Sebaceous Adenoma of oral cavity report of case and comparative proliferation study with Sebaceous gland hyperplasia and fordyce s granules

In the English literature, less than 10 cases of SA have been described in the oral cavity. The objective of this study was to examine the clinicopathologic features and evaluate the expression of epidermal growth factor and its receptor, estrogen receptor and androgen receptor in SA and in its differential diagnoses including Sebaceous gland hyperplasia (SGH) and Fordyce’s granules (FG). Additionally, we analysed the proliferative potential of Sebaceous cells from SA, SGH and FG by measuring proliferating cell nuclear antigen (PCNA) expression and quantification of argyrophilic nuclear organizer regions (AgNORs). The SA showed many clinicopathologic similarities to cases previously reported including the biphasic population of cells, in the periphery of lobules undifferentiated basaloid cells whereas the central area was formed by mature sebocytes. SA was composed of 198 lobules of Sebaceous cells, whereas SGH and FG showed a mean of 21 ± 7.81 and 5.84 ± 2.83, respectively. The AgNOR and PCNA indices were similar in SA, SGH and FG. These data suggest that lobule counts may be used as additional criteria in distinguishing SA of the oral cavity from other intraoral Sebaceous gland lesions. Oral Diseases (2003) 9, 323–327

Many neoplasms have been reported to arise in association with SNJ, most commonly trichoblastoma and syringocystAdenoma papilliferum. The coexistence of 3 or more tumors arising simultaneously within 1 lesion of SNJ is very rare. We report a case of a 66-year-old woman with a coexistence of trichoblastoma, trichilemmoma, Sebaceous Adenoma, tumor of follicular infundibulum, syringocystAdenoma papilliferum, and mucoepidermoid or adenosquamous carcinoma arising in 2 separate and persistent SNJ exhibiting on her face with subsequent parotid node metastasis. This report illustrates an extraordinary case of adnexal neoplasms displaying diverse lines of differentiation arising within lesions of SNJ. We also reported the second instance of mucoepidermoid arising within the lesion of SNJ and also added Sebaceous Adenoma to the list of Sebaceous tumors arising within this lesion. Prophylactic excision or at least close clinical surveillance for sudden development of new growths is warranted.

Comparative clinicopathological study of intraoral Sebaceous hyperplasia and Sebaceous Adenoma

Clinical data, microscopical characteristics, and ki-67 immunoexpression were comparatively analyzed on 2 intraoral Sebaceous Adenomas, 6 intraoral Sebaceous hyperplasias, and 21 normal intraoral Sebaceous glands. Results. Clinically, Sebaceous glands presented as multiple separated papules, Sebaceous hyperplasias as a single enlarged papule, and Sebaceous Adenoma as a well-defined nodule. Microscopically, Sebaceous Adenoma presented an increased number of lobules, smaller lobules, and a greater number of germinative/squamous cells. Sebaceous hyperplasia also had an increased number of lobules and fewer number of germinative/squamous cells, as compared to normal oral Sebaceous glands. Ki-67 expression was seen only in germinative cells and counts were higher in Sebaceous Adenomas followed by hyperplasias and normal glands. Conclusions. Sebaceous hyperplasias and Adenomas showed different clinical, microscopic, and proliferative characteristics, suggesting the usefulness of the studied criteria on diagnosis of these uncommon oral lesions. Observations A total of 20 Sebaceous tumors including 18 Sebaceous Adenoma and two sebaceomas of six patients with MTS were analysed. Two main clinico-dermoscopic features were observed: (1) clinically pink to white papules/nodules with a central crater, dermoscopically characterized by radially arranged, elongated crown vessels surrounding opaque structureless yellow areas at times covered by blood crusts (n = 13) and (2), clinically pink to yellow papules/nodules without a central crater, dermoscopically exhibiting a few, loosely arranged yellow comedo-like globules and branching arborizing vessels (n = 7). Confocal microscopy was available in three Sebaceous Adenomas and revealed a good histopathologic correlation; Sebaceous lobules were composed by clusters of ovoid cells with dark nuclei and bright, highly refractile glistening cytoplasm. They were delimited by a rim of epithelial cells, corresponding to basaloid cells. Conclusions A better characterization of clinical, dermoscopic and confocal microscopy features of Sebaceous tumors may improve their recognition and consequently, aid to rise the suspect for MTS.
