A kórkép hátterében az esetek kétharma- dában a mismatch-repair gének – leggyakrabban MutS Homolog-2 és MutL Homolog-1 – örökletes mutációja áll. A diagnózis felállításához legalább egy faggyúmirigy-eredetű bőrtumor (Sebaceoma, sebaceus adenoma, epithelioma, carcinoma vagy bazálsejtes carcinoma sebaceus differenciációval) és/vagy keratoacanthoma, valamint legalább egy belszervi daganat (többnyire jobb kolonfélre lokalizált carcinoma) jelenléte szükséges. A szerzők egy 52 éves férfi kórtörténetét mutatják be, akinek hátán halmozottan jelentkeztek sebaceus carcinomák. Immunhisztokémiai vizsgá- latok alapján a daganatsejtek nem expresszáltak MutL Homolog-1 proteint. Részletes daganatkutatás eredményekép- pen coecumtáji malignus folyamatra derült fény. The most common benign neoplasms occurring in nevus sebaceus are believed to be syringocystadenoma papilliferum, trichilemmoma, and trichoblastoma. A patient with nevus sebaceus developed not only syringocystadenoma papilliferum but also prurigo nodularis within her hamartomatous lesion; multiple biopsies were necessary to establish the diagnoses. Excision of the residual nevus sebaceus also revealed an apocrine cystadenoma, basaloid follicular proliferation, and Sebaceoma. Also, it is important to select the appropriate biopsy site and size when evaluating a patient for secondary neoplasms within their nevus sebaceous. Indeed, more than one biopsy may be required if additional diagnoses are suspected.

HNPCC is an autosomal dominant cancer predisposition syndrome characterised by early onset of colorectal cancer and other associated tumours.8,9 Several genes underlying HNPCC which are involved in DNA mismatch repair (MMR) have been identified within the last decade.10–13 Germline mutations in the DNA MMR genes were detected in a high proportion of MTS patients, demonstrating that MTS most often represents a phenotypic variant of HNPCC.14,15 Due to the underlying genetic mechanisms of tumourigenesis, tumours of these MTS patients exhibit high microsatellite instability (MSI-H), the characteristic feature of HNPCC tumours.16 Microsatellite analysis in tumour tissue of MTS patients therefore provides a useful tool to pre-select patients for mutation analysis in DNA MMR genes.3 Immunohistochemical testing for expression of the MSH2 and …

Observations A total of 20 sebaceous tumors including 18 sebaceous adenoma and two Sebaceomas of six patients with MTS were analysed. Two main clinico-dermoscopic features were observed: (1) clinically pink to white papules/nodules with a central crater, dermoscopically characterized by radially arranged, elongated crown vessels surrounding opaque structureless yellow areas at times covered by blood crusts (n = 13) and (2), clinically pink to yellow papules/nodules without a central crater, dermoscopically exhibiting a few, loosely arranged yellow comedo-like globules and branching arborizing vessels (n = 7). Confocal microscopy was available in three sebaceous adenomas and revealed a good histopathologic correlation; sebaceous lobules were composed by clusters of ovoid cells with dark nuclei and bright, highly refractile glistening cytoplasm. They were delimited by a rim of epithelial cells, corresponding to basaloid cells. Conclusions A better characterization of clinical, dermoscopic and confocal microscopy features of sebaceous tumors may improve their recognition and consequently, aid to rise the suspect for MTS.
