    background. Surgical simulators are an established part of surgical training and are regularly used as part of the objective structured assessment of technical skills. Specific artificial skin models representing cutaneous pathology are available, although they are expensive when compared with pigskin. The limitations of artificial skin models include their difficulty in representing lifelike cutaneous pathology. objective. Our aim was to devise an inexpensive, reproducible surgical simulator that provides the most lifelike representation of the Sebaceous Cyst. materials and methods. Pigskin, either pig's feet/trotters or pork belly, was incised, and a paintball was inserted subcutaneously and fixed with cyanoacrylic glue. results. This model has regularly been used in cutaneous surgical courses that we have organized. Either adding more cyanoacrylic glue or allowing more time for the paint ball to absorb fluid from surrounding tissue can also adjust the degree of difficulty. conclusions. The degree of correlation with lifelike cutaneous pathology is such that we recommend that all courses involved in basic skin surgery should consider using the porcine Sebaceous Cyst model when teaching excision of Sebaceous Cysts.

    Rationale Surgical removal of a Sebaceous Cyst is usually accomplished through an external incision, which inevitably results in a scar. Herein, we described an intraoral approach for excising Sebaceous Cysts located in the lip or cheek near lip commissure, to avoid a skin scar. Patient concerns Removal of the Cyst but without leaving a scar on the face. Diagnoses Six patients were diagnosed with a subcutaneous Cyst located in the lip or cheek near lip commissure. Interventions We implemented an intraoral approach to excise the Cyst, wherein an intraoral incision was made and blunt dissection was performed through the buccinator muscle or orbicularis oris muscle until the Cyst wall was seen. The Cyst was then dissected from the surrounding subcutaneous tissue by careful blunt dissection with a hemostat and completely removed through the intraoral incision. Outcomes All patients had complete recovery, with no recurrence or complaints for at least 6 months after the surgery. Lessons A Sebaceous Cyst located in the lip or cheek near lip commissure can be excised by an intraoral approach through the mouth, which avoids a visible scar on the skin.

    Abstract:

    Summary Background Echinococcosis is a zoonotic infection, affecting humans in a characteristic geographic distribution. The disease most commonly involves the liver and the lung and a soft tissue localization is very rare. Methods A case of a 68-year-old man, who presented with a Cystic lesion on his back, which was presumed to be a lipoma or a Sebaceous Cyst. Surgical excison of the lesion was performed. Results Histopathology revealed a hydatid Cyst and with further investigations, we identified the nature of a previously undiagnosed opacity in the right lungfield. Although the patient previously presented with symptoms of hydatid Cyst rupture (productive cough with haemoptysis), echinococcosis was not under consideration. Conclusions This unusual case serves to demonstrate that echinococcosis, though rare, should be considered in the differential diagnosis of Cystic lesions in every anatomic location, especially when they occur in endemic areas. We overview the diagnosis and treatment of this parasitic infection.
