The influence of two different dental local anaesthetic solutions on the haemo-dynamic responses of children undergoing restorative dentistry: a randomised, single-blind, split-mouth study

Cole, R R WelburyAbstract:Objectives This investigation was designed to study the haemodynamic effects of two different local anaesthetic solutions during restorative dental treatment in children. Design A randomised, single-blind, split-mouth cross-over design was employed using children undergoing bilaterally similar restorative treatments over two visits. Setting The study was performed in a dental hospital paediatric dentistry department. Methods Ten children participated. At one visit the local anaesthetic was 2% lidocaine (lignocaine) with 1:80,000 epinephrine (adrenaline); at the other the anaesthetic was 3% prilocaine with 0.03IU/ml felypressin. Local anaesthetic was administered at a dose of 0.5 ml/10 kg body weight. Blood Pressure and heart rate were measured before and during treatment with an automatic blood Pressure Recorder. Data were analysed by ANOVA and Student's paired t test. Results Significant differences between treatments in diastolic blood Pressure (F = 2.37; P = 0.05) and heart rate (F = 2.98; P < 0.02) were noted. The heart rate increased ten minutes following the injection of the epinephrine-containing solution. The diastolic blood Pressure fell 20 minutes after injection of lidocaine with epinephrine. Conclusion The choice of local anaesthetic solution influences the haemodynamic response during restorative treatment in children.

Five years of ground deformation monitoring on axial seamount using a bottom Pressure Recorder

FoxAbstract:The results of five years of Pressure measurements collected by a Bottom Pressure Recorder deployed within the summit caldera of Axial Seamount, on the central Juan de Fuca Ridge, are interpreted for monotonic offsets related to vertical seafloor movements. Results indicate five significant subsidence events during the observation period (3.7 years), or an average of 1.35 events per year. The displacements range from 3–10 cm with durations of 10–14 days. These results indicate an active volcanic system and can be viewed as positive evidence for future attempts at seafloor volcanic observatories and response to active events on oceanic ridge crests. Advanced instrumentation based on multiple sensors has been deployed at the site to reinforce the results of this pilot experiment.

Journal of Epidemiology and Community Health, 1992Co-Authors: P H Whincup, Nigel Uce, Derek G Cook, A G ShapeAbstract:STUDY OBJECTIVE--The aim was to compare the performance of the Dinamap 1846SX automated oscillometric blood Pressure Recorder with that of the Hawksley random zero sphygmomanometer during use under field study conditions. DESIGN--Two independent within subject measurement comparisons were made, one in adults and one in children, each conducted in three stages over several months while the Dinamap instruments were being used in epidemiological field surveys. SETTING--The studies were done in outpatients clinics (adults) and primary schools (children). PARTICIPANTS--141 adults (20-85 years) and 152 children (5-7 years) took part. MEASUREMENTS AND MAIN RESULTS--In adults a pair of measurements was made with each instrument, the order alternating for consecutive subjects. In children one measurements was made with each instrument, in random order. Measurements with the Dinamap 1846SX were higher than those with the random zero sphygmomanometer both in adults (mean difference 8.1 mm Hg; 95% CI 6.5 to 9.7 mm Hg) and in children (mean difference 8.3 mm Hg; 95% CI 6.9 to 9.7 mm Hg). Diastolic measurements were on average very similar both in adults and in children. The results were consistent at all three stages of both studies. The differences in systolic measurement were independent of blood Pressure level. However, the extent of agreement in diastolic Pressure depended on the diastolic blood Pressure level; in both studies Dinamap diastolic measurements were higher at low diastolic Pressures while random zero diastolic measurements were higher at high diastolic Pressures. CONCLUSIONS--Systolic measurements made with the Dinamap 1846SX instrument are not directly comparable with those of the Hawksley random zero sphygmomanometer and are unlikely to be comparable with those of earlier Dinamap models. These differences have important implications for clinical practice and for comparisons of blood Pressure measurement between epidemiological studies. However, the consistency of measurement by the Dinamap 1846SX over time suggests that the instrument may have a place in standardised blood Pressure measurement in the research setting.
