    Abstract:

    In 1977, a survey of American and Canadian dental schools was conducted by Weintraub to determine the delegation of laboratory procedures in preclinical and clinical complete and Removable partial denture programs. This was performed in light of challenges raised to the value of requiring dental students, rather than laboratory technicians, to perform the various laboratory procedures involved in the fabrication of complete and Removable partial dentures. Since 1977, the rate of edentulism has declined in the United States, and fewer edentulous patients have sought regular dental care. In addition, approximately 20% fewer curriculum hours have been spent in dental schools in Removable prosthodontic instruction; a declining number of graduating dental students have felt comfortable with their level of knowledge of Removable Prosthodontics, especially complete dentures, and have performed fewer Removable prosthodontic procedures in practice because of a perceived lack of treatment cost-effectiveness. The present study, an 18-year follow-up to the 1977 survey, was designed to determine if such trends toward a deemphasis on Removable prosthodontic treatment have affected the mechanisms of instruction in and the extent to which dental schools instruct students about Removable Prosthodontics laboratory procedures. The 1995 survey was mailed to 53 American dental schools. Forty-six schools returned the completed survey, for a response rate of 87%. Results showed that the proportion of schools with Removable prosthodontic preclinical programs, especially those in Removable partial dentures, increased, as did the relative amount of material presented to students in these programs. This was accomplished primarily through the enhancement of laboratory technician support, both in-house and commercial, of student preclinical Removable prosthodontic laboratory procedures. The conflict between these outcomes and the call by the dental community, as well as that of the 1995 Institute of Medicine Study, for a reduced emphasis on teaching students about the technical details involved in Removable prosthesis fabrication is recognized and explored.

    Today, dental implant therapy is considered a valuable asset of mainstream dental therapeutics by both patients and clinicians. In many ways, the availability of implant therapy to facilitate the support, stability, and retention of dental prostheses has revolutionized the profession and the procedures dentists accomplish on a daily basis. From another perspective, the procedures used to fabricate dental implant restorations are but modifications of previously existing, proven, and reliable techniques. This is particularly true when considering impression making for implant overdentures. Part 3 of this article series looks at the use of vinyl polysiloxane impression material systems for making definitive impressions for implant overdentures. Once constructed, it is critical that all Removable dental prostheses possess external contours that are geometrically compatible with the anatomic and functional requirements of the oral tissues. Therefore, this article also will address a technique for diagnostically assessing external denture contours using vinyl polysiloxane external impression procedures.

    : Recent advances in impression materials and stock edentulous impression trays have resulted in simplified approaches to impression making in Removable Prosthodontics. Once considered an absolute necessity, it is now possible to avoid the need for custom impression trays. In an effort to achieve reliable master casts in a single appointment, new and innovative procedures are now available. This article, the first in a 3-part series, will review historical information, basic concepts, materials considerations, and philosophic approaches to impression making in complete-denture therapy. A modern technique using readily available impression materials will be described and illustrated so readers can consider the benefits of incorporation into their daily management of edentulous patients.

    Abstract:

    Objectives The aim of this study was to systematically examine the outcomes of interest of trials in Removable Prosthodontics and to assess their overall quality. Material and methods Electronic databases were searched up to August 2011 and complemented by hand searching for published randomized controlled trials (RCTs) in the field of Removable Prosthodontics in the six major prosthodontic and implant journals. The primary outcome of RCTs was considered as the outcome of interest of this review, and only manuscripts in which the study's primary outcome was reported in the abstract were included. Outcomes were classified according to the Strength of Recommendation Taxonomy (SORT) classification. The quality of individual reports of RCTs was assessed following the Cochrane Handbook for Systematic Reviews of Interventions (Higgins & Green, www.cochrane-handbook.org). Results The search retrieved 86 reports of RCTs. Among these publications, 43% reported a patient-oriented primary outcome. Most of the publications did not present a clear description of sequence generation (54%) or allocation concealment (65%). Blinding of participants, clinicians, and outcomes assessors was not applicable or reported in most trials. Handling of withdrawals and losses was adequate in 52% of trials. Conclusions Although published RCTs in Removable Prosthodontics have increased over time, efforts should be made to increase patient-reported outcomes and the methodological quality of these reports. Such improvement will lead to a better body of evidence in the field, thus providing increased support for clinical decision-making.
