ParryAbstract:Objective The study objective was to review our initial experience with newly developed off-pump pulmonary valve implantation techniques and compare outcomes with the conventional approach. Methods Thirteen symptomatic patients with severe pulmonary regurgitation underwent pulmonary valve implantation, 6 without cardiopulmonary bypass (group 1: age, 28 ± 21 years; range, 12-62; body surface area range, 1.38-2.39 m 2 ) and 7 with cardiopulmonary bypass (group 2: age, 23 ± 13 years; range, 10-46; body surface area range, 1.31-1.89 m 2 ). Ten patients had previous repair of tetralogy of Fallot, and 3 patients had pulmonary valvotomy/valvuloplasty. Results Mean operation times were 166 minutes (range, 110-240) in group 1 and 299 minutes (range, 221-375) in group 2 ( P P 001). Postoperative chest drainage (median) was 78 and 300 mL in groups 1 and 2, respectively ( P = .003). Blood Product Requirement was zero and 3 units (median) in groups 1 and 2, respectively ( P 014). There was no significant difference in postoperative ventilation time or lengths of intensive care unit and hospital stays between the 2 groups. Mean follow-up was 15 months; all patients are in New York Heart Association I/II. Echocardiography showed that peak velocity across the pulmonary valve was 2.2 and 2.0 in groups 1 and 2, respectively ( P = .46). No patient had a paravalvular leak or more than mild pulmonary regurgitation. Conclusions Off-pump pulmonary valve implantation is a good alternative for pulmonary valve replacement. The procedure reduces operating time, blood loss, and blood Product Requirement.

Fast Diazaborine Formation of Semicarbazide Enables Facile Labeling of Bacterial Pathogens

Propuesta de mejora de gestión por procesos para incrementar la rentabilidad en la empresa extintores Santa Rosa S.R.L.

A través del diagrama de Ishikawa se lograron identificar 6 causas raíz que contribuyen al problema. A continuación, se estimaron las pérdidas que generaban las causas identificadas haciendo un total de 15’498.57 soles al año. Estas pérdidas son causadas por el reprocesamiento de no conformidades y una deficiente gestión logística, de modo que la propuesta de mejora está enfocada en aplicar herramientas de ingeniería como Descripción de Funciones, Procesos de Inducción y Capacitación, y Procedimientos Escrito de Trabajo para la reducción de no conformidades; y en cuanto a técnicas de gestión logística y de almacén, se propone la aplicación de Codificación y Ubicación de Materiales, Control de entrada y salida de materiales así como establecer un procedimiento de Requerimiento Interno y aunarlo a una Planeación de Requerimiento de Materiales. A Quality Improvement Model Based on Taguchi’s Loss Function Considering Imperfect Quality Inspection

In manufacturing, customers have a high quality Product Requirement. In producing high quality Products, the manufacturer must improve the Production process continuosly. Taguchi’s loss function is a common model to quantify the quality loss. Taguchi’s loss function can be applied to nominal the best, smaller the better, and larger the better quality characteristic. In this study, we developed an optimization model to determine the optimal process parameters by taking into consideration of the imperfection of quality inspection. The imperfect quality inspection involves two kind of errors, namely Type I and Type II errors. Inspectors may make an error in categorizing non-defective items as defectives and some defective items as non-defectives. The inspection errors will increase the quality cost as one components of total cost considered in this study. From the numerical example, the calculation results are the expected cost for 100% inspection is $ 3.1395, and the expected cost for proposed model is $ 3.0898 with the optimal process mean for the proposed model is µ _ n = 15.411 mm, the optimal standard deviation is σ _ n = 0.20076 mm and the optimal multiplier of standard deviation is r = 3.996.

in vitro assessment of the broad spectrum ultraviolet protection of sunscreen Products
Journal of The American Academy of Dermatology, 2000Co-Authors: B L Diffey, Paul Robert Tanner, P J Matts, Frank J NashAbstract:Abstract Background: There are considerable data to suggest that protection from solar ultraviolet (UV) radiation will reduce the risk of acute and chronic skin damage in humans. Whereas the sun protection factor (SPF) provides an index of protection against erythemally effective solar UV, largely confined to the UVB (290-320 nm) and short-wavelength UVA (320-340 nm) region, there is currently no agreed-upon method to measure broad-spectrum protection against long-wavelength UVA (340-400 nm). Objective: The objective of these studies was to assess the potential of in vitro UV substrate spectrophotometry and subsequent calculation of the "critical wavelength" value as a measure of broad-spectrum UV protection and as a routine, practical procedure for classification of sunscreen Products. Methods: The spectral absorption of 59 commercially available sunscreen Products and multiple experimental formulas with one or more UV filters was measured. Sunscreen Product, 1 mg/cm 2 , was applied to a hydrated synthetic collagen substrate, preirradiated with a solar simulator, and then subjected to UV substrate spectrophotometry. Multiple determinations from 5 independent samples per Product were used to calculate the critical wavelength value, defined as the wavelength at which the integral of the spectral absorbance curve reached 90% of the integral from 290 to 400 nm. Results: We found that a recognized long-wave UVA active ingredient such as titanium dioxide, zinc oxide, or avobenzone is a necessary but insufficient Product Requirement for achieving the highest proposed broad-spectrum classification, that is, critical wavelength of 370 nm or more. Although SPF and critical wavelength are largely independent of each other, UVA absorbance must increase commensurate with SPF to maintain the same critical wavelength value. Substrate spectrophotometry and the calculation of critical wavelength can readily account for sunscreen photostability by UV preirradiation. Finally, there is also a strong positive relationship between critical wavelength and a currently available in vivo measure of UVA protection. Conclusion: Determination of critical wavelength by means of UV substrate spectrophotometry provides a rapid, inexpensive, and reliable measure of broad-spectrum protection, which is largely independent of SPF, yet ensures long-wavelength UVA protection commensurate with SPF. The procedure provides a routine, sensitive means of differentiating and classifying sunscreen Products and, importantly, obviates the need to subject volunteers to acute exposures of high-dose, nonterrestrial UV, the health risks of which are still poorly understood.
