    Abstract:

    This study analyzed correlates of social problem solving in 63 2-parent families with a Preadolescent Child during structured discussions in the home. The longitudinal design assessed families when the Preadolescent was about 9.5 years old and again 2 years later. Links between problem solving and features of the immediate ecological context were examined. There was a substantial decline in the quality of family problem solving during preadolescence associated with changes in the participation and affective behavior of fathers and Preadolescents. Although problem solving was not less effective on autonomy topics, it was markedly worse when parents, rather than the Preadolescent, selected the topic for discussion. Preadolescent topic selection triggered a pattern of productive participation and affective behavior in both parents and Preadolescents. The results clarify the nature of ambivalent parent-Child relations just prior to puberty by specifying changes in family problem solving which correspond with Preadolescent autonomy striving.

    Abstract:

    This study tested the hypothesis that the mother-father coalition, parent-Child coalitions, and parental warmth expressed toward the Child are associated with family problem solving in families with a Preadolescent Child referred for treatment of behavior problems (n = 30), families with a Child at-rick for conduct disorder (n = 68), and a sample of comparison families (n = 90). Referred and at-risk families displayed less effective problem solving. A regression analysis, which controlled for gender of the Child, which controlled for gender of the Child, family structure, family income, marital satifaction, and severity of Child problems, showed that strong parental coalitions were linked to low levels of family problem solving in at-risk and referred families. Parent-Child coalitions had little apparent impact while parental warmth was highly correlated with better family problem solving. The results may be interpreted as evidence for a tendency for parents in at-risk and referred families to «scapegoat» a Preadolescent during family problem-solving sessions. This may undermine progress on family problem solutions and may complicate family-based prevention and treatment programs that use family problem-solving sessions

    Extra-abdominal desmoid tumors are circumscribed but non-capsulated neoplasms of differentiated fibrous tissue arising from musculoaponeurotic tissues. They tend to be locally infiltrative, resulting in a high rate of local recurrence without metastasis, following surgical resection. We report a 9-year-old boy who had a large desmoid tumor in his right buttock that was successfully excised.

    Abstract:

    This is in reference to our article published before [1]. On 1 year follow up, the patient had local recurrence measuring 8 cm X 5 cm infero-laterally to the previous scar. MRI of the local region showed a large well defined heterogeneously enhancing soft tissue mass in subcutaneous layers involving ilio-tabial tract with areas of deep multiple recurrences (Fig. 1). There was no evidence of metastasis. Local excision was done. Histopathology was identical as beforethe desmoid tumor. The surgical margins were free; the mass was surrounded with fibrocollagenous tissue. Keeping in view the unresectable deep recurrences, he was administered radiotherapy locally. MRI done after 6 weeks of radiotherapy did not show any residual disease. The Child is under close follow up.

    Abstract:

    Objective. To study healing patterns of anogenital trauma in prepubescent Children. Methods. A prospective 10-year study was conducted of 94 Children who had anogenital trauma and were followed to healing and documented using a colposcope with 35-mm camera attachment. Results. The 13 boys and 81 girls were referred with injuries as a result of sexual assault or anogenital trauma. Hymenal injuries occurred in 37 cases; 2 transections healed after surgery, and 15 persisted unchanged. Partial tears, hymenal abrasions, or hematomas healed completely or with minor nonspecific changes. Of the 47 injuries to the posterior fourchette, 22 abrasions, hematomas, or tears healed completely; 12 tears healed with vascular changes; 2 developed labial fusions; 10 lacerations required surgery; and 6 scarred and 4 healed with vascular changes. Only 2 of 39 cases of perihymenal trauma healed with vascular changes. All 17 cases of labial trauma healed completely. Anal trauma healed completely in 29 of 31 with scarring occurring in only 2 cases that required surgery. Conclusions. Anogenital trauma heals quickly, often without residua. Of the 94 cases, there were diagnostic anatomic changes in the 15 cases of hymenal transections (2 other cases healed completely with surgical reconstruction), 6 cases after surgical repair of posterior fourchette, and 2 cases of anal scarring after surgery.
