    Abstract:

    We report the case of a 23-year-old man who presented with late-appearing intracerebral calcification during the course of central diabetes insipidus. He had been diagnosed as having ectopic Pinealoma and had received brain irradiation at the age of 11 years. Bilateral calcifications of the basal ganglia and thalamus appeared 8 years after the development of diabetes insipidus. Thereafter, the calcified lesions increased in extent on follow-up computed tomograms. Though the influence of cranial irradiation was not ruled out, the lack of evidence for any underlying metabolic defect suggested that persistent hypernatremia (hyperosmolar dehydration) due to insufficiently treated diabetes insipidus was the most probable cause of the intracerebral lesions. This has been described in previous cases of nephrogenic diabetes insipidus associated with intracerebral calcification.

    • A fourth case of bilateral retinoblastoma (RB) associated with Pinealoma was identified among 56 patients with heritable RB. Using life-table analysis, the association was 5.6% 2 years after RB diagnosis. Retinoblastoma-Pinealoma (RB-P) was uniformly fatal in this series and in 40 additional patients described in the literature. Ninety-five percent of patients with RB-P had bilateral RB, a positive family history of RB, or both. All patients with Pinealoma presented with symptoms of increased intracranial pressure due to obstructive hydrocephalus. The advanced stage of the tumors at diagnosis may contribute to their uniform lethality. The incidence of a positive family history of RB in patients with Pinealoma was higher than expected (62% of patients), which may be related to earlier surveillance and treatment of patients with positive family histories of RB.

    Abstract:

    We report a boy with bilateral, familial retinoblastoma recognized at the age of 3 months. At the age of 2 1/2 years the patient developed a tumour in the pineal region. Both tumours were successfully treated by radiation. The chromosomes were normal when examined by the prophase technique, in particular there was no deletion of band q 14 of chromosome 13. We consider the simultaneous occurrence of retinoblastoma and Pinealoma as more then a pure coincidence, probably a consequence of a generally increased susceptibility to cancer and of the histogenetic similarities of retina and pineal body.

  • Occurrence of Pineal Gland Tumors in Combined Chronic Toxicity/Carcinogenicity Studies in Wistar Rats

    Abstract:

    Pineal gland tumors are very rare brain lesions in rats as well as in other species including humans. A total of 8 (out of 1,360 examined) Wistar rats from 3 different combined chronic toxicity/carcinogenicity or mere carcinogenicity studies revealed pineal gland tumors. The tumors were regarded to be spontaneous and unrelated to treatment. The morphology and immunohistochemical evaluation led to the diagnosis malignant Pinealoma. The main characteristics that were variably developed within the tumors were the following: cellular atypia, high mitotic index, giant cells, necrosis, Homer Wright rosettes, Flexner-Wintersteiner rosettes and pseudorosettes, positive immunohistochemical reaction for synaptophysin, and neuron-specific enolase. The pineal gland is not a protocol organ for histopathological examination in carcinogenicity studies. Nevertheless, the pineal gland can occasionally be encountered on the routine brain section or if it is the origin of a tumor protruding into the brain, the finding will be recorded. Therefore, although known to be a rare tumor in rats, pineal neoplasms should be included in the list of possible differential diagnoses for brain tumors, especially when the tumor is located in the region of the pineal body.
