Peritoneal Fluid leptin is associated with chronic pelvic pain but not infertility in endometriosis patients

The objective of this study was to evaluate the Peritoneal Fluid levels of leptin in patients with endometriosis and idiopathic infertility and compare them with a control group of tubal ligation/reanastomosis patients. METHODS: In this observational, prospective controlled study, Peritoneal Fluid from 108 women was obtained while they underwent laparoscopy for pelvic pain, infertility, tubal ligation or sterilization reversal. We measured the concentration of leptin in the Peritoneal Fluid and compared the levels among women who were divided into groups according to their post-surgical diagnosis. Sixty patients were diagnosed with endometriosis, 10 with idiopathic infertility and 38 had undergone tubal ligation or reanastomosis (control group). RESULTS: Peritoneal Fluid leptin was significantly higher in endometriosis 14.62 ± 9.79 (mean ± SD) ng/ml compared to idiopathic infertility [0.92 ± 1.57 ng/ml (P = 0.0007)] and to controls [0.78 ± 1.94 ng/ml (P < 0.0001)]. Leptin levels were positively correlated with the stage of endometriosis (r = 0.45; P = 0.03), and with pelvic pain in endometriosis patients (r = 0.49; P = 0.001). Peritoneal Fluid leptin levels in patients with idiopathic infertility were comparable to controls. CONCLUSIONS: Higher levels of leptin were observed in Peritoneal Fluid of patients with endometriosis compared to those without the disease. These data suggest that the proinflammatory and neoangiogenic action of leptin may contribute to the pathogenesis of endometriosis. Moreover, leptin may play a role in endometriosis-associated pain.

quantification of doripenem in human plasma and Peritoneal Fluid by high performance liquid chromatography with ultraviolet detection

Doripenem was stabilized by immediate mixing of the plasma or Peritoneal Fluid with 1 M 3-morpholinopropanesulfonic acid buffer (pH 7.0) (1:1). Doripenem and an internal standard were detected by measuring their ultraviolet absorbance at 300 nm. The calibration curves for doripenem in human plasma and Peritoneal Fluid were linear from 0.05 to 100 μg/mL. For plasma, both the intra- and the interday precision were less than 3.41% (CV), and the accuracy was between 97.4 and 101.7% above 0.05 μg/mL. For Peritoneal Fluid, the intra- and the interday precision were less than 2.98% (CV), and the accuracy was between 94.4 and 103.9% above 0.05 μg/mL. The limit of detection was 0.02 μg/mL in both plasma and Peritoneal Fluid. The assay has been applied to the therapeutic drug monitoring of doripenem in both plasma and Peritoneal Fluid.

The objective of this review was to evaluate the complex role of Peritoneal Fluid in the etiopathogenesis of endometriosis. Several studies suggest that Peritoneal Fluid is a key inflammatory environment associated with endometriosis. Many active substances (cytokines, growth factors, hormones and oxidative stress parameters) have been identified in endometriosis patients at different stages of the disease. Inflammatory mediators may be involved in the endometriosis associated-infertility and possibly pain. Furthermore, these mediators may represent a non surgical method for diagnosing endometriosis. Better understanding of the mechanism of cytokines, growth factor and reactive oxygen species production and detoxification and further investigation of their effects on the Peritoneal Fluid environment are essential to obtain new insight into this disease and eventually develop novel diagnostic and therapeutic remedies.
