    AbstractInfective endocarditis (IE), an infection of the endocardial surface, frequently leads to life-threatening complications, such as thromboembolism due to Platelet activation. We investigated the mean Platelet Volume (MPV) in Korean patients with IE and the serial changes thereof, in comparison with other laboratory parameters. We analyzed 248 MPV results from 22 patients diagnosed with IE in our hospital between January 2011 and April 2012. MPV was measured with an Advia 2120 (Siemens Healthcare Diagnostics, Tarrytown, NY) using EDTA-containing tubes. The mean MPV differed significantly between the patient and control groups, 8.74 vs. 7.96 fl, respectively. In addition, the Platelet count and MPV/Platelet count ratio were significantly decreased in the patient group. The total Platelet mass and Platelet size in IE might be increased. Further studies should examine more patients to verify the changes in the MPV and MPV/Platelet count ratio in IE and assess in greater detail the relationship between ...

    Abstract:

    We thank Dr. Behnam Shakiba for his recent interest in our article that evaluated mean Platelet Volume before and after renal transplant as a potential predictor of renovascular thrombosis.1 He asked for the method of blood sampling for measuring mean Platelet Volume in our study. More relevant has been reporting of the choice of anticoagulant, measurement time, and temperature when the mean Platelet Volume is analyzed. In general, for all hematologic laboratory tests and cell counting, the whole blood sample must be anticoagulated to inhibit coagulation before measurement.2 Previously, it was suggested that using tubes with a high concentration of sodium citrate would obtain more reliable measures of mean Platelet Volume.3 However, this recommendation did not meet our practice demands. Instead, most laboratories, as in our institution, continue to rely on ethylenediaminetetraacetic acid as the standard hematologic anticoagulant because of its shorter processing times.4 It has been shown that mean Platelet Volume values are independent of the anticoagulation with ethylenediaminetetraacetic acid or citrate, if the measurement is done within 1 hour of sampling.5 In accord with this recommendation, full blood counts of all the patients in our institution are routinely measured with ethylenediaminetetraacetic acid K3 within 15 minutes after venipuncture.   • Mean Platelet Volume as a potential predictor of renovascular thrombosis after renal transplant.

    OBJECTIVES: We sought to evaluate the importance of mean Platelet Volume as a marker to follow-up, the tendency for hemorrhagic diatheses, and/or thrombotic complications in patients before and after renal transplant. MATERIALS AND METHODS: Thirty-four patients (aged, 5 to 18 y) were included. Demographics of the patients, cause of chronic renal failure, dialysis modality, duration of dialysis, arterio-venous fistula thrombosis, and posttransplant immunosuppressive regimens were recorded and laboratory variables were evaluated. RESULTS: At the end of the first posttransplant month, mean Platelet Volume level was decreased significantly when compared with pretransplant levels (8.3 ± 1.5 vs 7.7 ± 0.9; P = .04). A significant increase was observed in Platelet levels during posttransplant measures (273.750 ± 97.700 vs 318.740 ± 84.586; P = .02). Prothrombin time and partial thromboplastin time levels did not differ before and after transplant. None of the patients had any thrombotic events and/or renal allograft loss. A negative correlation was observed between mean Platelet Volume and C-reactive protein (r=-0.53). Mean Platelet Volume level was not found to be related to the cause of renal failure, pretransplant dialysis modality, or posttransplant immunosuppressive regimens. CONCLUSIONS: Platelet numbers increased and mean Platelet Volume decreased after pediatric renal transplant, but the potential for increased thrombosis was not observed.

  • Impact of mean Platelet Volume on the occurrence and severity of sudden sensorineural hearing loss.

    OBJECTIVE: We aimed to determine: (1) whether mean Platelet Volume was elevated in patients with sudden sensorineural hearing loss, compared with healthy controls; and (2) whether mean Platelet Volume level was related to hearing loss severity. MATERIALS AND METHODS: The study included 31 patients with sudden sensorineural hearing loss and 31 age- and sex-matched, healthy controls. Peripheral venous blood samples were taken from subjects and mean Platelet Volume and levels of glucose, total cholesterol, high-density lipoprotein, low-density lipoprotein and triglyceride were measured. RESULTS: Mean Platelet Volume was significantly greater in the sudden sensorineural hearing loss group compared with the control group. However, there was no significant correlation between mean Platelet Volume level and hearing loss severity. CONCLUSION: Mean Platelet Volume, a determinant of Platelet activation, is elevated in patients with sudden sensorineural hearing loss. To our knowledge, this is the first report investigating mean Platelet Volume levels in such patients. Our findings indirectly support the hypothesis of vascular impairment as a pathogenetic factor in sudden sensorineural hearing loss.

  • The relationship of mean Platelet Volume with retinopathy in type 2 diabetes mellitus.

    BACKGROUND/AIM: Patients with diabetes mellitus have an increased risk of developing micro- and macrovascular complications. Retinopathy is one of the most important complications of diabetes whose pathophysiological cause has not yet been determined. One candidate may be Platelet Volume. In this study we aimed to investigate the association between mean Platelet Volume and diabetic retinopathy. MATERIALS AND METHODS: Mean Platelet Volume levels were investigated in type 2 diabetic patients with and without retinopathy, and in healthy participants. After reclassifying our diabetics in terms of the presence or absence of hypertension and hyperlipidemia, we compared their mean Platelet Volumes. We then checked to see if the mean Platelet Volume correlated with hemoglobin A1c and body mass index. RESULTS: The mean Platelet Volume levels were higher in all diabetic patients, and they were highest in diabetics with retinopathy. The mean Platelet Volume levels were not different in diabetic patients with and without hypertension or hyperlipidemia. Moreover, the mean Platelet Volume levels did not correlate with hemoglobin A1c or body mass index. CONCLUSION: We think that Platelets may play a role in diabetic retinopathy. We can also say that, in diabetic patients, hypertension, hyperlipemia, obesity, and control of diabetes were not related to mean Platelet Volume.

    BACKGROUND/AIM: Activation and size of Platelets have been suggested to be involved in the pathogenesis of vascular complications in diabetes mellitus. The purpose of the present study was to investigate the association of mean Platelet Volume with microalbuminuria in type 2 diabetic patients. MATERIALS AND METHODS: Mean Platelet Volume levels were investigated in healthy participants and type 2 diabetic patients with and without microalbuminuria. After their mean Platelet Volume values were compared, correlation of mean Platelet Volume with sex, duration of diabetes, microalbuminuria, fasting blood glucose, hemoglobin A1c, creatinine clearance, and body mass index was examined. RESULTS: Mean Platelet Volume levels were higher in all diabetic patients than those in controls. Mean Platelet Volume levels were highest in diabetics with microalbuminuria. The mean Platelet Volume levels had a positive correlation with microalbuminuria. CONCLUSION: Mean Platelet Volume values of diabetic patients were higher than those of nondiabetics, the highest levels being in diabetics with microalbuminuria. Our results suggest that microalbuminuria might be related with mean Platelet Volume in diabetic patients.
