The Burden of Acute Pesticide Poisoning and Pesticide Regulation in Korea.

Methods Disability-adjusted life-years (DALYs) were calculated as the sum of years of life lost (YLLs) and years lived with disability (YLDs) due to acute Pesticide Poisoning using the methods of the Global Burden of Disease Study 2013. The values of YLLs, YLDs, and DALYs were stratified by sex, age groups, intentionality of Poisoning, and causative agents. Results From 2006 to 2014, DALYs decreased by 69% (from 69,550 to 21,742). The decreasing tendency of DALYs was especially marked from 2011. The total burden of acute Pesticide Poisoning was mainly caused by YLLs and intentional Pesticide Poisoning. The highest DALYs due to acute Pesticide Poisoning occurred in those in their 40s and 50s; however, the decreased rates of DALYs were higher in those aged 10-49 years than in those aged more than 50 years. Herbicides including paraquat contributed to the largest proportion of total DALYs. Conclusion As this is the first study to quantify the burden of acute Pesticide Poisoning using DALYs, our results provide comprehensive evidence of the importance of using strict regulations to prevent public health hazards due to acute Pesticide Poisoning.

The annual average age-standardized rate of incidence from acute Pesticide Poisoning was 3.6 per 100 000. The majority of the cases were identified in the categories of the 1-4-year-old age-group (56.5%), outpatients (80.0%), single-day visit to a hospital (70.4%) and summer occurrence (43.3%).CONCLUSIONS: Acute Pesticide Poisoning is prevalent among children in South Korea; therefore, intervention efforts are needed to reduce the cases of Pesticide Poisoning among children. The aim of this study was to investigate the association between occupational Pesticide exposure and depressive symptoms among male farmers in South Korea. Methods A nationwide sampling survey of male farmers was conducted in South Korea. A total of 1958 male farmers were interviewed in 2011. Severity of occupational Pesticide Poisoning was evaluated according to symptoms, types of treatment and number of Pesticide Poisonings per individual. Depressive symptoms were assessed using the Geriatric Depression Scale. A survey logistic regression model was used to estimate the multivariate OR and 95% CIs. Results Among total farmers, 10.4% (n=197) reported depressive symptoms. After controlling for potential confounders, occupational Pesticide Poisoning in the previous year was positively associated with the risk of depressive symptoms (OR=1.61; 95% CI 1.10 to 2.34). Cases of more severe Pesticide Poisoning, such as moderate- or severe-symptom cases (OR=2.81; 95% CI 1.71 to 4.63), outpatient or hospitalisation cases (OR=2.52; 95% CI 1.15 to 5.53), and multiple Poisoning cases (OR=1.82; 95% CI 1.19 to 2.76) showed higher risks of depressive symptoms than did milder cases. Among the Pesticides causing the Poisonings, paraquat dichloride was found to be a significant predictor of depressive symptoms. No significant association was found with cumulative lifetime Pesticide application and depressive symptoms. Conclusions Our findings suggest that the risk of depression appears to be related to the severity of symptoms of Poisoning, type of care received and the number of previous episodes of acute Poisonings.

Those who engaged in inappropriate work behaviors such as not following Pesticide label instructions (OR = 1.61), applying the Pesticide in full sun (OR = 1.48), and applying the Pesticide upwind (OR = 1.54) had a significantly increased risk of Pesticide Poisoning. There was no significant risk difference by type of farming. In addition, the magnitude of these risk factors did not differ significantly by severity of acute Pesticide Poisoning. In fact, our findings suggest that work-related risk factors contributed to the development of acute occupational Pesticide Poisoning without relation to its severity. Therefore, prevention strategies for reducing occupational Pesticide Poisoning, regardless of severity, should be recommended to all types of farming and the level of Poisoning severity.

A total of 1,958 male farmers were interviewed in order to estimate the incidence of acute occupational Pesticide Poisoning in rural areas in 2010. Based on a self-reported definition, the incidence rate in 2010 and the lifetime hospitalization rate due to acute occupational Pesticide Poisoning among male farmers were estimated. RESULTS: The incidence rate of acute occupational Pesticide Poisoning was 24.7 (95% CI 22.1-27.2) per 100 male farmers, which corresponds to 209,512 cases across South Korea in 2010. About 12% of the Pesticide Poisoning cases included either visiting a medical doctor or hospitalization. The lifetime rate of hospitalization due to occupational Pesticide Poisoning was 6.1 (95% CI 4.9-7.3) among male farmers. Poisoning occurred mainly when farmers were applying Pesticides during summer. CONCLUSION: Our nationwide sampling survey demonstrated that acute occupational Pesticide Poisonings are highly prevalent among male South Korean farmers. Data collected from the National Emergency Department Information System were used to estimate the number of emergency department visits due to Pesticide Poisoning in South Korea for the period spanning 2006 through 2009. Emergency department visits for Pesticide Poisoning were defined by ICD-10 codes (T60.0–T60.9). National estimates and their 95% confidence intervals were calculated per 100 000 population. Results. Among the 65 877 total Poisoning-related emergency department visits in the data, 11 985 (18.2%) were emergency department visits resulting from Pesticide Poisoning. Structural equation modeling of Pesticide Poisoning, depression, safety, and injury.

The authors conducted structural equation modeling of cross-sectional data to examine the pattern of associations between Pesticide Poisoning, depressive symptoms, safety knowledge, safety behaviors, and injury. Interviews of 1637 Colorado farm operators and their spouses from 964 farms were conducted during 1993–1997. Pesticide Poisoning was assessed based on a history of ever having been poisoned. The Center for Epidemiologic Studies—Depression scale was used to assess depressive symptoms. Safety knowledge and safety behaviors were assessed using ten items for each latent variable. Using the WHO standard case definition of a possible acute Pesticide Poisoning, we investigated the prevalence and risk factors of acute work-related Pesticide Poisoning among farmers in Southern China. A stratified sample of 910 Pesticide applicators from two villages in southern China participated in face-to-face interviews. Respondents who self-reported having two or more of a list of sixty-six symptoms within 24 hours after Pesticide application were categorized as having suffered acute Pesticide Poisoning. The association between the composite behavioral risk score and Pesticide Poisoning were assessed in a multivariate logistic model. A total of 80 (8.8%) Pesticide applicators reported an acute work-related Pesticide Poisoning. The most frequent symptoms among applicators were dermal (11.6%) and nervous system (10.7%) symptoms. Poisoning was more common among women, farmers in poor areas, and applicators without safety training (all p < 0.001). After controlling for gender, age, education, geographic area and the behavioral risk score, farmers without safety training had an adjusted odds ratio of 3.22 (95% CI: 1.86-5.60). The likelihood of acute Pesticide Poisoning was also significantly associated with number of exposure risk behaviors. A significant "dose-response" relationship between composite behavioral risk scores calculated from 9 Pesticides exposure risk behaviors and the log odds of Pesticide Poisoning prevalence was seen among these Chinese farmers (R2 = 0.9246). This study found that 8.8% of Chinese Pesticide applicators suffered acute Pesticide Poisoning and suggests that Pesticide safety training, safe application methods, and precautionary behavioral measures could be effective in reducing the risk of Pesticide Poisoning.

Pesticide Poisoning and respiratory disorders in Colorado farm residents.

Beseler, Lorann StallonesAbstract:Respiratory hazards significantly contribute to the burden of occupational disease among farmers. Pesticide exposure has been linked to an increased prevalence of respiratory symptoms in several farming populations. The purpose of this study was to evaluate the association between respiratory symptoms and Pesticide Poisoning in a cross-sectional survey of farm residents. A total of 761 farm operators and their spouses, representing 479 farms in northeastern Colorado, were recruited from 1993 to 1997. A personal interview asked whether the resident had experienced a Pesticide Poisoning and several respiratory conditions including cough, allergy, wheeze, and organic dust toxic syndrome (ODTS). Spirometry testing was performed on 196 individuals. Logistic regression was used to model the association of Pesticide Poisoning with respiratory conditions, and linear regression was used to model the relationship of Pesticide Poisoning and forced vital capacity (FVC) and forced expiratory volume (FEV1). In unadjusted models, Pesticide Poisoning was associated with all four respiratory conditions, and stayed significant in adjusted models of allergies and cough in non-smokers. In age- and gender-adjusted models, Pesticide Poisoning was significantly associated with lower FVC and FEV1 in current smokers and in those who were not heavy drinkers. Although this study should be reproduced in a larger sample, it suggests that further evaluation of the respiratory effects of Pesticide exposure is warranted.

A cohort study of Pesticide Poisoning and depression in Colorado farm residents.

Beseler, Lorann StallonesAbstract:Purpose Depressive symptoms have been associated with Pesticide Poisoning among farmers in cross-sectional studies, but no longitudinal studies have assessed the long-term influence of Poisoning on depressive symptoms. The purpose of this study was to describe the associations between Pesticide Poisoning and depressive symptoms in a cohort of farm residents. Methods Farm operators and their spouses were recruited in 1993 from farm truck registrations using stratified probability sampling. The Center for Epidemiologic Studies-Depression scale was used to evaluate depression in participants using generalized estimating equations. Baseline self-reported Pesticide Poisoning was the exposure of interest in longitudinal analyses. Results Pesticide Poisoning was significantly associated with depression in three years of follow-up after adjusting for age, gender, and marital status (odds ratio [OR] 2.59; 95% confidence interval [CI] 1.20–5.58). Depression remained elevated after adjusting for health, decreased income, and increased debt (OR 2.00; CI 0.91–4.39) and was primarily due to significant associations with the symptoms being bothered by things (OR 3.29; CI 1.95–5.55) and feeling everything was an effort (OR 1.93; CI 1.14–3.27). Conclusions Feeling bothered and that everything was an effort were persistently associated with a history of Pesticide Poisoning, supportive of the hypothesis that prolonged irritability may result from Pesticide Poisoning.

Safety Practices, Neurological Symptoms, and Pesticide Poisoning

Beseler, Lorann StallonesAbstract:Depression, anxiety, inability to concentrate, and spatial disorientation associated with Pesticide Poisoning may influence farmers' ability to comply with established safety procedures. The purpose of this article is to describe the relationship between safety practices, neurological symptoms, and Pesticide Poisoning. A survey of farm residents was conducted in an eight-county area in Colorado. Multivariate logistic regression models were used to determine associations between safety practices, neurological symptoms, and previous Pesticide Poisoning. A number of safety practices were associated with the following neurological symptoms: difficulty concentrating; feeling irritable; relatives noticing memory difficulties; and difficulty understanding reading materials. The associations between safety practices and neurological symptoms were increased in the presence of Pesticide Poisoning. Factors associated with failure to engage in established safety practices in this study were neurological symptoms.

Incidence of acute occupational Pesticide Poisoning among male farmers in South Korea.

The objective of this study was to examine the incidence of acute occupational Pesticide Poisoning among male farmers in South Korea. METHODS: A nationwide sampling survey of male farmers was conducted in South Korea. This survey used a stratified multistage probability sampling design and adopted the face-to-face interview method. A total of 1,958 male farmers were interviewed in order to estimate the incidence of acute occupational Pesticide Poisoning in rural areas in 2010. Based on a self-reported definition, the incidence rate in 2010 and the lifetime hospitalization rate due to acute occupational Pesticide Poisoning among male farmers were estimated. RESULTS: The incidence rate of acute occupational Pesticide Poisoning was 24.7 (95% CI 22.1-27.2) per 100 male farmers, which corresponds to 209,512 cases across South Korea in 2010. About 12% of the Pesticide Poisoning cases included either visiting a medical doctor or hospitalization. The lifetime rate of hospitalization due to occupational Pesticide Poisoning was 6.1 (95% CI 4.9-7.3) among male farmers. Poisoning occurred mainly when farmers were applying Pesticides during summer. CONCLUSION: Our nationwide sampling survey demonstrated that acute occupational Pesticide Poisonings are highly prevalent among male South Korean farmers. For validity of Pesticide information, the purchased pes-ticide lists from the agricultural cooperative federation were compared with individual self-reported Pesticides used.Neurological symptoms were the most common among clinical symptoms related to Pesticide Poisoning, followed bygastrointestinal, dermal and eye symptoms. Insecticides were the main causative Pesticide class for Poisoning. Twentyone major symptoms were selected for screening for acute Pesticide Poisoning in a field study based on the pilot resultsand literature reviews. Substantial under-reporting was found in data on self-reported individual Pesticides used (9.2%),as well as low accuracy (36%) compared with their actual purchase lists. In this pilot study, we suggest the selectedsymptom lists may apply to a field survey of Pesticide Poisoning.
