    Abstract:

    This paper presents a case of co-design for people with Rheumatic Disorder to support the argument of opening up the design space to include interaction opportunities found in the physical world. The position argued for is that opening up the design space beyond common screen-based interfaces may contribute to the design of enabling technologies for people with Rheumatic Disorders by acknowledging their varying capabilities during both design and use. The presented results consist of one thematic analysis of home interviews and group discussions as well as one statistical analysis of the results from a formative evaluation of six conceptual prototypes developed along with the participants. The paper uses the combination of the thematic analysis, the six conceptual prototypes, and formative evaluation of performance scores and preference ratings to demonstrate how our co-design process involving users with Rheumatic Disorder in all phases allowed participants to discover both limitations and opportunities as they explored and co-designed alternative concepts.

    Spondyloarthritis (SpA) is a chronic inflammatory Rheumatic Disorder characterized by joint manifestations affecting the spine, peripheral joints and entheses, as well as extra-articular manifestations such as psoriasis, uveitis, or intestinal inflammation. This complex Disorder has a strong genetic component dominated by the HLA-B27 antigen of the major histocompatibility complex class I (MHC-I), which is present in more than 80% of SpA patients. Discovered 45 years ago, the association between HLA-B27 and SpA development remains unexplained. Several hypotheses have been proposed to explain this association at the molecular level, but all face experimental inconsistencies that seem to invalidate them. Therefore, it appeared to us essential to elaborate new and yet unexplored approaches in order to better understand the molecular role of HLA-B27 in SpA development. Drosophila melanogaster is a powerful genetic model that has led to considerable advances in understanding numerous functions of metazoan cells, as well as in describing the cellular and molecular processes of many human pathologies. To elucidate the molecular pathogenic mechanisms associated with HLA-B27, we have established several transgenic Drosophila lines for SpA-associated and non-associated of HLA-B alleles, as well as for the MHC-I invariant chain, the human 2-microglobulin (hβ2m). Expression of the HLA-B27 alleles, in the presence of hβ2m, in the Drosophila wing and eye led to two specific phenotypes. The crossveinless wing phenotype is due to a disturbance in the Bone Morphogenetic Protein (BMP) signaling pathway. Interestingly, this misregulation is associated with a co-localization of HLA-B27 and the BMP type I receptor named Sax. Our preliminary results obtained in SpA patient cells suggest that HLA-B27 also colocalizes with ALK2 receptor, which is ortholog to Sax. Altogether, our results suggest that the pathogenic role of HLA-B27 in SpA may depend on a BMP signaling misregulation at the crosstalk between ossification and inflammation.

  • Systematic Review of Psychological Treatment in Fibromyalgia

    Fibromyalgia (FM) is a debilitating Rheumatic Disorder characterized mainly by the presence of continual and widespread musculoskeletal pain, in addition to other disturbing symptoms. There is inconsistent evidence about the effectiveness of the treatments developed so far, making FM a chronic disease that is difficult to treat. The aim of this literature review was to analyze the empirical studies about psychological treatment of FM that have been published over the last twenty years. We conducted a literature search of studies published between 1990 and 2012 using Medline and PsycINFO in the Ovid and ProQuest platforms and hand searching. In total, 58 original studies were identified. The present review presents a comprehensive analysis of the main characteristics of these studies and a description of the interventions developed in order to improve FM symptoms. The most used intervention modality was group treatment with a cognitive-behavioral approach. We also found intensive and remote treatments as well as multimodal therapy, hypnosis, cognitive-behavioral therapy for insomnia, behavioral therapies, mind–body–based techniques, and biofeedback components. Finally, we discuss the clinical relevance of addressing the symptoms of patients with FM and its scientific validation.

    Abstract:

    Background Systemic sclerosis (SSc) is a Rheumatic Disorder characterized by inflammation and fibrosis involving the skin as well as internal organs, including the lung. Objectives This study was conducted to characterize SSc patients with ILD and determine predictor features for the presence of ILD. Methods A retrospective evaluation of SSc patients from our Rheumatology department was undertaken. ILD was defined according to imaging findings in high-resolution CT (HRCT) combined with functional parameters. Demographic features, auto-antibodies, spirometry, diffusion capacity for carbon monoxide (DLCO) measurement, echocardiography and 6-minute walk test were compared between groups with and without ILD using parametric tests and non-parametric tests. Predictor factors were established by logistic regression analyses. Results One hundred and three SSc patients with current mean age of 60.2±14.1y and mean disease duration of 10.0±9.6y were evaluated. Thirty-four (35.8%) patients were diagnosed with ILD, of whom 13 had limited cutaneous SSc, 14 had diffuse cutaneous SSc, 2 sine sclerodema and 5 overlap syndroms. ANA was positive in 100% cases, anti-Scl70 was found in 55% and anti-centromere antibody positivity was found in 15% of patients. HRCT revealed diffuse parenchymal lung disease involvement >20% in 18 (53%) cases, with the majority showing a predominant honeycombing pattern. Spirometry showed a restrictive pattern in 8 patients and 1 had obstruction. The DLCOsb was abnormal in 7 patients. Three patients had pulmonary hypertension confirmed by right heart catheterization, 2 patients classified as group 1 and 3 and 1 patient of group 5. About 63% of ILD patients were ever treated with corticosteroids and 27% with cyclophosphamide, which were the most common drugs used. Two patients were also treated with rituximab. In this ILD-group, 5 deaths were recorded, 2 due to cancer, 1 abdominal sepsis, 1 pulmonary hypertension and 1 of unknown cause. Comparison of these ILD patients with non-ILD patients revealed some significantly differences (Table 1). Multivariate analysis showed that digital ulcers (OR=31.6, 95% CI 3.09 to 321.91), Anti-Scl70 antibody positivity (OR=10.1, 95% CI 1.19 to 85.74) and anti-centromere antibody negativity (OR=0.08, 95% CI 0.01 to 0.83) were independently associated with the presence of ILD. Conclusions This study is one of the first studies carried out in Portugal regarding lung involvement in SSc. About one third of the patients had ILD, whose main characteristics are in accordance with other European cohorts. We confirmed that anti-SCL70 positivity, presence of digital ulcers as well as the absence of anti-centromere antibody is independently associated with ILD Disclosure of Interest None declared
