their experience of the Perimenopause. Findings were statistically adjusted for multiple testing. Results Women with prior depression showed significantly more depressive symptoms (U = 1215.5, p  Limitations Self-report data may affect the results.     While the clinical definition of Perimenopause focuses on functional changes in the reproductive system, the symptoms of Perimenopause are largely neurological and immunological in nature and are observed in women globally across cultures, races, and ethnicities. Estrogen is the master regulator of the metabolic system of the female brain and body. During the perimenopausal transition, what is known as the “estrogen receptor network” is disconnected from the bioenergetic system resulting in a hypometabolic state that is associated with neurological dysfunction, which in some women may increase risk for neurodegenerative disease. Moreover, an APOE4 genotype exacerbates that bioenergetic crisis. Neurological symptoms that emerge during the Perimenopause reflect the disruption in multiple estrogen-regulated systems including thermoregulation, sleep and circadian rhythms, sensory processing, affect, and multiple domains of cognitive function. Many of these symptoms are also associated with risk of Alzheimer’s disease (AD), which in women is twice as high than in men. Such elevated risk is correlated to obesity and systemic inflammation due to estrogen depletion occurring in Perimenopause and menopause. Aging and neurodegenerative brains are found to be associated with chronic neuroinflammation primarily due to a dysregulation of the innate immunity, mainly driven by senescent microglia. Identifying women with metabolic or inflammatory at-risk phenotypes for late-onset AD might translate into a target population that is likely to respond to estrogen replacement therapy and adjuvant therapies that serve as metabolic regulators. Transitions of female aging involve a set of sequential, system-level adaptations. The perimenopausal transition is a critical period in the neuro-adaptive landscape of the aging brain and represents a window of opportunity for precision hormone therapeutics.

    Perimenopause marks initiation of female reproductive senescence. Age of onset is only 47% heritable suggesting that additional factors other than inheritance regulate this endocrine aging transition. To elucidate these factors, we characterized transcriptional and epigenomic changes across endocrine aging using a rat model that recapitulates characteristics of the human Perimenopause. RNA-seq analysis revealed that hypothalamic aging precedes onset of Perimenopause. In the hypothalamus, global DNA methylation declined with both age and reproductive senescence. Genome-wide epigentic analysis revealed changes in DNA methylation in genes required for hormone signaling, glutamate signaling, and melatonin and circadian pathways. Specific epignetic changes in these signaling pathways provide insight into the origin of Perimenopause-associated neurological symptoms such as insomnia. Treatment with 5-aza-2'-deoxycytidine, a DNA-methyltransferase-1 inhibitor, accelerated transition to reproductive senescence/ whereas supplementation with methionine, a S-adenosylmethionine precursor, delayed onset of Perimenopause and endocrine aging. Collectively, these data provide evidence for a critical period of female neuroendocrine aging in brain that precedes ovarian failure and that DNA methylation regulates the transition duration of Perimenopause to menopause.

    Perimenopause is a midlife transition state experienced by women that occurs in the context of a fully functioning neurological system and results in reproductive senescence. Although primarily viewed as a reproductive transition, the symptoms of Perimenopause are largely neurological in nature. Neurological symptoms that emerge during Perimenopause are indicative of disruption in multiple estrogen-regulated systems (including thermoregulation, sleep, circadian rhythms and sensory processing) and affect multiple domains of cognitive function. Estrogen is a master regulator that functions through a network of estrogen receptors to ensure that the brain effectively responds at rapid, intermediate and long timescales to regulate energy metabolism in the brain via coordinated signalling and transcriptional pathways. The estrogen receptor network becomes uncoupled from the bioenergetic system during the perimenopausal transition and, as a corollary, a hypometabolic state associated with neurological dysfunction can develop. For some women, this hypometabolic state might increase the risk of developing neurodegenerative diseases later in life. The perimenopausal transition might also represent a window of opportunity to prevent age-related neurological diseases. This Review considers the importance of neurological symptoms in Perimenopause in the context of their relationship to the network of estrogen receptors that control metabolism in the brain.
