    Abstract:

    Peritoneal Mesothelioma is a rare and aggressive tumor. Early diagnosis of the disease is difficult, delaying effective treatment. We report a case of recurrent, biphasic, diffuse, malignant Peritoneal Mesothelioma. Initial abdominal computed tomography showed abnormal but nonspecific findings suggestive of an ovarian malignancy, with a negative endoscopy and laboratory studies. An abdominal exploratory laparotomy found widespread malignancy within the peritoneum with a pathological diagnosis of Peritoneal Mesothelioma. A PET/CT imaging showed diffusely increased metabolic activity throughout the peritoneum, with no evidence of thoracic or pleural involvement. This case demonstrates the PET/CT findings seen with malignant recurrent Peritoneal Mesothelioma.

  • Unusual radiologic presentations of malignant Peritoneal Mesothelioma

    BACKGROUND Malignant Peritoneal Mesothelioma is an unusual disease process characterized radiologically by ascites and infiltration of the peritoneum by multiple small tumor nodules. Both parietal and visceral peritoneum are involved by the multiple malignant tumor nodules. Computed tomography (CT) has been used to identify the anatomic pathology induced by the progression of this malignant process. AIM To identify and then describe unusual CT images in patients with malignant Peritoneal Mesothelioma. Recognition of these unusual radiologic findings can cause the radiologist to be suspicious of this rare malignant process. METHODS In 100 patients who were to undergo definitive treatment of malignant Peritoneal Mesothelioma, the findings on preoperative CT scans were catalogued. Many of these changes were repeatedly noted on the CT scans. Other pathologic CT images were less common. These unusual radiologic presentations were specially studied in this manuscript. RESULTS Eight unusual radiologic presentations of malignant Peritoneal Mesothelioma were selected for study. These unusual findings included a mass occurring within a Spigelian hernia, infiltration of the splenic parenchyma by spherical Mesothelioma masses, infiltration of the lower mediastinum by tumor, a Mesothelioma mass within a left inguinal canal, enlarged cardiophrenic angle lymph nodes, pleural plaques associated with the progression of malignant Peritoneal Mesothelioma, splenic notches caused by disease surrounding the spleen, and a mass greater than 5 cm associated with the proximal jejunum and directly adjacent to the anatomic location of the Treitz ligament. CONCLUSION There are unusual radiologic presentations of malignant Peritoneal Mesothelioma that are important to recognize in order to accurately diagnose this disease by CT.

  • Incidental Finding of Benign Multicystic Peritoneal Mesothelioma: A Case Report

    Abstract:

    Introduction : Benign multicystic Peritoneal Mesothelioma represents a rare benign variant of Peritoneal Mesothelioma, with fewer than 150 cases reported to date. Malignant transformation may occur. We present a patient with an incidental finding of an intra-abdominal mass consistent with benign multicystic Peritoneal Mesothelioma. Case presentation : A 51 year-old male presented to the ED with traumatic injuries. During workup, calcified cystic lesions in the pelvis were incidentally noted. Final pathology favored a diagnosis of benign multicystic Mesothelioma of the peritoneum. Follow-up imaging obtained three months post-operatively revealed no evidence of recurrent or metastatic disease. Conclusion : Multicystic Peritoneal Mesothelioma has been identified as a distinct subtype of Peritoneal Mesothelioma, with long-term survival achievable through the use of cytoreductive surgery and HIPEC. Although prognosis is relatively favorable, recurrence rates are high, with low potential for malignant transformation. Post-operative surveillance with routine imaging is warranted.

    Malignant Peritoneal Mesothelioma (MPM) is a rare malignancy arising from the serosa of the abdominal cavity; its natural history is hallmarked by intra-abdominal disease progression. Peritoneal Mesothelioma patients generally present with abdominal pain and/or ascites. Pathologically, a positive immunostain for calretinin has markedly increased the accuracy of diagnosis. A new staging system combining extent of tumour burden measured by the Peritoneal cancer index (PCI), abdominal nodal status and extra-abdominal metastases has been demonstrated to reliably stratify patient outcomes after cytoreductive surgery (CRS) and hyperthermic perioperative chemotherapy (HIPEC). Over the past decade, the management of these patients has evolved as a multimodality treatment similar to ovarian cancer treatment and now involves CRS and HIPEC.

    Background: This study was undertaken to measure survival of patients with multicystic Peritoneal Mesothelioma treated by cytoreductive surgery and hyperthermic intraPeritoneal chemotherapy through a multi-institutional collaboration. Methods: A multi-institutional data registry, established by the Peritoneal Surface Oncology Group, was used to identify patients with Peritoneal Mesothelioma and the subgroup with multicystic tumours, treated by cytoreductive surgery and hyperthermic intraPeritoneal chemotherapy. Outcomes for this subgroup are reported. The primary endpoint was overall survival. A secondary endpoint was the incidence of treatment-related complications. Results: Of 405 patients with Peritoneal Mesothelioma, 26 (6·4 per cent) had multicystic tumours. There were 20 women and six men with a mean(s.d.) age of 42(12) years. The median Peritoneal carcinomatosis index (PCI) was 14 (range 6–39). There was no perioperative mortality. Six patients developed grade III or IV complications. After a median follow-up of 54 (range 5–129) months, all 26 patients were still alive. Conclusion: Multicystic Peritoneal Mesothelioma appears to be a distinct subtype of Peritoneal Mesothelioma, where long-term survival may be achieved through cytoreductive surgery and hyperthermic intraPeritoneal chemotherapy.     Abstract:

    The proportion of Peritoneal Mesotheliomas among all Mesotheliomas has been decreasing, leading some to suggest that Peritoneal Mesothelioma occurs only after high levels of exposure to asbestos. To investigate the relationship between asbestos exposure and the development of Peritoneal Mesothelioma, a case-control study examined 40 cases of primary Peritoneal Mesothelioma from a single institution. This series differed from previous reports in that 75% of the cases and controls had attended college. Results show an odds ratio of 6.6 for asbestos exposure among this group of primary Peritoneal Mesothelioma cases with relatively slight asbestos exposures.

The conventional classification distinguishes diffuse malignant Peritoneal Mesothelioma (DMPM) and border-line forms: multicystic Peritoneal Mesothelioma (MCPM) and well-differentiated papillary Peritoneal Mesothelioma (WDPPM). Despite the novel achievements in the management of PM, there is difficulty in conducting randomized trials due to its rarity and aggressive biology in many cases. As there is, a necessity to standardize diagnosis and management of PM, the Peritoneal Surface Oncology Group International (PSOGI) commissioned a steering committee to elaborate clinical guidelines.
