    Abstract Early in 2015, I was asked to chair the judging panel for the 2015 International Psychogeriatric Association (IPA) Junior Research Awards in Psychogeriatrics, and was pleased to help in this way for the second Congress in succession. The other five judges who kindly gave up their time to assist with the selection of the three prize winners were Nicola Lautenschlager, an old age psychiatrist from Australia, co-opted to the panel in her role as the current Editor-in Chief of International Psychogeriatrics (the three winning papers are published in this issue as has been traditional for over 25 years), Glenna Dowling, a nurse from the USA who is also an associate editor of International Psychogeriatrics, Karen Ritchie, a psychologist working in France, Martina Roes, a nurse turned sociologist from Germany, the host country for the 2015 IPA Congress, and Huali Wang, a psychiatrist from Beijing, China. Depending on how you look at it, our panel consisted of three psychiatrists, one psychologist, two nurses and a bonus sociologist, five judges from developed countries and one from a developing nation, or five women and one man, which I think is the first time that such an IPA prize judging panel has had a female majority.

    In 2014, the International Psychogeriatric Association (IPA) celebrated the fact that IPA's flagship scientific journal International Psychogeriatrics (IPG) has matured, attaining 25 years of existence since its foundation in 1989. The IPG editorial team introduced several new features in the 2014 issues to celebrate this significant quarter century milestone. One of them was the new “paper of the month” category. Following an internal selection process amongst the editorial team, who ranked available accepted papers focusing on their scientific quality and clinical relevance each month, one paper was selected as paper of the month from the categories “original research articles” and “reviews”. This resulted in 12 papers of the month, of which eight were original research articles and four were reviews or meta-analyses. Each paper of the month was accompanied by a short commentary highlighting its importance, written either by an editorial team member, one of the reviewers of the paper, or an external international expert on the topic of the paper. The editorial team aimed to highlight various psychogeriatric topics, so there are papers covering the wide spectrum of cognitive impairment, from risk factors such a hippocampal atrophy (Ferrarini et al. , 2014 ) and traumatic brain injury (Gilbert et al. , 2014 ), via transition between diagnostic categories on the cognitive continuum (Ellis et al. , 2014) to variations in symptom expression (Rockwood et al. , 2014 ) and apathy (Theleritis et al. , 2014) to exercise interventions for people with Alzheimer's disease (AD) (Farina et al ., 2014) or antipsychotic use for patients with dementia living in residential care (Kleijer et al. , 2014 ). Other papers reported on topics relevant to the aging process in general, such as neuroplasticity (Valkanova et al. , 2014 ), loneliness (Yan et al. , 2014 ) or attitudes to aging (Shenkin et al. , 2014 ). Other psychogeriatric topics covered were delirium (Ritchie et al. , 2014 ) and geriatric depression (Steffens et al. , 2014 ). In the international spirit of IPA, authors of the 12 papers come from Australia, Canada China, Greece, Germany, Italy, Mexico, the Netherlands, Switzerland, the UK, and the USA. From those 12 papers an IPG paper of the year 2014 was selected with the help of three independent reviewers: Dr Doh Kwan Kim from South Korea, Dr Gill Livingston from the UK, and Dr Daniel Weintraub from the USA. Our three reviewers ranked all 12 papers of the month independently from each other and the highest ranked paper overall was “The association of traumatic brain injury with rate of progression of cognitive and functional impairment in a population-based cohort of Alzheimer's disease: the Cache County Dementia Progression Study” by Mac Gilbert, Christine Snyder, Chris Corcoran, Maria C. Norton, Constantine G. Lyketsos, and JoAnn T. Tschanz. The reviewers emphasized that traumatic brain injury (TBI) is recognized as an important risk factor for AD which needs more research, and that the study by Gilbert et al. contributes important new knowledge to the area with the specific strength of reporting long-term follow up. In this context, we refer to the excellent commentary on the paper of the year by Dr David Sharp (Sharp, 2014 ), which was published alongside the paper of the year when it was paper of the month in the October 2014 issue.

    Abstract:

    Judging for the 2013 International Psychogeriatric Association (IPA) Junior Research Awards in Psychogeriatrics was undertaken by a panel of six experts, selected by the IPA executive, which I had the honor to chair. All three award-winning papers appear in this issue of International Psychogeriatrics immediately following this guest editorial. I am confident that, like their many predecessors awarded over more than two decades, they will be highly cited (Pachana, 2012) and will be seen in due course as crucial to the development of the young and very promising researchers who have received this prestigious acknowledgment of their excellent work.

    To open volume 22 of International Psychogeriatrics we take great pleasure in publishing the papers submitted by the three recipients of the 2009 International Psychogeriatric Association (IPA) Junior Research Awards in Psychogeriatrics. As you can see from our new 2010 front cover, all of them accepted their prizes in person on Tuesday 1 September 2009, at the opening session of the IPA 14th International Congress in Montreal, Canada, where they were congratulated by George Grossberg (chair of the 2009 IPA Junior Research Awards committee) and Serge Gauthier (chair of the 2009 IPA 14th Congress). One of the highlights of the Congress was the plenary session on the afternoon of Thursday 3 September, which I was privileged to chair, at which the three prize recipients presented their research findings to a large and appreciative audience.

    Abstract:

    When we manage our patients both they and we would like to know that the interventions we prescribe have been tested and shown to be safe and effective for the uses to which they are put. The most powerful tool to determine the utility of specific interventions in the discipline of medicine is the double-blind placebo-controlled randomized clinical trial (RCT). Some of the complex problems encountered in Psychogeriatrics do not lend themselves to straightforward yes or no outcomes, and some of the multifaceted interventions developed for the management of common psychogeriatric syndromes are difficult to test using standard RCT design, especially with regard to effective blinding and appropriate control conditions (Llewellyn-Jones et al. 1999; Haynes, 1999; Ames, 1999). Nevertheless, there are specific interventions for which RCT data have been very useful in refining treatment guidelines and advice (e.g. Doody et al. , 2001) and, where this is the appropriate trial design, RCTs comprise the “gold standard” by which to assess the efficacy of a treatment or “management package”.

  • Plasma Homocysteine and Vascular Disease in Psychogeriatric Patients.

    Background: There is a high frequency (40–50%) of elevated plasma total homocysteine (tHcy) concentrations in elderly patients with mental disorders, and patients with a history of vascular disease exhibit significantly higher plasma tHcy concentration than patients without vascular disease. Method: The main objective of the present study was to further investigate the association between plasma tHcy concentration and vascular disease in psychogeriatric patients. We have therefore investigated 304 psychogeriatric patients and determined plasma tHcy and its most important determinants (folate and cobalamin status and renal function), and the natriuretic peptide N-terminal-pro brain natriuretic peptide (NT-proBNP). The patients were classified into several groups of vascular disease according to the findings of brain imaging and presence of a history/symptoms indicating manifest occlusive arteriosclerotic vascular disease. Results: Plasma tHcy concentration is associated with the presence of vascular disease in psychogeriatric patients. The presence of vascular disease is also associated with higher age, higher serum NT-proBNP, renal impairment and lower serum folate concentration than in patients without vascular disease. The significant association between plasma tHcy concentration and vascular disease remained after correction for age and for cystatin C differences between the groups of patients without and with vascular disease. In the present population with only 16% of the patients showing elevated plasma tHcy, renal function was a more important determinant for plasma tHcy concentration than folate status. Conclusion: Plasma tHcy concentration is associated with vascular disease. In the present population of psychogeriatric patients renal function is associated with vascular disease and elevated plasma tHcy concentration. Thus, the association between plasma tHcy concentration and vascular disease might partially be explained by impairment of renal function.

    Objective. In recent studies high plasma total homocysteine (tHcy) levels were reported to be associated with increased risk of osteoporotic fractures. In elderly psychogeriatric patients there is a high frequency of elevated plasma tHcy concentration. The present study therefore investigates the association between plasma tHcy concentration and markers of bone metabolism in psychogeriatric patients. Material and methods. A total of 152 psychogeriatric patients were investigated and plasma tHcy and its major determinants (serum folate, serum cobalamin and renal function) were measured. Osteocalcin and crosslaps were chosen as markers of bone metabolism. Results. Bone markers (crosslaps and osteocalcin) were increased in elderly patients with dementia compared to patients without dementia. Stepwise multiple regression analysis showed that plasma tHcy concentration made only a small contribution to the prediction of crosslaps in serum, whereas plasma tHcy concentration was not an independent predictor of serum osteocalcin. Conclusions. The present study does not provide support for the hypothesis that a moderately increased plasma tHcy concentration is a risk factor for impaired bone metabolism.

The patients were divided into two groups according to the presence of vascular disease. Eighty-seven patients had concomitant vascular disease. We also analysed the natriuretic peptide N-terminal pro brain natriuretic peptide (NT-proBNP) and protein S-100B in serum. NT-proBNP is a marker for congestive heart failure, whereas protein S-100B is a marker for brain damage. Results: The plasma tHcy concentration is elevated in the presence of dementia or vascular disease in psychogeriatric patients. The presence of dementia or vascular disease is also associated with higher age, renal impairment and lower serum folate concentration than in patients without dementia or vascular disease. Furthermore, we observed elevated serum concentrations of NT-proBNP in patients with dementia or vascular disease as a sign of poorer cardiovascular status. Likewise, protein S-100B concentrations were elevated in patients with dementia or vascular disease, possibly indicating brain damage in these groups of patients. Conclusion: The high frequency of comorbidity of vascular disease and mental illness indicates a possibility to prevent and treat psychogeriatric disease by actively counteracting vascular disease in patients with psychogeriatric symptoms. Routine determination of NT-proBNP is valuable for obtaining information about cardiovascular status.
