    While Professional trainings in psychotherapy and counselling vary considerably in the attention they pay to assessment, courses, conferences and workshops devoted to the subject are attracting an audience eager for more demonstration and discussion. In response, The Art and Science of Assessment in Psychotherapy offers an extended symposium on principles and practice. Guided by a belief that comparative study will spur critical reflection and innovation, its presentations encompass a wide range of therapeutic orientations, settings and interests. In the book twelve distinguished practitioners of widely used approaches within psychotherapy describe the methods they use in their assessments and how these have developed. They also discuss the uses and limitations of the therapies they offer. The approaches covered include psychoanalytic psychotherapy, in-patient psychotherapy, family therapy, group psychotherapy, psychodrama, cognitive-behaviour therapy, couple therapy and focal therapy. Additional chapters look at assessment in the light of psychotherapy research, question how far assessment can be separated from treatment, and ask whether the use of questionnaires and special tests aids or detracts from interviewing as a method of assessment. Without autonomy to practice their skills freely as primary health care providers, the roles of NPs will most likely be limited in any reformed health care system.     Abstract:

    Since its establishment over 40 years ago, the genetic counseling profession has grown to an estimated ~4,000 Professionals in North America. While the profession has maintained the Master’s degree as the entry-level and terminal degree, many other allied health professions have added advanced training pathways, such as the clinical doctorate (ClinD) either as an optional post-Professional degree or required entry-level degree. Discussions regarding advanced degrees have also occurred within the genetic counseling profession, dating back to as early as the 1980s. In 2011, the Genetic Counseling Advanced Degree Task Force (GCADTF) was convened to explore the Issue again, with the goal of “[engaging] all of the Professional leadership organizations in the field of genetic counseling in a decision-making process about whether the profession should move to a Clinical Doctorate”. As part of their work, the GCADTF surveyed practicing genetic counselors (n = 4,321) and genetic counseling students (n = 522) in the US and Canada regarding their interest in moving to the ClinD as the entry-level degree. This survey also included questions about other options for advanced training to generate data to inform future discussions around this very important Professional Issue. Herein, we describe the results of the survey, with particular attention to genetic counselor preferences for additional advanced education/certification opportunities and recommendations for future discussion.

    Abstract Through an exploration of the theory of the avant-garde, this paper explores the task for nursing in the new humanities of the 21st Century. Drawing on the theory of the avant-garde in general and the work of the French philosopher Jacques Derrida in particular, it argues that nursing must always return to its basic principles, that of the human condition ( humanitus ). But in doing so, it must also consider its place at the table of the humanities that are undergoing profound changes in western capitalist societies both in the education and practice sectors. It is through its connection with the humanities that the notion of the avant-garde is used to introduce the concept of reconnaissance —thinking again. The purpose and usefulness of this approach for nursing lies in the argument that progress or improvement, whether it be in some aspect of care or Professional Issue, always involves returning to something, to think of something in a new way. Thus, although nursing still has certain Issues to resolve (which I attend to below), thinking through these Issues with reference to the avant-garde gives ground for optimism, towards a future that is not yet determined.

    This thesis contributes knowledge to the areas of Reflective Practice and Clinical Effectiveness, both Professional Issues in clinical psychology, which have a direct link to patient care in the National Health Service (NHS). The literature review presented in Chapter 1 focuses on reflective practice. The review aimed to identify all published empirical research, which investigated the application and usefulness of reflective practice for staff qualified in healthcare professions, and working in health services specifically, including the NHS. The findings have clinical and Professional implications for health care staff, their managers, and the broader organisations in which they work. The use of reflective practice has been linked in the literature to increased clinical effectiveness, which is the second Professional Issue. The empirical paper presented in Chapter 2 explored the measurement and demonstration of clinical effectiveness as experienced by clinical psychologists working within NHS using a qualitative methodology. Demonstrating clinical effectiveness was found to be particularly relevant given the current, changing financial climate of the NHS and a context in which clinical psychologists are expected to prove their Professional worth. The review and empirical paper highlight the importance of practice-based evidence. Chapter 3 presents a reflective paper, which explores the impact of the review and research findings on the author who is approaching the end of her clinical psychology training. This paper may be of interest to more experienced clinicians who wish to understand the Issues from a newly qualified perspective, or to readers interested in the transition from training to Professional practice.
