    Abstract:

    Objective To compare the clinical Values of computed tomography (CT)and positron emission tomography CT (PET-CT)in the diagnosis of mediastinal metastasis of resectable non-small cell lung cancer (NSCLC). Methods Fourty-two patients with resectable NSCLC underwent CT and PET-CT with an interval of 1 month and subsequently underwent thoracotomy for clearance of lymph nodes. The specimens of the primary lesion and mediastinal lymph node underwent HE staining. The sensitivity, specificity,accuracy, positive Prediction Value, and negative Prediction Value of the two procedures in the diagnosis of metastasis of mediastinal lymph nodes were compared.Results The sensitivity, specificity, accuracy, positive Prediction Value, and negative Prediction Value in diagnosis of metastasis of mediastinal lymph nodes were 71.8%,60.0%,69.7% and 62.5% respectively for CT, and were 92.8%,87.5%,86.67% and 93.3%,respectively for PET-CT. The Kappa Value was superior in the PET-CT group to that in the CT group.Conclusion Conclusion PET-CT is superior to CT in the preoperative staging of metastasis of NSCLC to mediastinal lymph nodes and the primary lesion. PET-CT significantly increases the sensitivity in diagnosis.

    Abstract:

    Aims Several electrocardiographic (ECG) algorithms have been developed to identify the site of origin of ventricular premature contractions (VPCs) from right ventricular outflow tract (RVOT) based on pacemapping; however, their accuracy remains unclear. Methods and results We evaluated the accuracy of these algorithms in 52 consecutive patients (31 female, mean age 42.6 ± 14.6 years) with successful radiofrequency ablation of RVOT-VPC as guided by 3D electroanatomical non-contact mapping (Ensite, St Jude Medical, USA) and compared with a newly proposed ECG algorithm. As guided by 3D electroanatomical mapping, the successful ablation sites of RVOT-VPC were RVOT septum ( n = 31), RVOT free wall ( n = 19), and His region ( n = 2). Retrospective evaluation in the initial 39 patients shows that the overall positive Prediction Value to identify a successful ablation site of this newly proposed ECG algorithm is 77.3% and is higher than the 73.3% by Ito et al ., 73.3% by Joshi et al ., and 53.8% by Dixit et al . ( P > 0.05). Prospective evaluation in the subsequent 13 patients also demonstrate similar high overall sensitivity (79.0%), specificity (92.7%), and positive Prediction Value (88.2%) to identify a successful ablation site with this newly proposed ECG algorithm. Conclusion On the basis of detail 3D electroanatomical mapping of successful ablation sites, a newly proposed ECG algorithm was developed to improve the sensitivity, specificity, and positive Prediction Value in identification of targeted ablation sites for RVOT-VPC.

    Objective:To investigate the difference and relationships of serum vascular cell adhesion molecule-1(VCAM-1) and P-selectin levels in different type of coronary heart diseases and explore the potential predicting factors for the acute coronary syndrome(ACS).Methods:The levels of serum sVCAM-1 and sP-selectin in 29 acute myocardial infarction(AMI),31 unstable angina(UA),30 stable angina(SA) and 30 normal healthy control subjects were measured by ELISA method,comparing the Values of sVCAM-1 and sP-selectin among different.Results:Serum sVCAM-1 and sP-selectin levels were significantly higher in the ACS group compared with SA group and control group.There was moderate correlative relationship between sVCAM-1and sP-selectin and LDL-C.For ACS,the sensitivity was 72.8%,specificity was 83.1%with serum sVCAM-1 and sP-selectin levels.Conclusion:The elevated VCAM-1 and P-selectin levels suggested an obvious relationship with pathogenesis of ACS,which may be a potential markers of plaque instability and may be newly sensitive markers for identifying patients with ACS.

  • The Prediction Value of scoring systems in Mallory-Weiss syndrome patients

    Mallory-Weiss syndrome (MWS) is a relatively less common cause of nonvariceal upper gastrointestinal bleeding. There is limited data on whether scoring systems could be used to predict the clinical outcomes in patients with bleeding due to MWS. The aim of our study is to evaluate whether the Glasgow-Blatchford score (GBS), AIMS65, and shocking index are effective in predicting the clinical outcomes of MWS.One hundred twenty-eight patients from January 2010 to January 2017 with MWS in middle China were enrolled. Clinical features such as age, gender, causes of vomiting, endoscopic findings, GBS, AIMS65, and shocking index were recorded. The clinical outcomes including endoscopic treatment and transfusion were analyzed.MWS accounted for 6.1% of nonvariceal upper gastrointestinal bleeding. Male-to-female ratio was 3.6:1 and median age was 51 years. Patients between 40 and 60 years were more commonly affected; 43.8% of MWS was caused by overdrinking followed by underlying gastric diseases (33.6%). However, for female patients alone, underlying gastric diseases were the leading cause (42.9%). The tears were usually single and most frequently located on the left lateral wall. In receiver-operating characteristic curve analyses, GBS system and shocking index were useful in predicting transfusion (0.856 vs 0.675). But for endoscopic intervention, these scoring systems are not helpful (P > .05).Apart from drinking, underlying gastric disease is another important cause of MWS especially for female patients and should be paid more attention under endoscopy examination. GBS system and shocking index can be used to predict transfusion.
