Clinical Study Unconventional Use of Intense Pulsed Light

According to the literature, intense pulsed light (IPL) represents a versatile tool in the treatment of some dermatological conditions (i.e., pigmentation disorders, hair removal, and acne), due to its wide range of wavelengths. The authors herein report on 58 unconventional but effective uses of IPL in several cutaneous diseases, such as rosacea (10 cases), port-wine stain (PWS) (10 cases), disseminated porokeratosis (10 cases), Pilonidal Cyst (3 cases), seborrheic keratosis (10 cases), hypertrophic scar (5 cases) and keloid scar (5 cases), Becker’s nevus (2 cases), hidradenitis suppurativa (2 cases), and sarcoidosis (1 case). Our results should suggest that IPL could represent a valid therapeutic support and option by providing excellent outcomes and low side effects, even though it should be underlined that the use and the effectiveness of IPL are strongly related to the operator’s experience (acquired by attempting at least one specific course on the use of IPL and one-year experience in a specialized centre). Moreover, the daily use of these devices will surely increase clinical experience and provide new information, thus enhancing long-term results and improving IPL effectiveness

Diagnostic criteria consist of a sinus tract or Cyst containing hair follicles and inflammatory reaction in one or more partially or completely epithelialized sinus tracts. Follow-up of patients reported in the literature has been too short to provide a consensus in regard to the extent of surgery required to provide permanent cure. Case A 30-year-old patient was seen in consultation for evaluation of a chronic, draining periclitoral abscess that had been treated for approximately 2 years with multiple rounds of antibiotics and local incisions. Treatment consisted of a wide local excision of the Cyst and multiple sinuses extending into the periclitoral area and labia minora. The shaft and glans of the clitoris were preserved. Primary closure and healing were accomplished. Conclusion Diagnosis and curative therapy of a Pilonidal Cyst of the clitoris require a thorough resection which can be accomplished with preservation of the clitoris.

Cytotherapy, 2012Co-Authors: Carlo Maria Fronticelli Baldelli, Marco Ruella, S Scuderi, Manuela Monni, Roberto Passera, Paola Omede, Corrado TarellaAbstract:Abstract Background aims Stem cells, namely easily accessible bone marrow-derived cells (BMC), are reportedly capable of tissue repair in different damaged organs and might favor wound healing. The present study was undertaken to evaluate the feasibility and safety of BMC mobilization induced by granulocyte–colony-stimulating factor (G-CSF) in patients undergoing surgery for sacrococcygeal Pilonidal Cysts (SPC). To evaluate the possible clinical benefit of G-CSF in reducing the time to complete resolution, a comparison with a control group receiving surgery without G-CSF was performed. Methods Eight patients with complex SPC were included in this prospective trial. Patients were treated with G-CSF (5µg/kg b.i.d.) for 3 consecutive days; standard surgical exeresis of the Pilonidal Cyst was scheduled on day 2 of mobilization. Mobilization was assessed in terms of circulating CD34 + cells and granulocyte–macrophage colony-forming unit (CFU-GM) progenitors. Results Mobilization of CD34 + cells and CFU-GM occurred in all patients, along with a marked increase in white blood cells (median peak value 28 435/µL, day 3). G-CSF was well tolerated and no adverse events occurred. All patients received the planned surgical treatment without any complications. Interestingly, the G-CSF group patients had a median time to resolution (117 days, range 110–130) significantly shorter than control patients (145 days, range 118–168) ( P =0.034). Conclusions G-CSF administration, along with BMC mobilization, is feasible and well tolerated in patients undergoing surgery for SPC; clinical results compare favorably with those observed in controls not receiving G-CSF; the results suggest the potential use of G-CSF as an additional treatment to accelerate wound healing in patients undergoing surgery.
