    Abstract:

    Background: Nephron-sparing surgery has been shown to achieve oncological results equivalent to those of radical nephrectomy in patients with compromised renal function who have renal tumors <4 cm. However, technical difficulties and potential surgical complications remain challenges to the surgeon. Various factors, such as tumor size and location, collecting system invasion, and patient's expectations, are major concerns that may affect the feasibility of performing an effective partial nephrectomy. We report a patient with renal cell carcinoma who underwent open partial nephrectomy and suffered from complications of Perirenal Abscess because of stone migration, obstruction, and subsequence urine leakage. Case Presentation: A 63-year-old female with an incidental finding of a 4-cm renal tumor on the left kidney underwent an effective open partial nephrectomy at our institute. There were no intraoperative complications and the drainage tube was removed 3 days later. The patient visited our emergency department 1 month later with high fever. She was diagnosed with severe sepsis and Perirenal Abscess formation. Percutaneous nephrostomy was performed for prompt decompression. A sequential imaging study with CT and antegrade pyelography revealed one 0.5-cm stone, which migrated from the calix to proximal ureter postoperatively and subsequently caused obstruction and dramatic urine leakage. Ureteroscopic lithotripsy was performed with ureteral stenting. Postoperatively, the urinary leakage and Abscess formation resolved. Conclusion: Nephron-sparing surgery is one of the most challenging procedures in the management of renal cancer. Urine leakage after partial nephrectomy may occur when a large tumor involves the collecting system. Postoperative ureteral obstruction is also a risk factor of urinary leakage. Watertight repair with a Double J catheter before operation may be required.

  • Computed tomography findings associated with bacteremia in adult patients with a urinary tract infection

    The use of computed tomography (CT) in the diagnosis of urinary tract infection (UTI) has rapidly increased recently at acute stage, but the CT findings associated with bacteremia in UTI patients are unknown. 189 UTI patients were enrolled who underwent a CT scan within 24 h after hospital admission. We classified CT findings into eight types: a focal or multifocal wedge-shaped area of hypoperfusion, enlarged kidneys, perinephric fat stranding, ureteritis or pyelitis, complicated renal cyst, renal papillary necrosis, hydronephrosis, and renal and Perirenal Abscess. A retrospective analysis was conducted to evaluate the CT findings associated with bacteremia. The mean age of these patients was 60 ± 17.2 years, and 93.1 % were women. Concurrent bacteremia was noted in 40.2 % of the patients. Abnormal CT findings were noted in 96.3 % of the patients and 62.4 % had two or more abnormal findings. The most frequent abnormal CT finding was a focal or multifocal wedge-shaped area of hypoperfusion (77.2 %), followed by perinephric fat stranding (29.1 %). Perinephric fat stranding, hydronephrosis, and the presence of two or more abnormal CT findings were significantly associated with bacteremia in patients with community-acquired UTI. In the multivariate logistic regression analysis, age [odds ratio (OR) 1.03; 95 % confidence interval (CI) 1.009–1.062], two or more abnormal CT findings (OR 3.163; 95 % CI 1.334–7.498), and hydronephrosis (OR 13.160; 95 % CI 1.048–165.282) were significantly associated with bacteremia. Physicians should be aware that appropriate early management is necessary to prevent fatality in patients with these CT findings.

    Abstract:

    A 76-year-old woman with a history of dyspnoea, weight loss and abdominal pain, was admitted to our Hospital. Sonographic and tomographic examinations showed the presence of a large adrenal gland tumor and the promptly performed adrenalectomy and splenectomy proved that the lesion was an adrenal gland carcinoma infiltrating the spleen. One month after surgical treatment, the patient's general condition dramatically worsened due to development of Perirenal Abscess and renal infarction; finally, the patient died. In accordance with literature, we decided to only perform adrenalectomy and splenectomy that are the treatment of choice in these cases. In fact, complications are unforeseeable and avoiding the resection of the kidney surely offered the patient a better life quality.

    Acute abdominal pain during pregnancy may be urologic origin. In this report, perinephric Abscess was treated by percutaneous catheter drainage until a term delivery was achieved. We have experienced a case of perinephric Abscess of pregnancy and reported with brief review of related literatures.
